Statement of problem: Vat-polymerized casts can be designed with different bases, but the influence of the base design on the accuracy of the casts remains unclear. Purpose: The purpose of the present in vitro study was to evaluate the influence of various base designs (solid, honeycombed, and hollow) with 2 different wall thicknesses (1 mm and 2 mm) on the accuracy of vat-polymerized diagnostic casts. Material and methods: A virtual maxillary cast was obtained and used to create 3 different base designs: solid (S group), honeycombed (HC group), and hollow (H group). The HC and H groups were further divided into 2 subgroups based on the wall thickness of the cast designed: 1 mm (HC-1 and H-1) and 2 mm (HC-2 and H-2) (N=50, n=10). All the specimens were manufactured with a vat-polymerized printer (Nexdent 5100) and a resin material (Nexdent Model Ortho). The linear and 3D discrepancies between the virtual cast and each specimen were measured with a coordinate measuring machine. Trueness was defined as the mean of the average absolute dimensional discrepancy between the virtual cast and the AM specimens and precision as the standard deviation of the dimensional discrepancies between the virtual cast and the AM specimens. The Kolmogorov-Smirnov and Shapiro-Wilk tests revealed that the data were not normally distributed. The data were analyzed with Kruskal-Wallis and Mann-Whitney U pairwise comparison tests (α=.05). Results: The trueness ranged from 63.73 μm to 77.17 μm, and the precision ranged from 44.00 μm to 54.24 μm. The Kruskal-Wallis test revealed significant differences on the x- (P<.001), y- (P=.006), and z-axes (P<.001) and on the 3D discrepancy (P<.001). On the x-axis, the Mann-Whitney test revealed significant differences between the S and H-1 groups (P<.001), S and H-2 groups (P<.001), HC-1 and H-1 groups (P<.001), HC-1 and H-2 groups (P<.001), HC-2 and H-1 groups (P<.001), and HC-2 and H-2 groups (P<.001);on the y-axis, between the S and H-1 groups (P<.001

Despite increasing volume and complexity of emergency general surgery, minimal research has focused on improving surgical consultation processes at academic hospitals. In this needs assessment, we aimed to define the scope of a surgical consult curriculum by characterizing resident perceptions of the current general surgery consultation process at one institution. Review of literature and medical record consult orders was used to develop a branched survey that was disseminated to internal medicine and general surgery residents. The survey focused on eliciting potential learning needs from a range of surgical consult topics, presentation elements, and communication factors. Friedman and Mann–Whitney tests were applied in analyses. Twenty-six medicine residents and 24 surgery residents completed the survey; the majority of both groups (84.6%, 84.2% respectively) agreed that medicine residents need additional instruction on surgical consult topics overall. Medicine resident comfort level presenting various consult topics to surgery residents (p < 0.0001) and managing patients with these surgical problems (p < 0.0001) was statistically different between topics overall. Wound evaluation was most frequently identified by both groups in free response as an area in need of further instruction. The majority of medicine (84.6%) and surgery (95.8%) residents characterized consult interactions as respectful, but medicine residents were less comfortable communicating with surgery residents than the opposite (p = 0.0005). In this study, we demonstrated that medicine residents are uncomfortable preparing certain consult topics in general surgery consultation requests. We also revealed key differences in communication expectations between resident groups that may be undermining successful interdisciplinary communication. These results are being used to develop consult and communication curricula to improve the surgical consultation process at our institution.

