herbal medicines are the synthesis of therapeutic experiences of generations of practising physicians of indigenous systems of medicine for over hundreds of years while nutraceuticals are nutritionally or medicinally enhanced foods with health benefits of recent origin and marketed in developed countries. The marketing of the former under the category of the latter is unethical. herbal medicines are also in great demand in the developed world for primary health care because of their efficacy, safety and lesser side effects. They also offer therapeutics for age-related disorders like memory loss, osteoporosis, immune disorders, etc. for which no modern medicine is available. India despite its rich traditional knowledge, heritage of herbal medicines and large biodiversity has a dismal share of the world market due to export of crude extracts and drugs. WHO too has not systematically evaluated traditional medicines despite the fact that it is used for primary health care by about 80% of the world population. However, in 1991 WHO developed guidelines for the assessment of herbal medicine. Suggestions for herbal medicine standardization are outlined. The scenario and perceptions of herbal medicine are discussed.

Purpose of ReviewTo set the context of current knowledge regarding the role of herbal medicine in acute and chronic rhinosinusitis *** FindingsIt is estimated that adults experience 1-3 episodes of viral rhinosinusitis per year and this number increases up to 8-10 episodes in preschool children. The symptoms of acute rhinosinusitis tend to significantly overlap with symptoms of other upper respiratory infections, making the diagnosis quite difficult. The division of rhinosinusitis into bacterial or non-bacterial is clinically important in order to determine appropriate treatment and the administration of antibiotic treatment. Treatment of acute rhinosinusitis is symptomatic and includes nasal rinsing, decongestants, corticosteroids, and combinations of the above. herbal medicine has been traditionally underestimated in Western world. Nowadays, however, treatment of diseases with the use of medicinal plant treatments is gaining more and more followers. In this context, certain herbal extracts have been tested for viral, post- viral and chronic *** is an innovative pharmaceutical technique in research and production of herbal medicines. herbal extracts produced with phytoneering vary in quality and active substances. In terms of quality, safety and efficacy, herbal medicines are at least on par with synthetically produced medicines, having significantly less unwanted side effects. Certain herbal extracts have been tested, and in cases of acute rhinosinusitis are effective. In chronic sinusitis those extracts show promising results and might prove a good alternative without side effects.

Background: Psoriasis, a chronic skin condition influenced by both genetic and environmental factors, affects approximately 2 % of the global population. Treatment approaches range from topical agents for mild to moderate cases to systemic therapies for moderate to severe forms. Complementary and integrative medicine, including herbal remedies, is increasingly utilized by psoriasis patients, with herbal medicine being the most prevalent choice. Methods: This systematic review adhered to PRISMA guidelines and Cochrane recommendations, with prior registration. Searches were conducted in Medline/PubMed, Scopus, and the Cochrane Central Register of Controlled Trials through June 2024. Inclusion criteria encompassed randomized controlled trials (RCTs) evaluating herbal therapies in psoriasis patients of any age. Data extraction and risk of bias assessment were performed independently by two authors, with discrepancies resolved through discussion. Results: A total of 20 RCTs involving 1115 patients were included. Overall risk of bias varied across studies. herbal interventions evaluated included Aloe vera, avocado oil, chamomile-pumpkin seed oleogel, indigo naturalis, kukui oil, mahonia aquifolium, turmeric, St. John's wort, and various complex herbal preparations. Some herbal therapies, such as Mahonia aquifolium and Indigo naturalis, showed promise in reducing psoriasis severity, while others demonstrated mixed results. Conclusions: herbal therapies present a diverse array of options for psoriasis management, but further research is essential to elucidate their safety profile and true effectiveness. Healthcare providers should engage in shared decision-making with patients, considering individual preferences and treatment goals while remaining vigilant in guiding towards evidence-based treatments.

herbal medicine and non-hospital utilisation in Malaysia during the COVID-19 pandemic

