Hypercholesterolemia is a major risk factor in the development of cardiovascular diseases. Statins have been the primary treatment for reducing LDL cholesterol for years;however, adverse events related to their use have led to the development of alternative therapies, such as ezetimibe, bempedoic acid, and PCSK9 inhibitors, including alirocumab, evolocumab, and inclisiran. The study is aimed at comparing the consumption and costs of lipid-lowering drugs in the first half of 2023 (21,446,011.54 DDD and 10,974,495.47 ) and 2024 (19,493,540.71 DDD and 12,260,634.95 ) to assess the economic impact of new therapies in the management of dyslipidemia and adherence to guidelines. Dispensing data, without access to patient data, were collected from private community pharmacies and hospital and district pharmacies. Drug consumption was measured using the ATC classification system and the defined daily dose (DDD). The study revealed a trend toward a reduction in the use of traditional statin monotherapy and an increase in more recent and combined therapies. Statins showed a significant reduction from 42% in the first half of 2023 to 30% in the first half of 2024. Combined therapies, such as statins with ezetimibe, showed no change in trend, maintaining a 17% share in both periods. New therapies, such as PCSK9 inhibitors, demonstrated an increase from 33% in the first half of 2023 to 40% in the first half of 2024, while bempedoic acid, either as monotherapy or in combination with ezetimibe, remained below 2%. In recent years, the treatment of dyslipidemia has seen the introduction of many therapeutic alternatives. Bempedoic acid, recently introduced and less expensive, when used as an adjunct therapy with statins and ezetimibe, could reduce side effects and improve therapeutic efficacy without immediately resorting to higher-cost injectable drugs such as evolocumab, alirocumab, and inclisiran. Introduction: Prostate cancer and its treatment, particularly androgen deprivation therapy (ADT), can profoundly impact patients' quality of life. The aim of the prospective observational study reported here was to evaluate the effects of ADT on various aspects of quality of life in men with prostate cancer at a community-based hospital in Southern Italy. Methods: Eligible men initiating hormonal therapy were recruited between December 2021 and December 2023. Data were collected at baseline (T-0) and after 3 months (T-1) and 6 months (T-2) of ADT using standardized questionnaires (European Organization for Research and Treatment of Cancer [EORTC] QLQ-C30, EORTC QLQ-PR25) and semi-structured interviews. Results: Of the 52 participants, 43 completed all three assessments. The EORTC QLQ-C30 showed a statistically significant worsening in physical functioning (mean score decrease from 83.8 at T-0 to 76.7 at T-2;p < 0.001), increased fatigue (from 23.7 to 35.2;p < 0.001), and insomnia (from 23.7 to 31.8;p = 0.048) following ADT initiation. The QLQ-PR25 revealed a significant decline in sexual functioning (from 59 to 26.9;p < 0.001) and sexual activity (from 27.3 to 12;p = 0.001). Interviews revealed a significant rise in the number of patients reporting depressed mood. Interviews also highlighted a worsening in body image perception and sexuality, increased feelings of dependence, and challenges in the social and relational spheres. Conclusions: ADT significantly impacts various aspects of quality of life in men with prostate cancer, particularly physical functioning, fatigue, sexual function, body image, and emotional well-being. These results underscore the critical importance of a comprehensive, patient-centered approach that addresses both the physical and psychosocial aspects of care.

Background Despite the growing body of evidence supporting the use Rez & umacr;m in patients with benign prostatic hyperplasia (BPH) and large prostates, comparative studies with the current gold standard in this clinical setting are lacking. Aim To compare the efficacy and safety of Rez & umacr;m and Thulium Laser Enucleation of the Prostate (ThuLEP) in patients with BPH and large prostates. Methods We conducted a multicenter prospective comparative study. Consecutive patients with prostate volume (PV) >= 80 mL undergoing ThuLEP or Rez & umacr;m were included. Patient evaluation was performed at baseline and at 3, 6, and 12 months after surgery. International Prostate Symptom Score (IPSS)-Total was chosen as the primary outcome, while International Index of Erectile Function (IIEF-EF), four-item version of Male Sexual Health Questionnaire (MSHQ-EjD Short Form), and postoperative complications Clavien-Dindo (CD) grade >= III were selected as the secondary outcomes. Results A total of 246 patients were enrolled (126 ThuLEP, 120 Rez & umacr;m). Mean (SD) PV was 106.0 (16.5) mL in the ThuLEP group and 101.4 (14.8) mL in the Rez & umacr;m group (p = 0.55). Mean (SD) IPSS-Total was statistically significantly lower in the ThuLEP group compared to the Rez & umacr;m group at every postoperative time point (p < 0.001);however, the difference between the mean values never reached the Minimal Clinically Important Difference (MCID). IIEF-EF score significantly improved in both groups (p < 0.001), with no significant difference between the two procedures. Significant differences in MSHQ-EjD Function and Bothers in favor of Rez & umacr;m were observed at all postoperative time points (p < 0.001). Bleeding with clots requiring reintervention (CD >= III) for hemostasis and evacuation occurred in 5 (3.96%) men undergoing ThuLEP and 1 (0.83%) man undergoing Rez & umacr;m (p = 0.21). Conclusions Both ThuLEP and Rez & umacr;m significantly improve urinary symptoms in patients with BPH

