Prevalence of Oropharyngeal Dysphagia in Children: A Systematic Review and Meta-Analysis

ObjectiveThis study aims to determine the prevalence of oropharyngeal dysphagia (OD) in children, as diagnosed through instrumental evaluation, and to identify associated risk *** systematic search was conducted across six databases (Embase, LILACS, LIVIVO, PubMed/MEDLINE, Scopus, and Web of Science), as well as gray literature sources (ASHA, Google Scholar, and ProQuest). Studies involving children (under 12 years of age), diagnosed using instrumental methods such as videofluoroscopic swallow study (VFSS) or fiberoptic endoscopic evaluation of swallowing (FEES), were included. A random-effects meta-analysis was performed to estimate the prevalence, with heterogeneity assessed through meta-regression. The risk of bias was evaluated using the Joanna Briggs Institute Critical Appraisal *** total of 31 studies were included, yielding a pooled prevalence estimate of 69% (95% CI: 61%-77%;I-2 = 94%). Most studies involved children with predisposing conditions, potentially contributing to an overestimation of OD prevalence in the general pediatric population. Four studies were assessed as having a moderate risk of bias, while 27 were rated as having a low risk of *** findings suggest a high prevalence of OD, especially in children with underlying health conditions. These results underscore the importance of early diagnosis and tailored interventions to mitigate the health risks associated with deglutition disorders in pediatric populations.

Objective: To map the volitional and non-volitional devices used by speech and language pathologists (SLPs) in voice training and therapy and characterize their use in research on voice interventions. Methods: This scoping review is the first part of a larger study. The electronic search was carried out by mapping the references in PubMed/Medline, LILACS/BVS, Scopus, Web of Science, EMBASE, and the Cochrane Library, and the manual search was carried out in the grey literature. Two blind independent reviewers selected and extracted data;divergences were solved by consensus. The data extracted in this part of the study were the authorship and year of publication, country, study design, sample characteristics, intervention modality, ingredient, target, mechanism of action, dosage, and outcome measures. They were addressed with descriptive analysis. Results: Publications that use devices as ingredients are mostly from the last two decades, mainly carried out in the United States of America and Brazil, in adults of both sexes with behavioral dysphonia. Forty-two types of devices were used, many of them with similar approaches but different nomenclatures. Most devices were used voluntarily, focusing on vocal function, and aiming to increase source and filter interaction. Most studies used silicone tubes. The most reported technical specification to apply the ingredient was surface electrodes on the neck. Device dosage was time-controlled, and the most used outcomes were self-assessment and acoustic analysis. Conclusion: Devices are currently used as ingredients in vocal interventions, with a greater focus on increasing the source and filter interaction, associated with silicone tubes (the most used devices in these studies), which have been dosed with performance time. Outcomes were measured with self-assessment instruments. © 2023 The Voice Foundation

Aim: To investigate the auditory perception of vocal deviation by dysphonic and nondysphonic women and to correlate the accuracy rate with acoustic measurements. Methods: In total, 24 dysphonic women allocated to an experimental group (EG) and 10 nondysphonic women allocated to the control group (CG) participated in this study. The volunteers filled in the vocal screening form the vocal symptom scale (VoiSS) self-assessment protocol and made voice recordings during the emission of the vowel /Ɛ/, whose degree of deviation was examined through perceptual-auditory and acoustic evaluation;all the participants underwent audiometry. Three experiments were conducted for the perception tests. The first experiment consisted of randomly presenting 38 stimuli in isolation, 10 of which were healthy voices and 28 with different degrees of deviation, so that each participant could classify them as normal or altered. In the second experiment, the participants heard only breathy and healthy voices with different degrees of deviation and had to discriminate the presence or absence of breathiness. The third experiment was similar to the second experiment but with a rough voice. Results: Women in the EG had a lower hit rate (52.2%) in identifying dysphonic voices than women in the CG (69.6%) (P < 0.001). The women in the EG had a lower accuracy rate in identifying predominantly rough (62.7%) and breathy (62%) voices, compared with the accuracy rate of the women in the CG (73% and 75.6%, respectively) (P value < 0.001). There was a moderate negative correlation between the hit rate of nondysphonic women and the shimmer values. Conclusions: Dysphonia can interfere with the identification of dysphonic voices. Dysphonic women had a lower accuracy rate in identifying dysphonia and roughness and breathiness parameters than nondysphonic women. © 2025 The Voice Foundation

This article aimed to verify the possible advantages of using mobile applications to promote vocal health and monitor vocal rehabilitation. This integrative review researched five scientific databases, using the descriptors “voice” and “smartphone” or “mobile applications” or “software” and “speech-language.” The search yielded 570 articles, of which seven were included after careful analysis. The studies reveal that application use can assist in the rehabilitation process and raise patient/user awareness about the importance of voice care. Mobile applications aimed at vocal promotion or monitoring generate advantages for speech-language-hearing pathologists and patients/users, such as easy access and use, strengthening the bond between professionals and patients, and encouraging patients/users to change their vocal behaviors positively. © 2025

