BackgroundThe Medical Information Database Network (MID-NET (R)) in Japan is a vast repository providing an essential pharmacovigilance tool. Gastrointestinal perforation (GIP) is a critical adverse drug event, yet no well-established GIP identification algorithm exists in MID-NET (R). MethodsThis study evaluated 12 identification algorithms by combining ICD-10 codes with GIP therapeutic procedures. Two sites contributed 200 inpatients with GIP-suggestive ICD-10 codes (100 inpatients each), while a third site contributed 165 inpatients with GIP-suggestive ICD-10 codes and antimicrobial prescriptions. The positive predictive values (PPVs) of the algorithms were determined, and the relative sensitivity (rSn) among the 165 inpatients at the third institution was evaluated. ResultsA trade-off between PPV and rSn was observed. For instance, ICD-10 code-based definitions yielded PPVs of 59.5%, whereas ICD-10 codes with CT scan and antimicrobial information gave PPVs of 56.0% and an rSn of 97.0%, and ICD-10 codes with CT scan and antimicrobial information as well as three types of operation codes produced PPVs of 84.2% and an rSn of 24.2%. The same algorithms produced statistically significant differences in PPVs among the three institutions. Combining diagnostic and procedure codes improved the PPVs. The algorithm combining ICD-10 codes with CT scan and antimicrobial information and 80 different operation codes offered the optimal balance (PPV: 61.6%, rSn: 92.4%). ConclusionThis study developed valuable GIP identification algorithms for MID-NET (R), revealing the trade-offs between accuracy and sensitivity. The algorithm with the most reasonable balance was determined. These findings enhance pharmacovigilance efforts and facilitate further research to optimize adverse event detection algorithms.

Determining Palliative Care Penetration Rates in the Acute Care Setting

Context. Intermountain Healthcare, in collaboration with Cerner Corporation, developed a hospital-based electronic palliative care algorithm. Objectives. This study aims to improve identification of patients who would benefit from palliative care services, and calculate palliative care penetration rates. Methods. This study used a mixed-methods nonrandomized retrospective study design. Three 30-day iterations of clinical data were analyzed for patients identified by the electronic algorithm. During the second and third 30-day iterations, palliative care clinicians conducted chart reviews on a weekly basis for identified patients and determined whether the patients were appropriate for a palliative care consult. Positive predictive values (PPVs) were calculated. Based on the PPV, palliative care consult penetration rates were also calculated. Results. During the first iteration, the algorithm triggered 2995 times on 1384 unique patient encounters (69.3% of the total inpatient population). In the second iteration, the algorithm triggered 851 times on 477 unique patient encounters (26.4% of the total inpatient population). Eight hundred twenty-one chart reviews were completed on 420 unique patient encounters. The PPV was 68.3%. Based on the PPV, the projected palliative care penetration rate was 17.6%. During the third iteration, the algorithm triggered 1229 times on 539 unique patient encounters (33.3% of the total inpatient population). Nine hundred sixty-seven chart reviews were completed on 505 unique patient encounters. The PPV was 80.1%. Based on the PPV, the projected palliative care penetration rate was 26.4%. Conclusion. This study successfully optimized an electronic palliative care identification algorithm with a PPV of 80.1% and indicates appropriate palliative care penetration rates may be as high as 26.4% of the total inpatient population. In this paper, a new isothermal tank test circuit composed of two tanks and a new data-processing algorithm called Cbm method are proposed to measure the flow-rate characteristics of some pneumatic valves. The double-tank circuit is used to test some solenoid valves such as inner pilot solenoid, which the traditional isothermal discharge method can not apply to. And Cbm method changes the identification procedure of multivariable unconstrained optimal algorithm, which evidently improves the identification precision of critical pressure ratio and subsonic index.

Individual differences in functional connectivity during naturalistic viewing conditions

Naturalistic viewing paradigms such as movies have been shown to reduce participant head motion and improve arousal during fMRI scanning relative to task-free rest, and have been used to study both functional connectivity and stimulus-evoked BOLD-signal changes. These task-based hemodynamic changes are synchronized across subjects and involve large areas of the cortex, and it is unclear whether individual differences in functional connectivity are enhanced or diminished under such naturalistic conditions. This work first aims to characterize variability in BOLD-signal based functional connectivity (FC) across 2 distinct movie conditions and eyes-open rest (n=31 healthy adults, 2 scan sessions each). We found that movies have higher within- and between-subject correlations in cluster-wise FC relative to rest. The anatomical distribution of inter-individual variability was similar across conditions, with higher variability occurring at the lateral prefrontal lobes and temporoparietal junctions. Second, we used an unsupervised test-retest matching algorithm that identifies individual subjects from within a group based on FC patterns, quantifying the accuracy of the algorithm across the three conditions. The movies and resting state all enabled identification of individual subjects based on FC matrices, with accuracies between 61% and 100%. Overall, pairings involving movies outperformed rest, and the social, fasterpaced movie attained 100% accuracy. When the parcellation resolution, scan duration, and number of edges used were increased, accuracies improved across conditions, and the pattern of movies > rest was preserved. These results suggest that using dynamic stimuli such as movies enhances the detection of FC patterns that are unique at the individual level.

