Relationships Between Wheelchair-Provision Time for Hospital Inpatients and Their Lengths of Stay and Costs of Hospitalization: A Cohort Study

Objectives: To test the hypotheses that wheelchair-provision time (WPT) (from when a loaner wheelchair was ordered to when the wheelchair arrived at the hospital site) has a significant relationship with length of stay (LOS) and total cost of hospitalization (COH). Design: Cohort study. Setting: Hospital inpatient units. Participants: Hospital inpatients (N=97). Intervention: Order for loaner wheelchair placed during their admissions. Main Outcome Measures: Demographic, clinical, and process data from 4 available databases, from which we derived WPT, LOS, and COH. To test the hypotheses, we used Spearman correlation coefficients, negative binomial regression for LOS (n=90), and linear regression for COH (n=69). Results: The median values for WPT, LOS, and total COH were 3.8 days, 51.0 days, and $43,062 Canadian dollars. Multivariable regression revealed that WPT was associated to a statistically significant extent with LOS (P=.0049) (a 18% increase in LOS for each additional day in WPT), but not with COH. However, LOS was associated to a statistically significant extent with COH (P<.0001). The Spearman correlation between LOS and COH was 0.8915 (P<.0001). Conclusions: Statistically significant associations exist between WPT and LOS and between LOS and COH. Although this study does not establish causality and further research is needed, our findings suggest that more rapid provision of loaner wheelchairs to hospital inpatients could have a positive effect on the health care system. © 2025 American Congress of Rehabilitation Medicine

We calculated the spatial distribution of the reduced density and pair distribution function (PDF) of solvent hard spheres near a solute using three-dimensional Ornstein–Zernike (OZ) equations coupled with closures in which Percus–Yevick (PY) and hypernetted–chain (HNC) approximations were used or in which the bridge functions (BFs)proposed by Verlet, Duh and Henderson, and Kinoshita were incorporated. The solute was formed by two solvent hard spheres in contact with each other, with the result that the system was not radial-symmetric, necessitating the extension of the Verlet, Duh–Henderson, and Kinoshita BFs to three-variable functions considered in the Cartesian coordinate system. The results were compared with those from grand canonical Monte Carlo (MC) simulations. In terms of the PDF, HNC was superior to PY in the sense that the results from the former are closer to those from MC. Incorporation of the three BFs made the results even closer to those from MC. The three BFs shared almost the sameperformance for a solute immersed in a one-component solvent at the infinite-dilution limit. Analyses on the triplet distribution function (TDF) were also performed. It was found that in terms of the TDF, the incorporation of the BFs does not necessarily lead to improvement. © 2023, The Authors. All rights reserved.

Accurate and rapid calculation of hydration free energy and its physical implication for biomolecular functions

Here we review a new method for calculating a hydration free energy (HFE) of a solute and discuss its physical implication for biomolecular functions in aqueous environments. The solute hydration is decomposed into processes 1 and 2. A cavity matching the geometric characteristics of the solute at the atomic level is created in process 1. Solute-water van der Waals and electrostatic interaction potentials are incorporated in process 2. The angle-dependent integral equation theory combined with our morphometric approach is applied to process 1, and the three-dimensional reference interaction site model theory is employed for process 2. Molecular models are adopted for water. The new method is characterized by the following. Solutes with various sizes including proteins can be treated in the same manner. It is almost as accurate as the molecular dynamics simulation despite its far smaller computational burden. It enables us to handle a solute possessing a significantly large total charge without difficulty. The HFE can be decomposed into a variety of physically insightful, energetic, and entropic components. It is best suited to the elucidation of mechanisms of protein folding, pressure and cold denaturation of a protein, and different types of molecular recognition. © 2020, International Union for Pure and Applied Biophysics (IUPAB) and Springer-Verlag GmbH Germany, part of Springer Nature.

This work consists of a pilot study in which brain signals captured by electroencephalography (EEG) of children diagnosed with Autism Spectrum Disorder (ASD) are evaluated during the interaction with a social robot. Social skills and interaction of children with ASD with the robot are proposed and assessed, using quantitative scale and images recorded by video cameras. During interaction with the robot, results show high activation of alpha and beta rhythms in brain regions important to social skills. Quantitative scales indicate a positive children–robot interaction and point out the social robot as a potential tool to stimulate social skills and facilitate the interaction with other people.

