Introduction:Mongolia's health ministry prioritizes control of Sexually Transmitted Infections,including syphilis screening and treatment in antenatal care(ANC).Methods:Adult syphilis prevalence trends were fitted using the Spectrum-STI estimation tool,using data from ANC surveys and routine screening over *** were combined with programmatic data to estimate numbers of treated and untreated pregnant women with syphilis and associated incidence congenital syphilis(CS)and CS-attributable adverse birth outcomes(ABO),which we compared with CS case ***:Syphilis prevalence in pregnant women was estimated at 1.7%in 2000 and 3.0%in *** estimated 652 CS cases,of which 410 ABO,in *** larger,annually increasing numbers of CS cases and ABO were estimated to have been prevented:1654 cases,of which 789 ABO in 2016thanks to increasing coverages of ANC(99%in 2016),ANC-based screening(97%in 2016)and treatment of women diagnosed(81%in 2016).The 42 CS cases reported nationally over 2016(liveborn infants only)represented 27%of liveborn infants with clinical CS,but only 7%of estimated CS cases among women found syphilis-infected in ANC,and 6%of all estimated CS cases including those born to women with undiagnosed ***/Conclusion:Mongolia's ANC-based syphilis screening program is reducing CS,but maternal prevalence remains *** eliminate CS(target:<50 cases per 100,000 live births),Mongolia should strengthen ANC services,limiting losses during referral for treatment,and under-diagnosis of CS including still-births and neonatal deaths,and expand syphilis screening and prevention programs.

Trends and Associations with PrEP Prescription Among 602 New York City (NYC) Ambulatory Care Practices, 2014–2016

Background Concern over equitable access to HIV preexposure prophylaxis (PrEP) informs comprehensive scale-up efforts in New York City (NYC). We examined trends plus patient and practice factors associated with PrEP prescribing by NYC ambulatory care practices. Methods We queried electronic health records (EHR) from Q1 2014 to Q2 2016 using the NYC Health Department’s “Hub.” Data from 602 practices were aggregated quarterly by patient factors, including age (18–29, 30–100); sex (male, female); and race/ethnicity (Asian, Black, Hispanic, White, other, missing). Practice factors included location (Manhattan, other); type (community health center [CHC], hospital, independent); number of infectious disease (ID) specialists; and proportion of patients (ranked by quartile) from high poverty neighborhoods (ZIP codes in which ≥20% of residents live below the federal poverty level). PrEP prescription was defined as tenofovir/emtricitabine prescription without other antiretrovirals or diagnoses of HIV, HIV-related opportunistic infections or hepatitis B. Rates were calculated per 10 5 patients seen. We used generalized estimating equations clustered by practice to examine trends overall and by sex, as well as associations among males. Factors and time interactions that were significant ( P < 0.05) in bivariate analysis were assessed for inclusion in the final model. Results Overall, PrEP prescription rose from 38.9 per 10 5 in Q1 2014 to 418.5 per 10 5 in Q2 2016, a 976% increase. Increases were significant for both sexes ( P < .0001; Figure 1). In multivariate analysis (Table 1), PrEP prescription was associated with both patient (younger age, white race/ethnicity) and practice factors (Manhattan location, CHCs, and on-site ID specialists). While practices with a greater proportion of patients from high poverty neighborhoods were less likely to prescribe PrEP initially, this association weakened over time (Table 2). Background: High-resolution estimates of HIV burden across space and time provide an important tool for tracking and monitoring the progress of prevention and control efforts and assist with improving the precision and efficiency of targeting efforts. We aimed to assess HIV incidence and HIV mortality for all second-level administrative units across sub-Saharan Africa. Methods: In this modelling study, we developed a framework that used the geographically specific HIV prevalence data collected in seroprevalence surveys and antenatal care clinics to train a model that estimates HIV incidence and mortality among individuals aged 15–49 years. We used a model-based geostatistical framework to estimate HIV prevalence at the second administrative level in 44 countries in sub-Saharan Africa for 2000–18 and sought data on the number of individuals on antiretroviral therapy (ART) by second-level administrative unit. We then modified the Estimation and projection Package (EPP) to use these HIV prevalence and treatment estimates to estimate HIV incidence and mortality by second-level administrative unit. Findings: The estimates suggest substantial variation in HIV incidence and mortality rates both between and within countries in sub-Saharan Africa, with 15 countries having a ten-times or greater difference in estimated HIV incidence between the second-level administrative units with the lowest and highest estimated incidence levels. Across all 44 countries in 2018, HIV incidence ranged from 2·8 (95% uncertainty interval 2·1–3·8) in Mauritania to 1585·9 (1369·4–1824·8) cases per 100 000 people in Lesotho and HIV mortality ranged from 0·8 (0·7–0·9) in Mauritania to 676·5 (513·6–888·0) deaths per 100 000 people in Lesotho. Prevalence and Predictors of Self-Reported Consistent Condom Usage among Male Clients of Female Sex Workers in Tamil Nadu, India

Clients of female sex workers (FSWs) possess a high potential of transmitting HIV and other sexually transmitted infections from high risk FSWs to the general population. Promotion of safer sex practices among the clients is essential to limit the spread of HIV/aids epidemic. The aim of this study is to estimate the prevalence of consistent condom use (CCU) among clients of FSWs and to assess the factors associated with CCU in Tamil Nadu. 146 male respondents were recruited from the hotspots who reportedly had sex with FSWs in exchange for cash at least once in the past one month. Data were analyzed using bivariate and multivariate methods. Overall, 48.6 and 0.8 percent clients consistently used condoms in the past 12 months with FSWs and regular partners, respectively. Logistic regression showed that factors such as education, peers' use of condoms, and alcohol consumption significantly influenced clients' CCU with FSWs. Strategies for safe sex-behaviour are needed among clients of FSWs in order to limit the spread of HIV/aids epidemic in the general population. The role of peer-educators in experience sharing and awareness generation must also be emphasized.

We evaluated the integration of rapid syphilis tests (RSTs) and penicillin treatment kits into routine antenatal clinic (ANC) services in two rural districts in Nyanza Province, Kenya. In February 2011, nurses from 25 clinics were trained in using RSTs and documenting test results and treatment. During March 2011-February 2012, free RSTs and treatment kits were provided to clinics for use during ANC visits. We analyzed ANC registry data from eight clinics during the 12-month periods before and during RST program implementation and compared syphilis testing, diagnosis, and treatment during the two periods. Syphilis testing at first ANC visit increased from 18% (279 of 1,586 attendees) before the intervention to 70% (1,123 of 1,614 attendees) during the intervention (P < 0.001); 35 women (3%) tested positive during the intervention period compared with 1 (<1%) before (P < 0.001). Syphilis treatment was not recorded according to training recommendations; seven clinics identified 28 RST-positive women and recorded 34 treatment kits as used. Individual-level data from three high-volume clinics supported that the intervention did not negatively affect HIV test uptake. Integrating RSTs into rural ANC services increased syphilis testing and detection. Record keeping on treatment of syphilis in RST-positive women remains challenging.
