Dry eye disease (DED) is a multifaceted ocular surface disorder that significantly impacts patients' daily lives and imposes a substantial economic burden on society. Oxidative stress, induced by the overproduction of reactive oxygen species (ROS), is a critical factor perpetuating the inflammatory cycle in DED. Effectively scavenging ROS is essential to impede the progression of DED. In this study, boronophenylalanine- containing polydopamine (PDA-PBA) nanoparticles are developed loaded with melatonin (MT), which are blended with poly(vinyl alcohol) (PVA) to create eye drops PVA/ PDA-PBA@MT (PPP@MT). In vitro and in vivo studies demonstrate that PPP@MT exhibits dual functionalities in reducing ROS production and downregulating inflammatory pathways, thereby preserving mitochondrial integrity and further inhibiting programmed cell death. Following PPP@MT treatment, tear secretion, corneal structure, and the number of goblet cells are markedly restored in a mouse model of dry eye, indicating the therapeutic efficacy of this agent. Collectively, PPP@MT, characterized by minimal side effects and favorable bioavailability, offers promising therapeutic insights for the management of DED and other ROS-mediated disorders.

Axial length as a predictor of myopic macular degeneration: a meta-analysis and clinical study

Purpose: This study aims to investigate the relationship between axial length (AL) and the severity of myopic macular degeneration (MMD). Methods: We conducted a comprehensive search of PubMed, Web of science, and China National Knowledge Infrastructure (CNKI) databases from their inception until October 1, 2023, to identify population-based or hospital-based studies reporting AL across different grades of MMD. Only studies employing the International Photographic Classification and Grading System for Myopic Maculopathy (META-PM) were included. A meta-analysis was performed to assess the association between AL and MMD severity. To further validate our findings, we analyzed data from 395 eyes of 206 participants at beijing Tongren Hospital, Capital Medical University. Results: The meta-analysis included 20 high-quality studies from seven countries, with 33822 patients studied. Information, including the study name, year of publication, country, sample size, basic demographic characteristics of the participants, AL of different grades of MMD, best corrected visual acuity (BCVA), and spherical equivalent (SE), was extracted. The meta-analysis revealed a significant overall increase in AL as MMD progressed from category C0 to C4 (P < 0.0001). AL exhibited a consistent increasing trend from categories C0 to C3;however, this trend appeared to level off between categories C3 and C4, with no further increase observed. This trend was confirmed by the distribution of our new dataset. A higher prevalence of MMD was significantly associated with longer AL (per 1 mm increase: OR, 1.90;95% CI, 1.75-2.07;P < 0.001), older age (per 1-year increase: OR, 1.04;95% CI, 1.02-1.05;P < 0.001), and female gender (OR, 1.89;95% CI, 1.24-2.89;P < 0.01). Compared with C0, each 1 mm increase in AL was associated with an increasing likelihood of MMD progression, with ORs of 2.8 for C1, 3.6 for C2, 5.2 for C3, and 5.7 for C4. Association between sleep quality and dry eye disease: a literature review and meta-analysis

Objective The purpose of this article is to systematically review the association between dry eye and sleep quality. Methods PubMed, EMBASE, Cochrane, Web of science, and grey literature databases were searched for observational studies published before April 2023. Meta-analysis was performed using STAT15 software. Results A total of 21 studies with 419,218 participants were included. The results showed that the dry eye subjects had a worse sleep quality than the healthy population, with poorer subjective sleep quality, longer sleep latency, and a higher risk of unhealthy sleep duration such as insufficient sleep or excessive sleep. The Pittsburgh Sleep Quality Index (PSQI) scores of the dry eye subjects were significantly higher than those of the control subjects (WMD = 1.78, 95%CI: 1.06, 2.50, P < 0.001). The dry eye subjects scored higher than the control subjects in sleep quality, sleep latency, and sleep disturbance in PSQI;there was no difference between the dry eye individuals and control subjects in sleep duration, sleep efficiency, daytime dysfunction, and sleep medication scores. The risk of sleep disorders in the dry eye subjects was significantly higher than that in the non-dry eye subjects (RR = 2.20, 95%CI: 1.78, 2.72, P < 0.001);the risk of insufficient sleep in the dry eye subjects was higher than that in the control subjects (RR = 3.76, 95%CI: 3.15, 4.48, P < 0.001), and the prevalence of excessive sleepiness in dry eye subjects was higher than that in the control subjects (RR = 5.53, 95%CI: 3.83, 7.18, P < 0.001). The ESS scores of the dry eye subjects were significantly higher than those of the control subjects (WMD = 3.02, 95%CI: 2.43, 3.60, P < 0.01). Conclusion Our meta-analysis suggests that individuals with dry eye have a worse sleep quality than the healthy population, with poorer subjective sleep quality, longer sleep latency, and higher risk of unhealthy sleep duration such as insufficient sleep or excessive sleepiness.