Preservation of executive function, like inhibition, closely links to the quality of life in senior adults. Although neuroimaging literature has shown enhanced inhibitory function followed by aerobic exercise, current evidence implies inconsistent neuroplasticity patterns along different time durations of exercise. Hence, we conducted a 12-week exercise intervention on 12 young and 14 senior volunteers and repeatedly measured the inhibitory functionality of distinct aspects (facilitation and interference effects) using the numerical Stroop task and functional Magnetic Resonance Imaging. Results showcased improved accuracy and reduced reaction times (RT) after 12-week exercise, attributed to frontoparietal and default mode network effects. In young adults, the first phase (0 to six weeks) exercise increased the activation of the right superior medial frontal gyrus, associated with reduced RT in interference, but in the second intervention phase (six to twelve weeks), the decreased activation of the left superior medial frontal gyrus positively correlated with reduced RT in facilitation. In senior adults, the first six-week intervention led to reduced activations of the inferior frontal gyrus, inferior parietal gyrus, and default mode network regions, associated with the reduced RT in interference. Still, in the second intervention phase, only the visual area exhibited increased activity, associated with reduced RT in interference. Except for the distinctive brain plasticity between the two phases of exercise intervention, the between-group comparison also presented that the old group gained more cognitive benefits within the first six weeks of exercise intervention;however, the cognitive improvements in the young group occurred after six weeks of intervention. Lacking inspiration is a major challenge for designers when performing a design task, and the number and the quality of design ideas are critical indicators of the performance of a designer. With 60 volunteered participants, 70% women and 30% men, who in average had more than three years of design experience, this study tested whether environment color (red, green, and white) affects designers' image processing performance at the design development stage. The data collected were submitted to ANOVA, and the result shows (1) no significant effect of environment color on the number of design ideas (F = 0.553; P = 0.578), (2) no significant effect of environment color on the quality of design ideas (F = 0.156; P = 0.856), and (3) a weak correlation (r = 0.277) between the number of design ideas and the quality of design ideas. A suggestion for future research exploring the effect of environment color on enhancing image processing performance is to refine the experimental control.

INTRODUCTION The radiocarpal joint, which includes articulations between the radius and the proximal carpal row, transmits force through the central column of the wrist. Chronic disruptions to radiocarpal biomechanics are associated with the development of scapholunate advanced collapse (SLAC) pattern OA. While the radiolunate joint is thought to be typically spared in SLAC progression, degenerative changes can present on the proximal lunate. An investigation of radiolunate arthrokinematics may improve understanding of posttraumatic and postsurgical outcomes, including salvage procedures for SLAC wrist such as scaphoid excision with four-corner arthrodesis (SEFCA). Four-dimensional CT, which captures CT volumes over time or motion, has enhanced our ability to quantify arthrokinematics during functional tasks. OBJECTIVE We hypothesize that 4DCT-derived radiolunate arthrokinematics during flexion-extension and radioulnar deviation will differ between healthy, uninjured wrists and a wrist three years post-SEFCA. METHODS Data were collected from the uninjured wrists of ten participants enrolled in a prospective study of unilateral wrist injury and one participant following SEFCA (n=11, 36% female). The SEFCA arthrodesis was fixed using Kirschner wires prepositioned in the head of the capitate and subsequently advanced into the lunate. Included participants were aged between 18 and 60 years; exclusion criteria included history of injury, surgery, or congenital variant in the contralateral wrist; rheumatologic disease; or connective tissue disease. Static-neutral and dynamic CT data were collected using a dual-source scanner (SOMATOM Flash, Siemens, DE) and commercially-available cardiac sequential scan protocol. 4DCT data were collected over a 1.5-second period as signaled by an ECG simulator. Thermostabilization of a membrane proteins, especially G-protein-coupled receptors (GPCRs), is often necessary for biochemical applications and pharmaceutical studies involving structure-based drug design. Here we review our theoretical, physics-based method for identifying thermostabilizing amino acid mutations. Its novel aspects are the following: The entropic effect originating from the translational displacement of hydrocarbon groups within the lipid bilayer is treated as a pivotal factor;a reliable measure of thermostability is introduced and a mutation which enlarges the measure to a significant extent is chosen;and all the possible mutations can be examined with moderate computational effort. It was shown that mutating the residue at a position of N_BW = 3.39 (N_BW is the Ballesteros-Weinstein number) to Arg or Lys leads to the stabilization of significantly many different GPCRs of class A in the inactive state. Up to now, we have been successful in stabilizing several GPCRs and newly solving three-dimensional structures for the muscarinic acetylcholine receptor 2 (M2R), prostaglandin E receptor 4 (EP4), and serotonin 2A receptor (5-HT_2AR) using X-ray crystallography. Effectiveness of the wheelchair skills training program: A systematic review and meta-analysis

Introduction/Background The Wheelchair Skills program (***) is a set of assessment and training protocols–the Wheelchair Skills Test (WST), the questionnaire version of the WST (WST-Q) and the Wheelchair Skills Training program (WSTP). Evidence has been accumulating about the important role that wheelchair skills training has in the wheelchair-provision process recommended by the World Health Organization. The objective of this study was to conduct a systematic review synthesizing the evidence for the effectiveness of the WSTP. Material and method We searched PubMed, the Cochrane Library, CINAHL and Embase databases, as well as grey literature, up to October 10, 2017. We selected studies that were randomized controlled trials (RCTs) assessing the effectiveness of the WSTP. Two independent reviewers screened articles and extracted data. Methodological quality was assessed using Cochrane's Risk of Bias Tool. Meta-analyses (including sub-group analyses) were conducted for the WST and WST-Q total capacity scores. We qualitatively assessed retention, WST/WST-Q subtotal and individual-skill capacity scores and other identified outcomes. The quality of evidence was determined using the Grades of Recommendation, Assessment, Development and Evaluation (GRADE) approach. Results Thirteen RCTs (581 participants) were included for analysis. The level of evidence was of moderate quality. The WSTP increased the post-training WST/WST-Q total capacity scores by 14.0% (95% CI: 7.4, 20.8; P < 0.0001) compared to no treatment, standard care or educational controls, a relative increase of 21.2%. Subgroup analyses showed that training was more effective for new wheelchair users. Retention of training effects was suggested by the absence of significant declines between tests post-training and at follow-up. The WSTP was found to have positive effects on some other outcomes.