Purpose: Ice hockey and field hockey are contact sports with the potential for injury, especially to the head and neck regions. The purpose of this study is to estimate and compare hospital admission (injury severity) between ice hockey and field hockey of those who presented to the emergency department with head and neck injuries. Methods: The investigators designed and implemented a 20-year retrospective cohort study using the National Electronic Injury Surveillance System database. We included data related to ice hockey and field hockey injuries from January 2000 to December 2019 in this study. The primary predictor variable was sport played (ice hockey vs field hockey). Secondary predictor variables and covariates were derived from patient and injury characteristics. The primary outcome variable was hospital admission. Logistic regression was used to determine independent risk factors for the outcome variable. Results: Our final sample was composed of 5,472 patients: 4,472 patients suffered head and neck injuries from ice hockey while the remaining 1,000 patients suffered head and neck injuries from field hockey. Players less than 18 years old were associated with 2.07-fold odds of admission (P <.01). Injury to the head (odds ratio [OR] = 14.339;95% confidence interval [CI], 2.0 to 105.1;P <.01) and neck (OR = 89.260;95% CI, 11.2 to 712.6;P <.01) were independently associated with an increased odds of admission. Relative to contusions/abrasions, players who suffered a concussion (OR = 141.637;95% CI, 11.5 to 1,741.5;P <.01), fracture (OR = 155.434;95% CI, 17.0 to 1,419.2;P <.01), internal organ injury (OR = 186.450;95% CI, 15.5 to 2,236.8;P <.01), or hematoma (OR = 23.046;95% CI, 1.2 to 442.5;P <.05) were all independently associated with an increased odds of admission. Ice hockey was not an independent risk factor for admission relative to field hockey. Conclusions: The findings of this study suggest that ice hockey was more associated with injuries to the head

Statement of problem: Although immediate implant loading has shown promising clinical results and high survival rates, an increased risk of implant failure and complications has been reported. Achieving consistently predictable outcomes with this approach remains a challenge, but evidence-based guidelines to assist in selecting suitable patients are lacking. Purpose: The purpose of this retrospective clinical study was to investigate the success rate, survival rate, and complications of immediate implant loading compared with early and delayed loading. Also, it sought to identify the potential risk factors of immediate implant loading. Material and methods: Dental records for dental implants that were surgically placed at 1 center between January 2016 and October 2021 were reviewed for different loading protocols. Time from surgical placement to load was calculated, and the implants were divided into 3 groups: immediate (IL: <3 weeks), early (EL: 3 weeks to 3 months), and delayed (DL: >3 months). Patient, procedural, and restorative parameters were also recorded. Adverse events reported were implant failure and mechanical and biological complications, and the incident rates per 100 person-years (100 PY) for each adverse event were calculated. The Kaplan-Meier and Cox Proportional Hazards models with time to event approach were stratified by time-to-load (unit of analysis: the implant). Five-year survival rates were evaluated with Kaplan-Meier survival estimates. All statistical analyses were completed with a statistical software program (α=.05). Results: A total of 258 implants (20.1%) were in the IL group, 24 implants (1.9%) in the EL group, and 985 implants (76.8%) in the DL group. The incident rate of any adverse event was 3-fold more among implants in the IL group and EL groups as compared with the DL group (21.0/100 PY, 19/100 PY, 5.8/100 PY, respectively). Immediate implant loading was a significant predictor of implant failure (IL, HR: 30.94;95% CI: 8.23–116.

Background Patient specific reconstruction is becoming more ubiquitous in craniomaxillofacial surgery. There is a paucity of information on what factors may influence case success. Purpose The study purpose was to estimate patient specific hardware failure rate and to identify risks factors associated with hardware failure. Study Design, Setting, Sample A retrospective cohort study was implemented using data from subjects treated with selectively laser melted (SLM) reconstruction plates at the University of Iowa. Subjects were excluded if follow up was less than 3 months, data was not complete, or the subjects were not treated with a SLM plate. Predictor Variables The predictor variables were composed of heterogenous variables grouped into the following categories: demographics, etiology, mandibular characteristics, and reconstruction plate characteristics such as number of screws proximal and distal to fracture/defect. Main Outcome Variable The primary outcome variable was time to screw failure (yes/no) which was determined by examining subject radiographic data, and if it was clinical reason for removal of the reconstruction plate. Covariates The only covariate is sex. Analyses Descriptive statistics were calculated for each variable. Bivariate Cox regression analyses were performed to assess the association between each variable and the hazard of screw failure. Alpha=0.05 was considered significant. Results The sample included 131 subjects. Median follow up time was 11.0 (IQR 14.0) months. There was 1 (0.8%) plate fracture and 10 (7.6%) screw failures. Subjects with one additional screw proximal to the fracture/defect (e.g., from 3 to 4, or 4 to 5, or 5 to 6) had a 63% higher hazard of screw failure at any given time over the follow up period (HR = 1.63, p=0.04, 95% CI 1.02-2.63). Subjects with one additional screw distal to the fracture/defect had a 58% higher hazard of screw failure (HR = 1.58, p=0.01, 95% CI 1.10-2.26). Among patients with continuity defects (