Lens parameters and myopic shift in nuclear cataract after vitrectomy

Purpose To evaluate the relationship between lens parameters and spherical equivalent refraction in the development of nuclear cataract following pars plana vitrectomy (PPV). Methods This prospective cohort study included patients who were scheduled for unilateral PPV surgery. All patients underwent subjective refraction, ocular biometry, and swept-source anterior segment optical coherence tomography (SS-ASOCT) before and after PPV. Lens power and equivalent refractive index were calculated based on the Bennett and Lensmaker formula, and lens nuclear density was quantified based on SS-ASOCT images. Results A total of 110 patients who developed nuclear cataract after PPV were included in the analysis, with a mean follow-up of 10.38 +/- 3.12 months. The operated eyes showed a myopia refractive shift (-2.81 +/- 2.36D), along with an increase in lens power, lens refractive index, and lens thickness, as well as a protruding of the radius of posterior lens surface. Multivariate linear regression analysis showed that the lens power was the main factor contributing to the myopic refractive shift (R-squared: 0.80, P < 0.001). Additionally, the refractive index of the lens was found to be the primary determinant of changes in the refractive power of lens (R-squared: 0.98, P < 0.001). Lens density showed a linear correlation with both the refractive index and lens power (correlation coefficient: 0.48 and 0.50, respectively). Conclusions Changes in lens refractive index are the main cause of lenticular myopia induced by nuclear cataract after PPV. Corneal Epithelial Dendritic Cells: An Objective Indicator for Ocular Surface Inflammation in Patients with Obstructive Meibomian Gland Dysfunction?

PurposeTo examine whether corneal epithelial dendritic cells (CEDC) could serve as an indicator to distinguish obstructive meibomian gland dysfunction (MGD) with or without ocular surface inflammation (OSI).MethodsWe performed a case-control study on patients with diagnosed obstructive MGD between August 2017 and November ***30 MGD cases and 25 healthy controls were recruited. The classification of MGD patients with and without OSI was based on the tear pro-inflammatory cytokine levels. Compared with the MGD without OSI and the control group, a higher CEDC density was detected in the MGD with OSI subgroup. The presence of >15.6 cells/mm(2) CEDC had a sensitivity of 73% and specificity of 75% for the diagnosis of MGD with *** is not present in all patients with obstructive MGD. Evaluation of CEDC density in the central cornea may help identify whether MGD is concomitant with OSI.

Efficacy of botulinum toxin and surgery in managing acute acquired comitant esotropia

Background Acute acquired comitant esotropia (AACE) can significantly impair binocular vision, and its prevalence is increasing. This study aims to compare the effectiveness of botulinum toxin (BTX) injections with strabismus surgery in patients diagnosed with AACE and to investigate the factors predicting success. Methods Sixty AACE patients were included in this prospective comparative clinical study. Twenty-seven patients underwent incisional strabismus surgery (surgery group) and 33 patients received BTX injection (chemodenervation group). Patients were followed up visit at 1, 3, and 6 months post-treatment. The primary outcome was the success rate at 6 months post-treatment, defined as a horizontal deviation of 10 prism diopters (PD) or less with confirmed binocular single vision. Secondary outcomes included risk factors for the recurrence of AACE. Results The present study included 27 patients in the surgery group and 33 in the chemodenervation group. No significant differences were observed in the motor success rate at 1 and 3 months post-treatment between the two groups. However, the surgery group had a significantly higher motor success rate compared to the chemodenervation group at 6 months post-treatment (100% vs. 69.7%, P < 0.001). The success rate of achieving stereopsis at near <= 100 arcsec in the chemodenervation group was significantly higher than in the surgery group at 1 month post-treatment (51.5% vs. 14.8%, P < 0.001). By the 6 months post-treatment, no significant differences were observed in sensory outcomes between the chemodenervation and surgery groups (P > 0.05 for all). In the chemodenervation group, patients with an anisometropia less than 1 dioptor (D)demonstrate significantly higher motor success rate, and better sensory outcomes, including stereopsis at near (84%, 21/25) and stereopsis at near <= 100 arcsec (56%, 14/25), compared to those with anisometropia of 1 D or more (P = 0.044). Correlation Between Relative Peripheral Refraction and Efficacy of Myopia Control After Wearing Multifocal Soft Contact Lenses in Children

