目的探讨联合早期肠内营养(EEN)和生态免疫肠内营养(EIN)对重症急性胰腺炎(SAP)大鼠肾功能保护作用。方法选取健康雄性SD大鼠120只随机分为4组:正常对照(SO)组,SAP模型(SAP)组,EEN组,EIN组。将每组分为12、24、48 h 3个亚组(n=10)。除SO组外,其余各组采用逆行胰胆管注射5%牛磺胆酸钠法制备SAP模型。分别于营养支持后各时间点处死大鼠,采集大鼠腹主动脉血检测血清淀粉酶(AMY)、血尿素氮(BUN)、血肌酐(Cr)水平;采用酶联免疫吸附试验(ELISA)检测血清中肿瘤坏死因子(TNF)-α、白细胞介素(IL)-6、IL-10水平;取胰腺和肾脏组织进行苏木素-伊红(HE)染色,观察组织病理改变。结果EIN组制模后各时间点AMY、BUN和Cr水平显著低于同期SAP组、EEN组(P<0.05);各时间点EIN组与同一时间点SAP组、EEN组相比血清TNF-α、IL-6水平显著下降,IL-10水平明显升高(P<0.05)。结论联合EEN和EIN可明显改善SAP大鼠胰腺及肾脏组织病理损伤,具有肾功能保护作用。
