Variation in Rates of Neonatal Opioid Withdrawal Syndrome and Opioid-related Hospitalizations in New York State, 2018

Background and Objective: Rates of neonatal opioid withdrawal syndrome (NOWS) among infants and opioid-related hospitalizations among adults have increased in recent decades. We examined rates of NOWS births and opioid-related hospitalizations among adult females of childbearing age overall and by sociodemographic and geographic factors using hospitalization data. Methods: We used data from the New York State Inpatient Database for 2018 from the Healthcare Cost and Utilization Project. We identified infants with NOWS and opioid-related hospitalizations using diagnosis codes and calculated rates of NOWS per 1,000 live births and opioid-related hospitalizations per 1,000 hospitalizations among females of childbearing age. We compared rates by geographic and sociodemographic factors and examined the correlation between county-level rates. Results: The NOWS rate was 4.66 per 1,000 live births, while the rate of opioid-related hospitalizations was 36.6 per 1,000 hospitalizations. There was variation by county and sociodemographic factors including urban-rural classification, race and ethnicity, insurance payer, and median income. We found the highest rates for both NOWS and opioid-related hospitalizations in non-metropolitan areas, among patients with public insurance, and among non-Hispanic White or Native American populations. There was a significant linear correlation (r=0.28;p=0.03) between county-level rates;however, only 32% of counties were concordant in quartiles for both measures. Conclusion: Our findings indicate variation in rates of NOWS and opioid-related hospitalizations by geographic and sociodemographic factors, with only moderate concordance in county-level rates. These findings may provide a nuanced interpretation by examining measures impacting both women and infants and have important implications in prioritizing treatment access. © 2025 The Author(s). Subjective impact of COVID-19 pandemic on youth with tic and OCD spectrum disorders

Objectives: This study investigated the perceived impact of the COVID-19 pandemic on youth with chronic tic disorders (CTD) and/or obsessive–compulsive disorder (OCD) based on subjective reports, objective measures, and parental feedback. This study also sought to investigate whether and how these reported experiences differed based on the presence of underlying tic and/or OCD spectrum diagnoses. Methods: Children with CTD, OCD, and Tics + OCD and their parents were recruited to complete an online survey from July 2020 through April 2021. Forty-eight responses were received;child respondents had a median age of 12 years. Results: On average, youth reported that the pandemic negatively impacted them in several domains, particularly after-school activities, relationships with friends, and social/community gatherings. Despite the small sample size, youth with OCD appeared to experience a greater negative impact compared to other subgroups. Median screen use in this sample was 3–8 hours a day, and youth who reported > 8 hours on weekends trended towards increased depressive symptoms. Conclusion: Consistent with the physician-authors’ clinical experiences, youth with CTD, OCD, and Tics + OCD and their parents reported a subjective negative impact of the pandemic on various symptoms and psychosocial domains. Going forward, if another lockdown loomed, it would be valuable to stay attuned to these vulnerable youth, particularly those with OCD symptoms, and consider providing support in specific psychosocial domains, such as relationship with peers and home life. © 2023 Elsevier Inc.

INTRODUCTION: Oxidative stress (OS) is implicated in dementia. While elevated peripheral OS biomarkers were observed in vascular mild cognitive impairment (vMCI), the role of central antioxidants remains unclear. We assessed levels of the major brain antioxidant glutathione (GSH) in vMCI compared to cognitively normal coronary artery disease (CAD) controls (CN). METHODS: In vivo tissue-corrected GSH in the anterior cingulate cortex (ACC) and occipital cortex (OC) were quantified in persons with vMCI and CN using MEscher–GArwood Point RESolved magnetic resonance Spectroscopy. RESULTS: Among participants (vMCI, n = 22, age [mean ± SD] = 67.4 ± 7.3;CN, n = 21, age = 66.7 ± 7.8), ACC-GSH (i.u. ± SD) was higher in vMCI (4.42 ± 0.59) versus CN (3.72 ± 1.01) (Z = −2.5, p =.01), even after controlling for age and sex (B [SE] = 0.74 [0.26], p =.007). Increased ACC-GSH correlated with poorer executive function (EF) (B [SE] = −0.31 [0.14], p =.04). OC-GSH showed no effect. DISCUSSION: Higher ACC-GSH in vMCI may reflect a compensatory response to OS. ACC-GSH was negatively correlated with EF, suggesting a linkage between regional brain antioxidants and disease-relevant cognitive domains. Highlights: Brain GSH was measured in vascular MCI and matched controls using MEGA-PRESS. In contrast to GSH deficits in AD, anterior cingulate GSH was elevated in vMCI. Brain GSH was correlated with disease-relevant cognitive domains in vMCI. The GSH antioxidant system may be etiologically implicated in vMCI. © 2025 The Author(s). Validation of the Self-Assessed Dynamic Gait Index in People with Multiple Sclerosis

