A Fine Excision ATLS Manual No Longer Defines Trauma as a Surgical Disease

作者： Millard, William B. The new 8th edition of the Advanced Trauma Life Support (ATLS) course manual contains a small but significant change. The phrase “trauma is a surgical disease” long a point of contention with other specialties caring for trauma patients has been removed. Now used in over 50 countries as the basis for training in the initial assessment and management of trauma this publication reflects the research and clinical experience of the American College of Surgeons (ACS) Committee on Trauma and expresses that organization's philosophy toward triage diagnosis and clinical care. Astute readers of the ATLS materials have noticed that a certain message is conspicuous by its absence. The preface to the 7th edition of the ATLS describes the ACS's role as follows: In accordance with that role and recognizing that trauma is a surgical disease the ACS Committee on Trauma (COT) has worked to establish guidelines for the care of the trauma patient. The 8th edition includes a substantially similar sentence minus the crucial phrase on trauma as a “surgical disease.” John B. Kortbeek MD FACS professor of surgery and critical care at the University of Alberta and a member of the COT who was instrumental in the revision process for the manual confirms that the deletion is intentional. Dr. Kortbeek explains the change in historical terms. “The intent of making that statement” he says “was to emphasize that to have a successful trauma system and a successful trauma hospital surgeons needed to be included in the management team and the care of the trauma patient. That remains true today. What changed over time is that that statement became a focal point and could be interpreted in varying ways including in a negative exclusive way suggesting that only surgeons should be managing trauma patients which is not correct and never was the intent of the statement.” The ATLS he says presents a “common language” for a safe and effective response to trauma not a mandatory formula. 作者： M. Feemster D.M. Dawson A. Behal W. Dixon Matthew Feemster received the B.S degree in Electrical Engineering from Clemson University Clemson South Carolina in December 1994. Upon graduation he remained at Clemson University and received the M.S. degree in Electrical Engineering in 1997. During this time he also served as a research/teaching assistant. His research work focused on the design and implementation of various nonlinear control algorithms with emphasis on the induction motor and mechanical systems with friction present. He is currently working toward his Ph.D. degree in Electrical Engineering at Clemson University. Darren M. Dawson was born in 1962 in Macon Georgia. He received an Associate Degree in Mathematics from Macon Junior College in 1982 and a B.S. Degree in Electrical Engineering from the Georgia Institute of Technology in 1984. He then worked for Westinghouse as a control engineer from 1985 to 1987. In 1987 he returned to the Georgia Institute of Technology where he received the Ph.D. Degree in Electrical Engineering in March 1990. During this time he also served as a research/teaching assistant. In July 1990 he joined the Electrical and Computer Engineering Department and the Center for Advanced Manufacturing (CAM) at Clemson University where he currently holds the position of Professor. Under the CAM director's supervision he currently leads the Robotics and Manufacturing Automation Laboratory which is jointly operated by the Electrical and Mechanical Engineering departments. His main research interests are in the fields of nonlinear based robust adaptive and learning control with application to electro-mechanical systems including robot manipulators motor drives magnetic bearings flexible cables flexible beams and high-speed transport systems. Aman Behal was born in India in 1973. He received his Masters Degree in Electrical Engineering from Indian Institute of Technology Bombay in 1996. In this paper, we extend the observer/control strategies previously published in [25] to an n -link, serially connected, direct drive, rigid link, revolute robot operating in the presence of nonlinear friction effects modeled by the Lu-Gre model. In addition, we also present a new adaptive control technique for compensating for the nonlinear parameterizable Stribeck effects. Specifically, an adaptive observer/controller scheme is developed which contains a feedforward approximation of the Stribeck effects. This feedforward approximation is used in a composite controller/observer strategy which forces the average square integral of the position tracking error to an arbitrarily small value. Experimental results are included to illustrate the performance of the proposed controllers.