Transcranial direct current stimulation (tDCS) can be used to temporarily and reversibly modulate brain functions and is an increasingly utilized tool to investigate the relationships between brain and behavior. This study aimed to identify the brain area where tDCS has the greatest effect on vocal quality, vocal range, neuromotor functioning of the larynx, and self-reported vocal effort in vocally healthy individuals. Sixteen adults, non-singers, with a mean age of 22.5 (±1.9) years and no vocal symptoms or brain dysfunction, participated in this double-blinded randomized crossover clinical trial. The participants underwent four anodal tDCS session (20 minutes;2 mA) in targeting different areas—cerebellum, primary motor cortex (M1), primary somatosensory cortex (S1), and dorsolateral prefrontal cortex (DLPFC)—with a 7-day washout period between sessions. Volunteers were evaluated for vocal performance before and after each tDCS session. Voice recordings were taken to assess the following vocal parameters: vocal range profile (VRP) at weak and strong intensities;vocal quality (jitter, shimmer, irregularity, glottal-to-noise excitation ratio, and dysphonia acoustic index) at weak, habitual, and strong intensities;laryngeal diadochokinesis, and self-reported vocal effort using the adapted Borg CR10 scale. M1 stimulation enhanced the VRP by increasing maximum intensity and improving vocal quality, particularly at habitual and strong intensities. Similarly, S1 stimulation improved vocal quality at strong intensities but reduced the VRP by lowering the maximum fundamental frequency. Cerebellum stimulation enhanced neuromuscular control and coordination of the larynx without affecting vocal quality. Additionally, self-reported vocal effort decreased across all stimulated areas following tDCS. Our findings indicate that tDCS applied to different brain areas produces varying effects on vocal parameters. © 2024 The Voice Foundation

Objective: To analyze the internal consistency of the Voice Handicap Index (VHI) for evaluating the vocal handicap of individuals with dysphonia. Methods: This is a systematic review. Studies with a cross-sectional design and including a population of individuals with dysphonia, which validated the VHI and analyzed its internal consistency, were included. The following electronic databases were searched: Cochrane Library, EMBASE, LILACS, and PubMed, including Medline, Scopus, and Web of Science. A manual search was performed in gray literature through the Biblioteca Digital Brasileira de Teses e Dissertações and ProQuest Dissertation & Theses. In addition, the list of references of studies selected in the electronic search was mapped, and an expert in the area was consulted. Two reviewers blindly and independently conducted the selection, data extraction, and analysis of the risk of bias, the certainty of the evidence, and good psychometric measures. A meta-analysis was performed with a random effects model using the JAMOVI 2.3.2 software. Results: Forty-nine studies were analyzed. In risk of bias assessment, the studies were classified as having inadequate structural validity and very good internal consistency. The analysis of good psychometric properties indicated indeterminate structural validity and insufficient internal consistency. The overall value of Cronbach's alpha was estimated at 0.94, thus suggesting a very good internal consistency. However, there was high heterogeneity. The level of certainty of the evidence was too low for internal consistency. Conclusion: The VHI proved to be a consistent and reliable patient-reported outcome measure to evaluate voice handicap in individuals with dysphonia;however, studies are heterogeneous, and the certainty of evidence is very low. © 2023 The Voice Foundation

Uncontrolled manifold method to speech production

speech production is a highly skilled sensorimotor activity defined by articulatory or acoustic coordinates. To compare the variabilities of those two conceptualizations, issues of dimension reduction, normalization, incompleteness of information, etc., need to be taken into account. Uncontrolled manifold (UCM) method analyzes high-dimensional movement dataset with respect to the outcomes that count as successful tasks. It divides the variability in the data into two parts: “bad” variability associated with motion within the controlled manifold (CM) that would lead to an error in the task and “good” variability within the uncontrolled manifold (UCM) that do not harm to the task. The smaller ratio indicates both tighter control (less variability in the CM) and greater flexibility (more variability in the UCM). The UCM method is applied to the Wisconsin X-ray microbeam data. We first constructed a neural-net-based forward mapping from articulators to acoustics. The inter-layer weight matrices and the outputs of each layer in the trained forward model are then used to compute the elements of this forward model’s Jacobian matrices; the Jacobians are then used to compute σCM/σUCM ratios. We further compare these ratios across data obtained in various linguistic conditions.

The results of a retrospective analysis of 85 dysphagia assessments conducted in a geriatric rehabilitation hospital are reported. Descriptive pilot data about the population are presented including diagnosis, anatomical and functional problems, results of videofluoroscopy and treatment recommendations. The findings of the analysis are compared to recent literature in the field.