Background: COVID-19 is an infectious disease that led to over 3 million deaths globally during the pandemic. herbal medicine (HM) has become popular during the COVID-19 pandemic;however, it is unclear whether this reliance on HM has affected conventional medical practices, including hospital utilisation. Objective: The study’s objective was to examine the usage of HM from January 2021 to December 2021 amidst the COVID-19 pandemic and its association with non-hospital utilisation among Malaysians. Method: A cross-sectional study was conducted on the Malaysian population aged 18 years and older. Findings: 393 respondents from diverse ethno-religious backgrounds (Malays 26.5%, Chinese 34.1%, Indians 34.6%, Indigenous peoples 3.3% and others 15%) participated in the study, with 59% being females (n = 232). Most respondents (n = 286, 72.8%) reported fear of visiting hospitals during the pandemic. As a result, 51.4% (n = 202) turned to HM as an alternative due to its availability (n = 38, 18.8%) and cost-effectiveness (n = 36, 17.8%). Most consumers (n = 133, 33.8%) preferred HM certified by the Ministry of Health (MOH), Malaysia and purchased them from pharmacy retailers (n = 114, 29%). A significant association (P < 0.05) between HM consumption and non-hospital utilisation was found. Nearly half of the respondents (n = 186, 47.3%) alleviated the severity of their symptoms. Logistic regression revealed that HM users were 1.96 times more likely not to utilise hospital than non-HM user. Conclusion: Overall, HM consumption during the pandemic was based on personal knowledge and was significantly associated with non-hospital utilisation. Fear of COVID-19 was one of several contributing factors to this trend. © The Author(s) 2025.

Chinese herbal medicine has offered a great treasure for discovering intrinsically bioactive low molecular weight gelators (LMWGs). Herein, the two-component hydrogels comprising glycyrrhizic acid (GA) and puerarin (PUE), the primary bioactive components, respectively, from herbs Glycyrrhiza uralensis Fisch and Pueraria lobata are successfully prepared. Combined spectroscopic characterizations reveal that hydrogen bonds are formed between GA and PUE molecules, which further drives the growth of nanofiber assemblies into gel networks. Importantly, micromorphological observation by scanning electron microscopy (SEM), synchrotron small-angle X-ray scattering (SAXS), and molecular dynamic simulation suggest that a coassembly pathway is involved in the gelling process. Such two-component hydrogels exhibit good injectable, self-healing, and adhesive properties. Interestingly, the mixed GA-PUE hydrogels demonstrate a more efficient and selective antibacterial activity toward S. aureus instead of E. coli, and a PUE ratio-dependent antibacterial activity toward S. aureus is also observed. Our work highlights that CHM-derived LMWGs can provide a scaffold for developing multicomponent hydrogels, which may afford novel and distinct properties compared with their individual ones. It is assumed that more multicomponent supramolecular hydrogels derived from CHM would appear to better address the challenges, particularly in the biomedical field.

Chinese herbal medicine for obstructive sleep apnoea: a systematic review with meta-analysis

Purpose Chinese herbal medicine (CHM) may have a place in the treatment of obstructive sleep apnoea (OSA), but the evidence has not been systematically assessed. The objective of this study is to evaluate the evidence for the effectiveness of CHM for OSA. Methods CENTRAL, EMBase, PubMed, CINAHL, CNKI, Wanfang and Chongqing VIP were searched from database inception to the 13th of August 2023. Randomised-controlled trials in which the effect of CHM on sleep variables was tested in the treatment of OSA were selected. The primary outcomes of this review and meta-analysis were the apnoea hypopnoea index (AHI) measured with polysomnography. Results A total of 58 studies, involving 4,590 participants, were included. The meta-analysis showed that CHM reduced AHI more significantly than placebo (mean difference = -7.10 events/hour, 95% CI = -11.95, -2.25, P < 0.01, 7 studies, 583 participants) and the combination of CHM and Continuous Positive Airway Pressure (CPAP) more effective than CPAP alone (mean difference = -4.71 events/hour, 95% CI = -5.62, -3.80, P < 0.001, 28 studies, 2,267 participants). CHM improved sleepiness, quality of life, body weight, oxidative biomarkers, inflammatory biomarkers, cognitive function, blood pressure and respiratory function. Conclusion Our results suggest that CHM, alone and as an adjunct treatment, can improve various aspects of OSA and its comorbidities and is therefore a viable therapeutic option for OSA.

