1 INTRODUCTION
Trochleitis usually presents as an orbital pain without obvious ocular signs. Diagnosis is confirmed by peritrochlear steroid injection, which produces quick relief of periocular symptoms and may improve headache control. Trochleitis is a local inflammatory process of the superior oblique tendon trochlea and is character- ized by severe periocular pain. Eye movement in supraduction typically aggravates the pain. Idiopathic trochleitis is a cause of superimposed ocular pain in patients with migraine. Physical examination shows exquisite point tenderness over trochlea of the superior oblique muscle.
