Predictors of sustained research involvement among md/phd programme graduates

Contextmd/phd programmes provide structured paths for physician-scientist training. However, considerable proportions of graduates of these programmes do not pursue careers in research consistent with their training. ObjectivesWe sought to identify factors associated with sustained involvement in research after completion of all postgraduate training. MethodsAnonymised data from a national survey of Canadian md/phd programme graduates who had completed all physician-scientist training (n = 70) were analysed. Multivariable logistic regression was used to measure the associations between characteristics of graduates and five indicators of sustained research involvement following postgraduate training: (i) protected research time in the current appointment;(ii) percentage of time dedicated to research;(iii) planned future involvement in research;(iv) role as a principal investigator on a recent funded project, and (v) receipt of funding from a federal granting agency since graduation. ResultsThe majority of graduates were significantly involved in research on the basis of at least one outcome. Completion of a research fellowship, number of first-authored or co-authored manuscripts published during md/phd training, and duration of md/phd training were positively associated with continued research involvement. Completion of a Masters degree prior to md/phd training, female gender, debt greater than CAD$50 000 at completion of training, and pursuit of a clinical specialty other than internal medicine, paediatrics, neurology, pathology and the surgical specialties were negatively associated with sustained research involvement. ConclusionsMost md/phd programme graduates remain significantly involved in research, but this involvement often does not correspond to traditional physician-scientist roles, in which a majority of time is dedicated to research. The N protein of phage Mu was indicated from studies in Escherichia coil to hold linear Mu chromosomes in a circular conformation by non-covalent association, and thus suggested potentially to bind DNA double-stranded ends. Because of its role in association with linear Mu DNA, we tested whether fluorescent-protein fusions to N might provide a useful tool for labeling DNA damage including double-strand break (DSB) ends in single cells. We compared N-GFP with a biochemically well documented DSB-end binding protein, the Gam protein of phage Mu, also fused to GFP. We find that N-GFP produced in live E. coif forms foci in response to DNA damage induced by radiomimetic drug phleomycin, indicating that it labels damaged DNA. N-GFP also labels specific DSBs created enzymatically by I-Scel double-strand endonuclease, and by X-rays, with the numbers of foci corresponding with the numbers of DSBs generated, indicating DSB labeling. However, whereas N-GFP forms about half as many foci as GamGFP with phleomycin, its labeling of I-SceI-and X-ray-induced DSBs is far less efficient than that of GamGFP. The data imply that N-GFP binds and labels DNA damage including DSBs, but may additionally label phleomycin-induced non-DSB damage, with which DSB-specific GamGFP does not interact. The data indicate that N-GFP labels DNA damage, and may be useful for general, not DSB-specific, DNA-damage detection.

The capacity of the cerebrovasculature to buffer changes in blood pressure (BP) is crucial to prevent stroke, the incidence of which is three- to fourfold elevated after spinal cord injury (SCI). Disruption of descending sympathetic pathways within the spinal cord due to cervical SCI may result in impaired cerebrovascular buffering. Only linear analyses of cerebrovascular buffering of BP, such as transfer function, have been used in SO research. This approach does not account for inherent nonlinearity and nonstationarity components of cerebrovascular regulation, often depends on perturbations of BP to increase the statistical power, and does not account for the influence of arterial CO2 tension. Here, we used a nonlinear and nonstationary analysis approach termed wavelet decomposition analysis (WDA), which recently identified novel sympathetic influences on cerebrovascular buffering of BP occurring in the ultra-low-frequency range (1.11,F;0.02-0.03Hz). WDA does not require BP perturbations and can account for influences of CO2 tension. Supine resting beat-by-beat BP (Finometer), middle cerebral artery blood velocity (transcranial Doppler). and end-tidal CO2 tension were recorded in cervical SCI (n = 14) and uninjured (n = 16) individuals. WDA revealed that cerebral blood flow more closely follows changes in BP in the ULF range (P = 0.0021, Cohen's d = 0.89), which may be interpreted as an impairment in cerebrovascular buffering of BP. This persisted after accounting for CO2. Transfer function metrics were not different in the ULF range, but phase was reduced at 0.07-0.2 Hz (P = 0.03, Cohen's d = 0.31). Sympathetically mediated cerebrovascular buffering of BP is impaired after SCI. and WDA is a powerful strategy for evaluating cerebrovascular buffering in clinical populations.

INTRODUCTION:Many individuals with high-level spinal cord injury (SCI) experience secondary conditions such as autonomic dysreflexia (AD), which is a potentially life-threatening condition comprising transient episodes of hypertension up to 300 mmHg. AD may be accompanied by symptoms and signs such as headache, flushing, and sweating. Delay in AD recognition and management is associated with increased incidence of cardiovascular events and disease. As it is commonly triggered by bladder distension, AD continues to be a major concern for individuals living with SCI, both on a daily basis and in the long-term. CASE PRESENTATION:A 58-year-old woman with C3 AIS B SCI presented with low resting blood pressure (BP) at 100/64 mmHg. She reported frequent episodes of AD that were most commonly attributed to urinary bladder filling. During our testing session, her systolic BP rose to 130 mmHg, at which point her care aide stepped in to utilize the Credé maneuver, which was part of her daily routine for bladder emptying. Application of suprapubic pressure further elevated her systolic BP to 230 mmHg. Throughout the episode of AD, the participant experienced a pounding headache and erythema above the LOI. DISCUSSION:Clinical guidelines for bladder management after SCI recommend avoiding the Credé maneuver due to potential complications such as hernia or bruising. This current case report demonstrates the additional risk of inducing AD and dangerously high BP elevation.

The purpose of this study was to examine the consistency of measurements of maximal isometric force production in children using an electronic strain gauge. A standardized protocol of force testing from the Tufts Quantitative Neuromuscular Examination was adapted to obtain isometric force measurements in seven muscle groups bilaterally. Ten children with Duchenne's muscular dystrophy (Dmd) and 10 age-matched children without Dmd were recruited for the study. A generalizability framework was used to examine variations in measurements attributable to group differences, test occasions within the same day, and extremity side. Intertester reliability estimates were also completed on a subsample of children with and without Dmd. A three-way analysis of variance between groups, sides, and test occasions demonstrated a significant group main effect for all muscle groups. In general, significant main effects were not present for side or test occasion. Test-retest intraclass correlation coefficients (ICCs) for all muscle groups ranged from .88 to .99 for the children with Dmd and from .85 to .98 for the children without Dmd. Intertester ICCs were slightly lower, ranging from .74 to .97 for the children with Dmd and from .71 to .98 for the children without Dmd. The results indicate that the electronic strain-gauge protocol produces reproducible force measurements in seven selected muscle groups in children with and without Dmd.