IntroductionThis study investigated the relationship between relative peripheral refraction (RPR), retinal choroidal vessel, and the growth rate of axial length (AL) in children with myopia wearing multifocal soft contact lenses (MiSight CLs).MethodsForty-four eyes of 44 children with myopia wearing MiSight CLs were included in the study. RPRs and retinal choroidal vessel were measured at baseline, 3 months, 6 months, and 1 year using multispectral refraction topography (MRT) and optical coherence tomography angiograghy (OCTA). MRT presented RPR at different fields of view and OCTA presented retinal choroidal vessel characteristics centered on the macula. The correlation between the changes of RPR (Delta RPRs) and the growth rate of AL was *** baseline AL, spherical equivalent, age, and average growth rate of AL of children are 24.76 +/- 0.95 mm, - 3.22 +/- 1.77 D, 11.25 +/- 2.42 years, and 0.22 +/- 0.17 mm/year. At 3 months, TRPR (RPR of the circular visual field of 53 degrees centered on macular fovea) (p = 0.0007), RPR-15 (p = 0.0042), RPR-30 (p = 0.0012), RPR-45 (p = 0.0031), and RPR-I (p = 0.0082) of children with myopia wearing MiSight CLs were significantly lower than baseline. The Delta RPR was negatively correlated with the growth rate of AL (Delta TRPR, r = - 0.43, p = 0.003;Delta RPR-45, r = - 0.43, p = 0.004;Delta RPR-I, r = - 0.35, p = 0.02;Delta RPR-N, r = - 0.32, p = 0.04). At 1 year, there was a significant difference in the retina vessel volume index compared to baseline (p = 0.0002).ConclusionRPR showed significant changes at 3 months after wearing multifocal soft contact lenses, but noticeable changes in retinal vessel volume index occurred only at 1 year. MRT demonstrates a higher efficacy in the early detection of the effectiveness of myopia control measures.

A case report of implantable collamer lens removal with cataract extraction and TECNIS Symfony extended range of vision intraocular lens (ZXR00) implantation

BackgroundAnterior subcapsular cataract (ASC) is a leading complication of implantable collamer lens (ICL) implantation and a common indication for secondary ICL removal. However, the visual outcomes of combined ICL explantation, cataract extraction, and implantation of a TECNIS Symfony ZXR00 IOL with a targeted myopic shift (-0.50D to -0.75D) remain uncertain in highly myopic patients with prior ICL and photorefractive keratectomy (PRK) *** presentationA 39-year-old male highly myopic patient who underwent concurrent ICL removal, phacoemulsification, and TECNIS Symfony ZXR00 IOL implantation in both eyes, spaced two weeks apart, is reported in this case. Surgical planning prioritized spectacle independence, occupational visual needs, and the IOL's optical properties, with a deliberate residual myopic refraction (-0.50D to -0.75D). At the 3-month follow-up, comprehensive evaluation was conducted, included manifest refraction, anterior segment OCT (Casia2), defocus curves, OPD-Scan III, and the NEI-VFQ-14 questionnaire. The patient achieved excellent near and intermediate vision, high overall optical quality, and reported exceptional *** patients developing ASC post-ICL implantation, particularly those with strong preferences for spectacle independence and specific visual demands, the TECNIS Symfony ZXR00 IOL, combined with optimized residual myopia, may provide outstanding visual performance and patient satisfaction.