Objective: To study the relationship between intensity of physical activity (PA) and semen quality in healthy young men. Design: A prospective cohort study with repeated measures for each subject. Patient(s): Healthy high school and university students who did not regularly smoke tobacco, drink alcohol, or take drugs or medicine, with normal body mass index and abdominal circumference. Exposure: The participants underwent urologic visit, fasting blood and semen sampling, and anthropometric measurements, and fi lled in the International Physical Activity Questionnaire, at enrollment and after 4 and 8 months. Duration and frequency of walking, moderate-intensity, and vigorous-intensity activities in the last week were assessed, and a score was computed for total PA. Main Outcome Measure(s): Semen specimens were taken at each visit through masturbation, after 3-5 days of abstinence, and analyzed by an expert urologist. Sperm concentration, total and progressive motility, and proportion of spermatozoa with normal morphology were measured. Linear and generalized linear mixed models with the Poisson family were fi tted to assess the relationships between PA variables and sperm parameters, after adjusting for season, time, and study arm. The shape of the relationship was modeled through restricted cubic spline regression. Result(s): A total of 143 male subjects, aged 18-23 years (median, 20 years), were enrolled. They had a median PA of 1,960 (95% confidence interval, 1,055-3,182) Metabolic Equivalent of Tasks in min/wk. Statistically significant differences were found for total, progressive motility, and percent of cell with normal morphology across categories of total PA;the highest medians of total (47%) and progressive motility (34%) and of the percentage of normal morphology cells (7%) were found for medium PA. Congenital penile curvature is a rare condition identified by an abnormal penile curvature present from birth. Surgical intervention is currently the most effective treatment and is generally deferred until after puberty;however, early correction is recommended by some due to its impact on psychosexual development. This systematic review aims to evaluate the literature on surgical techniques for congenital penile curvature correction, focusing on their efficacy, safety, and patient-reported outcomes. Conducted in accordance with PRISMA guidelines and registered with PROSPERO (ID: CRD42024526737), the review includes 59 studies meeting inclusion criteria. The findings indicate that numerous surgical techniques have been documented over the past decades, predominantly focusing on shortening procedures such as Nesbit, Yachia, tunica albuginea plication, and Essed-Schr & ouml;der. These historically utilized methods remain effective for correcting congenital penile curvature, but they may result in side effects like penile shortening and erectile dysfunction. The review emphasizes the necessity for well-designed studies to better compare the benefits of various surgical techniques.

Sinusitis is a common condition that can lead to various neurological complications due to the spread of infection to the intracranial and orbital regions. Fortunately, the availability of antibiotics has significantly improved the prognosis of sinusitis-associated intracranial complications. As a result, the overall incidence of neurological complications arising from sinusitis remains low. We present a rare case of a 13-year-old male who developed epidural empyema and orbital cellulitis as a complication of acute sinusitis. The patient initially exhibited signs and symptoms of orbital cellulitis, including eyelid swelling, erythema, and pain. Subsequently, the patient's condition worsened, with the development of fever and an intensifying headache. Imaging revealed an epidural empyema, necessitating urgent medical intervention. This case highlights the importance of early recognition and prompt management of sinusitis-related intracranial and orbital complications to prevent potentially life-threatening outcomes.

Aims The last released European guidelines on the management of heart failure (HF) recommend in patients with chronic HF with reduced ejection fraction (HFrEF) a pharmacological approach based on four fundamental drugs to be rapidly implemented and then uptitrated to modify disease progression. The aim of the Optimization of Therapy in the Italian Management of Heart Failure (OPTIMA-HF) registry is to collect data on chronic HF outpatients in different settings of care. In the present analysis, we report the first analysis of the OPTIMA-HF registry, focusing on the real-life use of guideline-directed medical therapy in patients affected by HFrEF. Methods OPTIMA-HF is an observational, cross-sectional, multicentre, real-life Italian registry conducted in two different clinical settings: HF outpatients' clinics of Italian hospitals and community HF outpatients' services. The study comprises a T0 phase-retrospective data collection, in which data of consecutive HF outpatients seen between January and October 2022 were collected;an educational activity phase;and a T1 phase-prospective data collection, in which data of consecutive HF outpatients seen between September 2023 and November 2023 were collected. In the present analysis, we describe the T0 phase focusing on HFrEF drug prescription rates, types, doses, combination therapy, the presence of contraindications and reasons of non-optimized treatment. Results Twenty-nine centres enrolled 2110 HF patients, of which 1390 (65.9%) had HFrEF [69.5 +/- 11.9 years, 76.2% males, 4.1 years since HF diagnosis, median ejection fraction (EF) 33%]. Among HFrEF patients, 89.1% were on treatment with renin-angiotensin-aldosterone system inhibitor (RAASi)/angiotensin receptor neprilysin inhibitor (ARNI) (72% ARNI and 17.1% RAASi), 95.1% with beta-blockers, 75.8% with mineralocorticoid receptor antagonists (MRA) and 63.2% with sodium/glucose cotransporter 2 inhibitors (SGLT2i).