Methodological challenges in pilot trials of herbal medicine: barriers to evidence-based practice

Objectives: The growing popularity of herbal medicine (HM) underscores the need for high quality clinical trials to support its evidence-based integration. Pilot trials are essential for addressing methodological challenges in this field. This study evaluates the design quality, feasibility, and reporting of HM pilot trials, with a focus on their capacity to inform future full-scale studies. Study Design and Setting: A comprehensive collection of HM pilot trials was conducted using PubMed, Web of Science, and Em-base, based on predefined inclusion criteria. Data were extracted on trial characteristics, reporting quality, and progression to full-scale studies. To gather additional information on follow-up studies, authors of selected trials were contacted directly by email. Adherence to Consolidated Standards of Reporting Trials (CONSORT) guidelines for pilot trials was evaluated, and Poisson regression was applied to identify factors influencing reporting completeness. Results: A total of 123 HM pilot trials were reviewed, predominantly from Asia (78.1%). Trials most commonly addressed respiratory (14.6%), nervous (14.6%), and reproductive systems (13.0%). Key gaps in reporting included feasibility assessments (13.1%), sample size rationale (47.2%), and randomization methods (35.8%). HM-specific details, including ingredient processing, quality control, and safety assessments, were inconsistently reported. Among the trials, 4 (3.3%) progressed to full-scale studies. Factors such as trial registration (incidence rate ratio (IRR) = 1.20, 95% CI: 1.11-1.30) and protocol publication (IRR = 1.16, 95% CI: 1.08-1.24) were positively associated with reporting completeness. Moreover, an analysis of the origin of HMs revealed that modern HM trials were 4.7 times more likely to progress to full-scale studies compared to traditional HM trials (odds ratio = 4.70, 95% CI: 0.37-252.91), although the result did not reach statistical significance (P = .300). Sunlight is composed of various wavelengths, including visible light, ultraviolet (UV) rays, and infrared radiation that serves as a double-edged sword for humans via providing the energy for sustaining life on Earth and also acting as a source of hazardous UV radiation. The skin, as the largest protective part of the body, is exposed to sunlight daily, making it critical to protect this organ from its harmful effects. Accordingly, this research aims to fabricate a new type of light-responsive liposome to deliver herbal medicine as protective compounds with antioxidant and anticancer properties. The light-responsive part of this liposome has the capability of cleavage after exposure to UV-A light (the main UV-parts of sunlight) and improves drug release pattern. In detail, a light-responsive compound was fabricated at first and then was used along with phospholipids and curcumin (a type of herbal drug)-loaded cyclodextrin for the fabrication of liposomes using the thin-film hydration method. The physicochemical analysis confirmed the fabrication of spherical liposomes approximately 145 nm in size, which released around 62% of the therapeutic cargo over 120 h when exposed to UV irradiation. Besides, it showed anticancer ability (against melanoma cancer cells) while having a protecting effect for the normal cell line. Therefore, it could be a candidate for further application in skin-protecting products like wound healing compounds or anticancer usage.

The prevalence of herbal medicine use and related factors among diabetic patients in Tabriz, Iran, 2023: a cross-sectional study

Objectives This study was designed to characterize the prevalence, pattern of herbal use, and related factors among diabetic patients in Tabriz, Iran. Methods A descriptive cross-sectional study was carried out on 322 diabetic patients with random cluster sampling of specialized and subspecialized clinics in Tabriz, Iran. Binary logistic regression analysis was performed to evaluate the association between predictor variables (sociodemographic and disease-related characteristics and patient preference for treatment type) with herb use Interviews were conducted using a structured questionnaire from October 1, 2022, to April 23, 2023. Data was analyzed using SPSS software. Results Out of 322 patients, 155 individuals (48.1%) used herbal medicines to lower their blood sugar. The most commonly used herbs included Urtica dioica L. (51.6%), Abelmoschus esculentus L. (14.8%), Eryngium planum L. (11%), Trigonella foenum-graecum L. (9%) and Cinnamomum verum (9%). The chance of herb use in women was 1.64 times (adjusted odds ratio (AOR) = 1.64, 95% confidence interval (CI) = 1.06 to 2.55) higher than men. With each passing year of diabetes diagnosis, the probability of using herbal medicines increased by 5% (AOR = 1.05, 95% CI = 1.01 to 1.08). Diabetic patients who use herbal medicines significantly prefer to use their conventional medications alongside herbal remedies (P-value < 0.001) and only a small percentage of them prefer to rely solely on herbs. Conclusions Some of the diabetic patients in Tabriz, Iran, use herbal medicines for managing diabetes. Female gender, longer duration of diabetes, and patients who prefer to use both treatment (conventional and traditional) methods were identified as predicting factors for herbal medicinal use among diabetic patients.
