Granular cell tumor (GCT) is a rare clinical entity derived from Schwann cells of the outer sheath of the peripheral nerves, which commonly affects the skin and subcutaneous tissue. Specifically, GCT of the vulva is rare and usually occurs in the labia majora. The prevalence and etiology of GCT remain unknown; however, a risk for malignancy has been reported at 2%. We present a case study of 52-year-old postmenopausal woman with a slowly enlarging vulvar mass over 2 years without associated symptoms. On physical examination, a nodular subcutaneous mass was observed on mons pubis and pelvic magnetic resonance imaging revealed an irregularly bordered solid mass in this area. An aspiration cytology was performed, suggesting a diagnosis of GCT. After excision, histopathological analysis revealed no malignant characteristics and negative surgical margins were reported. This case report describes a rare presentation of GCT involving the mons pubis of the vulva, which has been previously reported in only three cases. Given its large size, a more aggressive surgical treatment combined with multidisciplinary management was necessary. During follow-up care, vulva and other anatomical locations should be closely monitored to early diagnosis of local or extragenital recurrence.
