目的:为通过糖尿病健康宣教效果,进行健康饮食、适量运动、心理支持、血糖自我监测和药物治疗多方面的健康知识宣教,实现对糖尿病患者在日常生活中进行疾病院外延续护理。方法:利用采取将传统的全程健康宣教与智慧问答系统相结合,建设基于微信小程序糖尿病健康宣教问答系统,同时调查患者满意度,系统自动构建宣教闭环,实现数据可追溯、可监控、可评测。结果:提高控制血糖的意识以及了解控制血糖的重要性,培养偏远地区或难再次入院就医糖尿病患者解决问题和缓解病情的能力,提高患者用药的依从性,改善生活质量,提高幸福指数。结论:及时更改糖尿病护理诊疗健康宣教系统中展示给糖尿病患者的自我管理方案,对于后续针对糖尿病诊治和护理工作中提供丰富和宝贵的经验,推动糖尿病有效控制的进度,也为其他疾病的诊疗护理向信息化转变提供制定标准。Objective: Through health education on diabetes, health knowledge education on healthy diet, proper exercise, psychological support, blood glucose self-monitoring and drug treatment was carried out, so as to realize continuous care of diabetes patients outside the hospital in their daily lives. Methods: Utilizes a combination of traditional full process health education and intelligent Q&A systems to push health education content to patient phones and collect patient satisfaction. The system automatically constructs a closed loop for education, achieving data traceability, monitoring, and evaluation. Results: Improve the awareness of blood sugar control and understand the importance of blood sugar control, cultivate the ability of diabetes patients in remote areas or difficult to readmission to the hospital to solve problems and ease the condition, improve the compliance of patients with medication, improve the quality of life, and improve the happiness index. Conclusion: Timely changes in the self-management plan displayed to diabetes patients in the diabetes care diagnosis and treatment health education system, provide rich and valuable experience for the follow-up diabetes diagnosis and treatment and nursing work, promote the progress of effective control of diabetes, and also provide standards for the transformation of diagnosis and treatment of other diseases to informatization.