Developing automation solutions that enable remote communications, monitoring and control for subsea applications are key steps in designing subsea power distribution systems. These systems require fast local control to protect the multiple electrical loads and the capability of transferring prompt real-time trip signals. This study introduces a data-driven distributed fault detection and identification algorithm to monitor multiple subsea loads. The proposed scheme is divided into three steps. First, a stochastic hidden-Markov model (HMM) is developed to model the dynamic evolution of different potential conditions of multiple subsea loads. Simultaneously, the second step computes a model of the transition probability between the current operating condition and the potential response of an individual load. In the third step, using real-time measurements, the HMM is updated to predict an unobserved degradation of the load's current condition. This is achieved through an integrated perturbation analysis and sequential quadratic programming method. An assessment of case studies on subsea AC power system is presented, which includes different subsea motor loads for compressors and pumps. Results show robustness against uncertainty in measurement noise and changes in equipment mean time between failures, providing enhanced reliability.

Dynamic analysis of planar elastic mechanisms using the dyad method

Based on graph representation of planar linkages, a new algorithm has been developed to identify the different dyads of a mechanism. A dyad, or class 11 group, is composed of two binary links connected by either a revolute (1) or a slider (0) pair, with provision for attachment of other links by lower pair connectors located at the end of each link. There are five types of dyad: D111, D101, D011, D001 and D010. The dyad analysis of a mechanism is predicated on the ability to construct the system from one or more of the five binary structure groups or class 11 groups. If the mechanism is complicated and several dyads are involved, the task of identifying these dyads, by inspection, can be difficult and time consuming for the user. This algorithm allows complete automation of this task. It is based on Dijkstra's algorithm for finding the shortest path in a graph. When compared with algorithmic methods, such as the Newton-Raphson method, the dyad method proved to be a very efficient one and requires as little as one-tenth of the time needed by the method using the Newton-Raphson algorithm. The second part of this work presents an extension of the dyad method to non-rigid or elastic mechanisms. Here also, this method is predicated on the ability to subdivide the elastic mechanism into elastic dyads. The solution for each type of elastic dyad is derived and can be applied to each dyad in the mechanism. Therefore, a solution of the complete elastic mechanism is possible when the mechanism is made of dyads only. This method makes a powerful and simple toot for analysing complex elastic mechanisms. Moreover, the complexity of the model does not increase as the mechanism becomes more complex. The D111 dyad is taken as an example to demonstrate this method. A finite element (FE) analysis was made for this type of dyad, and an experimental set-up was built to validate the analysis. The dyad-FE results were in good agreement with the experimental ones.

This paper introduces a method extending possibilities of the relay feedback identification of dynamic systems describable by their transfer function. The proposed "shifting" method enables to estimate three points on a plant frequency response from a single relay feedback test without using the FFT algorithm. The approach enables to estimate up to five parameters of the transfer function from sustained oscillations obtained by one relay test. For this purpose the method uses a biased relay. The introduced technique offers a tool for a large class of common control problems. The applicability of the proposed frequency-response identification method is demonstrated by estimating five parameters of the anisochronic model and by estimating three points on the frequency response of the physical model called "Air Aggregate". In this paper, a recursive stochastic gradient algorithm based on two-step update estimation and an iterative stochastic gradient algorithm based on two-step update estimation are established for the Wiener system by introducing a relaxation factor, which controls the relative importance of the two estimation parts. In addition, the convergence performance of the proposed SG-TSU and ISG-TSU algorithms are then analyzed. It is shown by a numerical example that if the relaxation factor is appropriately chosen, the proposed SG-TSU and ISG-TSU algorithms converge more quickly and have higher convergence precision than the standard SG and ISG algorithms, respectively.

Aiming at the disturbance characteristic identification problem caused by the spacecraft's active payload,this paper proposes a ground test equipment based on the air bearing *** overall design scheme,control principle and control effect of the system are expounded in this *** paper identifies the quality and moment of inertia of the air bearing robot based on recursive least squares ***,set up the system dynamics equations and momentum conservation equation to calculate the pose information of the ***,use filtering algorithm to filter the *** disturbance characteristics can be calculated by identification ***,build a simulation program to evaluate the efficiency of the algorithm,and the results show that the identification accuracy of the algorithm is better than 95%.

Designing machine tools is an extremely complicated process, requiring thorough knowledge from the constructors in terms of statics, dynamics, contact mechanics, etc. Therefore, it is necessary to support this process by analyzing their models. However, only on the basis of experimentally identified models, reliable information about the object can be obtained. Presented paper contains numerical studies of the efficiency of optimization algorithms to identify parameters of FEM models. Numerical studies were carried out for the FEM model of the hybrid machine tool body structural component, consisting of steel and polymer concrete. In order to identify the parameters of the model, an interface was developed to enable data exchange between the FEM preprocessor of the Nastran FX and Matlab to cooperate. Numerical studies were then conducted using a developed identification algorithm based on accelerance functions, developed on the basis of reduced order FEM model and introduced correction parameters.
