Influence of base design on the manufacturing accuracy of vat-polymerized diagnostic casts: An in vitro study

Statement of problem: Vat-polymerized casts can be designed with different bases, but the influence of the base design on the accuracy of the casts remains unclear. Purpose: The purpose of the present in vitro study was to evaluate the influence of various base designs (solid, honeycombed, and hollow) with 2 different wall thicknesses (1 mm and 2 mm) on the accuracy of vat-polymerized diagnostic casts. Material and methods: A virtual maxillary cast was obtained and used to create 3 different base designs: solid (S group), honeycombed (HC group), and hollow (H group). The HC and H groups were further divided into 2 subgroups based on the wall thickness of the cast designed: 1 mm (HC-1 and H-1) and 2 mm (HC-2 and H-2) (N=50, n=10). All the specimens were manufactured with a vat-polymerized printer (Nexdent 5100) and a resin material (Nexdent Model Ortho). The linear and 3D discrepancies between the virtual cast and each specimen were measured with a coordinate measuring machine. Trueness was defined as the mean of the average absolute dimensional discrepancy between the virtual cast and the AM specimens and precision as the standard deviation of the dimensional discrepancies between the virtual cast and the AM specimens. The Kolmogorov-Smirnov and Shapiro-Wilk tests revealed that the data were not normally distributed. The data were analyzed with Kruskal-Wallis and Mann-Whitney U pairwise comparison tests (α=.05). Results: The trueness ranged from 63.73 μm to 77.17 μm, and the precision ranged from 44.00 μm to 54.24 μm. The Kruskal-Wallis test revealed significant differences on the x- (P<.001), y- (P=.006), and z-axes (P<.001) and on the 3D discrepancy (P<.001). On the x-axis, the Mann-Whitney test revealed significant differences between the S and H-1 groups (P<.001), S and H-2 groups (P<.001), HC-1 and H-1 groups (P<.001), HC-1 and H-2 groups (P<.001), HC-2 and H-1 groups (P<.001), and HC-2 and H-2 groups (P<.001);on the y-axis, between the S and H-1 groups (P<.001

A need for consult and communication curricula to improve the surgical consultation process

Despite increasing volume and complexity of emergency general surgery, minimal research has focused on improving surgical consultation processes at academic hospitals. In this needs assessment, we aimed to define the scope of a surgical consult curriculum by characterizing resident perceptions of the current general surgery consultation process at one institution. Review of literature and medical record consult orders was used to develop a branched survey that was disseminated to internal medicine and general surgery residents. The survey focused on eliciting potential learning needs from a range of surgical consult topics, presentation elements, and communication factors. Friedman and Mann–Whitney tests were applied in analyses. Twenty-six medicine residents and 24 surgery residents completed the survey; the majority of both groups (84.6%, 84.2% respectively) agreed that medicine residents need additional instruction on surgical consult topics overall. Medicine resident comfort level presenting various consult topics to surgery residents (p < 0.0001) and managing patients with these surgical problems (p < 0.0001) was statistically different between topics overall. Wound evaluation was most frequently identified by both groups in free response as an area in need of further instruction. The majority of medicine (84.6%) and surgery (95.8%) residents characterized consult interactions as respectful, but medicine residents were less comfortable communicating with surgery residents than the opposite (p = 0.0005). In this study, we demonstrated that medicine residents are uncomfortable preparing certain consult topics in general surgery consultation requests. We also revealed key differences in communication expectations between resident groups that may be undermining successful interdisciplinary communication. These results are being used to develop consult and communication curricula to improve the surgical consultation process at our institution.

Lateral Lumbar Interbody Fusion (LLIF) is a minimally-invasive, retroperitoneal surgical technique utilized as an alternative to direct anterior and traditional posterior approaches. LLIF can be performed as a trans-psoas or psoas-sparing approach. The majority of LLIF systems utilize neuromonitoring to minimize nerve root injury. The purpose of this manuscript is to review the methods of neuromonitoring and navigation employed in LLIF, and discuss potential advances on the horizon. © 2022

Introduction: The American Board of Pathology (ABPath) has ongoing efforts to better align certification with graduate medical education, training program requirements, and pathology practice. The present study focused on the subspecialty of cytopathology. We evaluated the current content and scope of fellowship programs, practice patterns and needs of diplomates, and program director (PD) and diplomate perceptions of the ABPath certification examination to identify gaps and provide an evidence base to guide harmonization in these areas. Methods: Two surveys were administered: one directed to PDs of all 93 Accreditation Council for Graduate Medical Education (ACGME) cytopathology fellowship programs and the other to cytopathology diplomates submitting continuing certification reporting to the ABPath. Results: Most (86%) cytopathology diplomates work in smaller groups. Only 11% do >50% cytopathology in practice. Diplomates’ cytopathology-related practice tasks varied, as did their perception of the content of fellowship training aligning with practice needs. In fellowship training programs, the specimen types, volumes, techniques of specimen acquisition, and graduated responsibility varied significantly. We identified areas in which current training and certification requirements are challenging for some programs. Diplomates and PDs had differing perceptions of the cytopathology examination;diplomates regarded image-based and microscopic glass slide questions as the best assessment of their knowledge. Conclusions: First, fellowship training programs could benefit from shared resources and should provide more graduated responsibility for fellows. Second, the ACGME Review Committee could consider this data in future program requirement revisions. Finally, information from these surveys will be useful as the ABPath adjusts certification examination content and delivery. © 2021 The Author(s)

Purpose: This study investigated the experiences and perceptions of Saudi patients with insomnia disorder. Method: A cross-sectional design study (July 2019) using a self-administered questionnaire survey. Results: Sixty-eight patients with insomnia disorder participated (53% male, 47% female;60% were younger than 45 years and a majority had a university-degree education level). Of the respondents, 64.7% were dissatisfied with their knowledge of insomnia and 97% wanted more information about its treatment. Approximately 59% used prescribed sleep medicines from the first instance and only 6% were referred to a psychologist for counselling. Benzodiazepines and Z-drugs were prescribed for 16% and 19% of respondents, respectively. A majority of respondents were given antihistamines or over-the-counter herbs. While 60% of patients used medications for long periods, 87% were not involved in treatment decisions. For those on long-term use, 80% were not seen regularly by doctors and 93% were provided with no plan for medicine discontinuations. More than 60% of respondents said that sleeping pills were best for treating insomnia and 31% believed that they are safe and would continue taking them. More than 45% would not prefer non-pharmacological treatments (e.g., counselling). Conclusions: Responses to the survey indicate a lack of knowledge about insomnia disorder and its treatment among Saudi patients. Prescribing hypnotics is the most common treatment, non-pharmacological treatments are seldom used, and responses indicated inappropriate and long-term use of hypnotics. Patients should be better educated about insomnia and its treatment. Physicians should consider using non-pharmacological treatments and the Saudi Ministry of Health should develop country-specific guidelines. © 2021, The Author(s), under exclusive licence to Springer Nature Singapore Pte Ltd.