USING VIRTUAL ENVIRONMENTS IN THE DESIGN OF SHIPS

Germany and an MS in naval architecture and marine engineering from MIT. He then joined Litton Ship Systems where he was responsible for the preliminary design of the DD-963 hull structure and then for ship system integration as manager LHA ship systems engineering department. From 1972 to 1974 he was the principal research scientist at Hydronautics. In 1974 as technical director he helped establish the Crystal City office of Designers and Planners. Mr. Jons was one of the co-founders of Advanced Marine Enterprises Inc. in 1976 where he serves as corporate vice president engineering. J. Christopher Ryan:earned his bachelors and masters degrees in naval architecture from Webb Institute and MIT respectively. He spent three years at the advanced marine technology division of Litton Industries working on the DD-963 class ship design and related computer aided design projects. He subsequently went to the Navy Department concentrating on early stage design of surface combatants for twelve years including work on the FFG-7 Sea Control Ship CSGN and CVV aircraft carrier projects. He then shifted focus and became the Technical Director for the Computer Supported Design Program in NavSea for five years. Mr. Ryan has served in several supervisory positions within the Ship Design Group in NavSea since that time. He is currently the director of the future ship concepts division. Gary W. Jones:graduated from the University of Tennessee in 1971 with a BS in mechanical engineering and followed up with graduate work at George Washington University and the University of Maryland. Mr. Jones was with the Naval Sea Systems Command from 1971 until 1988 where he was a naval architect in the submarine section of the hull form design division. A major contributor to the expense and length of time to design, build, and test new systems has been the need to build and test hardware prototypes to determine their effectiveness in meeting operational requirements. Recent and dramatic advances in computer simulation technologies hold forth the promise of revolutionizing design and acquisition strategies by providing the means to validate end users' requirements prior to hardware construction. By designing and operationally testing virtual prototypes in a virtual environment, these technologies will soon offer naval architects the ability to build and launch ships in computer-based cyberspace in lieu of the shipbuilder's ways. The authors of this paper provide the background for these developments, explore the significance and ramifications of these technologies to the current process of ship and system design, outline challenges lying ahead, and present their vision and recommendations for future development.

He has published numerous papers in IEEE and international journals in the areas of spread spectrum and networking. In this paper, the performance of a new movable boundary accessing (MBA) technique for future integrated services multibeam satellite systems is studied. The multiservice environment considered includes both asynchronous and isochronous traffic consisting of video, voice, file transfer and interactive data. The movable boundary access technique proposed here will maximize the utilization of the up-link frame capacity. It is shown that the potential user population is substantially increased with the use of a moving boundary policy with minimal overhead.

In this paper, we study the performance of a prioritized on-board baseband switch in conjunction with a multibeam satellite handling integrated services. The services considered for the analysis include voice, video, file transfer and interactive data. The prioritized switch uses both input and output buffering, switch speed-up as well as a two-phase head-of-line resolution algorithm, in order to reduce the buffer loss while maintaining acceptable user delays. The minimum required buffer capacity and switch speed-up for each service in a prioritized environment are found under uniform traffic conditions. It is shown that under uniform traffic conditions, only minimal buffering and switch speed-up are needed even for the lowest priority users. The performance dependence on the switch size is also substantially reduced with head of line resolution and buffering even in a prioritized environment.

This paper addresses the multi-variable optimisation of bioreactor performance. The central idea here is to use a time series model, which is identified on-line, to evaluate the steady-state performance of the bioreactor. A simple algorithm has been developed around this theme and analysed in Kambhampati et al (1989). A fermentation process model has been developed to provide a test system for the optimisation scheme. The model takes into account the effect of temperature and dilution rate on the biomass production. For this process a multi-variable, model is identified on-line. It has been shown that the model provided optimal results.

The basis of a comprehensive real-time knowledge based system for control structure design and supervisory control is developed using the MUSE AT toolkit. Representation of control engineering knowledge as procedural code in an object-based structure ensures that the knowledge base is easily understood and modified by domain experts. The simple learning capability that has been implemented has proved to be extremely useful and is being developed further as part of continuing research in this area. It is also hoped to extend the capabilities of the supervisory modules particularly in dealing with loop interactions, the prediction and diagnosis of faults and the provision of an intelligent operator interface.< >
