This article provides a detailed analysis of the physiological, psychological, and environmental risk factors associated with falls in older adults. It also explores assessment methods for these risks, including an analysis of the effectiveness and reliability of commonly used assessment tools and techniques. Based on the assessment results, this study proposes individualized preventive nursing interventions and emphasizes the importance of a multidisciplinary collaborative model in the prevention and management of fall risks among the elderly. Research indicates that through comprehensive assessment and targeted nursing interventions, the incidence of falls in older adults can be significantly reduced, contributing to an improvement in their quality of life and sense of security. The findings of this study are of significant practical importance for improving the risk assessment system for elderly falls and developing effective nursing intervention strategies.