Association between Fundus Tessellation and Contrast Sensitivity in Myopic Eyes

Purpose: To assess the association of fundus tessellation with contrast sensitivity, Quality of Vision questionnaire, and other factors at five years postcorneal refractive ***: This is a cross-sectional study. Both eyes of 98 subjects (196 eyes) who received femtosecond laser in situ keratomileusis (FS-LASIK) or small incision lenticular extraction (SMILE) five years prior were enrolled in this study. Fundus tessellation was imaged using wide-angle fundus photographs and graded into four categories with the assistance of the ETDRS grid. Photopic and mesopic contrast sensitivity were measured under the best correction. The Quality of Vision (QoV) questionnaire was used to assess visual ***: Fundus tessellation was classified as follows: 19 eyes were grade 0 (9.7%), 28 eyes were grade 1 (14.3%), 59 eyes were grade 2 (30.1%), and 90 eyes were grade 3 (45.9%). Higher degrees of fundus tessellation were associated with lower photopic contrast sensitivity, a significant difference was observed at spatial frequencies of 6cpd (p = 0.030, grade 1 >grade 3 p = 0.011). Higher degrees of fundus tessellation were also associated with lower mesopic contrast sensitivity, a significant difference was observed at spatial frequencies of 18cpd (p = 0.011, grade 0 >grade 3 p = 0.012). The preoperative degree of myopia was positively associated with fundus tessellation grade (p < 0.001). However, in linear mixed-effect model analysis, no significant influence of parameters (contrast sensitivity, preoperative myopia, and QoV scores) upon different tessellation grades was found (p > 0.05).Conclusions: Patients with moderate and high myopia were more likely to have higher grades of fundus tessellation. Higher degree of fundus tessellation associates with lower contrast sensitivity. Patients with moderate and high myopia should be concerned with retinal-choroidal changes. Swept-source optical coherence tomography features of choroidal tumors and tumor mimics: A 5-year retrospective study

Background: These lack a thorough comparison of swept-source optical coherence tomography (SS-OCT) features among different kinds of choroidal tumors and tumor mimics. Methods: This retrospective case series study based on 305 eyes from 298 consecutive patients with choroidal tumors or tumor mimics treated at beijing Tongren Hospital between July 2019 and March 2024. The diagnostic performance of SS-OCT features in distinguishing malignant tumors were evaluated. Results: The present study included choroidal melanoma (n = 113), choroidal lymphoma (n = 11), choroidal metastasis (n = 9), choroidal hemangioma (n = 92), choroidal nevus (n = 9), choroidal osteoma (n = 32), RPE adenoma (n = 4), choroidal tuberculoma (n = 2), posterior scleritis (n = 2), and choroidal leiomyoma (n = 1). The median thickness and largest basal diameter were 2.9 mm (interquartile range: 2.0-4.2) and 9.0 mm (interquartile range: 5.7-11.3). Among all SS-OCT features, multivariable logistic regression showed optical shadowing (Odds Ratio [OR]=104.1, 95 %CI: 5.33-2028.8), subretinal fluid (<3 mm from margin: OR=100.9, 95 %CI: 2.21-4607;>3 mm from margin: OR=24.2, 95 %CI: 2.14-274.0), and overlying choriocapillaris complete loss (OR=52.3, 95 %CI: 4.85-552.7) highly indicated malignant choroidal tumor. Choriocapillaris loss was more sensitive (0.962, 95 %CI: 0.926-0.992) in identifying malignant tumors than optical shadowing (0.791, 95 %CI: 0.730-0.850) and subretinal fluid >3 mm from the tumor margin (0.714, 95 %CI: 0.639-0.788). Choriocapillaris loss also showed high sensitivity (0.923, 95 %CI: 0.919-1.000) and specificity (0.973, 95 %CI: 0.940-1.000) in identifying choroidal melanoma from choroidal nevus. The appearance of choriocapillaris loss or subretinal fluid >3 mm from the tumor margin showed 100 % sensitivity in identifying malignant amelanotic tumors. Conclusions: Choriocapillaris loss, optical shadowing, and extensive subretinal fluid can be used to identify malignant tumors.