Background and Purpose The Dynamic Gait Index (DGI) is a validated clinical measure of walking ability in people with multiple sclerosis (MS). There is need for a self-assessed version of the DGI that can be conducted remotely and that demonstrates concurrent validity with the original DGI in people with MS. We therefore developed a self-assessed Dynamic Gait Index (sDGI). The purpose of this study is to evaluate the relationship between the sDGI, clinically-assessed DGI, and falls in people with MS with gait impairment. Methods We will enroll 50 ambulatory people with MS with gait impairment. Participants will complete the sDGI (online questionnaire) and then, within 4-weeks, the DGI (in-person evaluation by a physical therapist). Other outcome measures include: modified Telephone Interview for Cognitive Status (TICS-M) and self-reported fall history in the past 3 months. We will calculate concordance between the sDGI and DGI, and use linear regression to determine if concordance is impacted by cognition. The ability of the sDGI to identify fallers will be evaluated using the receiver operating characteristic curve. Results At time of submission, 41 participants are enrolled, 31 have completed the study. Anticipated date of study completion is March 2023. Full study results will be presented at the meeting. Discussion The sDGI has potential to be used by physicians, physical therapists, and others to remotely assess walking ability and identify fallers in people with MS. Information from this study will determine how self-assessment of walking ability compares with clinically-assessed walking ability in people with MS. Our study will also determine if cognition impacts the validity of the sDGI. If concordance between sDGI and DGI is lower in those with worse cognitive performance, users of the sDGI might exercise greater caution using this measure to approximate DGI and predict fall risk in individuals with cognitive impairment.

Background: Brain-computer interface (BCI) systems are controlled by users through neurophysiological input for a variety of applications, including communication, environmental control, motor rehabilitation, and cognitive training. Although individuals with severe speech and physical impairment are the primary users of this technology, BCIs have emerged as a potential tool for broader populations, especially with regards to delivering cognitive training/interventions with neurofeedback. Methods: The goal of this study was to investigate the feasibility of using a BCI system with neurofeedback as an intervention for people with mild Alzheimer's disease (AD). The study focused on visual attention and language since AD is often associated with functional impairments in language and reading. This study enrolled 5 adults with mild AD in a nine- to 13-week BCI EEG-based neurofeedback (NFB) intervention to improve attention and reading skills. Two participants completed intervention entirely. The remaining three participants could not complete the intervention phase because of COVID-19 restrictions. Pre- and post-assessment measures were used to assess reliability of outcome measures and generalization of treatment to functional reading, processing speed, attention, and working memory skills. Results: Participants demonstrated steady improvement in most cognitive measures across experimental phases, although there was not a significant effect of NFB on most measures of attention. One subject demonstrated statistically significant improvement in letter cancellation during NFB. All participants with mild AD learned to operate a BCI system with training. Conclusions: Results have broad implications for the design and use of BCI systems for participants with cognitive impairment. Background: The Global COVID Vaccine Safety (GCoVS) project was established in 2021 under the multinational Global Vaccine Data Network (GVDN) consortium to facilitate the rapid assessment of the safety of newly introduced vaccines. This study analyzed data from GVDN member sites on the background incidence rates of conditions designated as adverse events of special interest (AESI) for COVID-19 vaccine safety monitoring. Methods: Eleven GVDN global sites obtained data from national or regional healthcare databases using standardized methods. Incident events of 13 pre-defined AESI were included for a pre-pandemic period (2015–19) and the first pandemic year (2020). Background incidence rates (IR) and 95% confidence intervals (CI) were calculated for inpatient and emergency department encounters, stratified by age and sex, and compared between pre-pandemic and pandemic periods using incidence rate ratios. Results: An estimated 197 million people contributed 1,189,652,926 person-years of follow-up time. Among inpatients in the pre-pandemic period (2015–19), generalized seizures were the most common neurological AESI (IR ranged from 22.15 [95% CI 19.01–25.65] to 278.82 [278.20–279.44] per 100,000 person-years);acute disseminated encephalomyelitis was the least common (<0.5 per 100,000 person-years at most sites). Pulmonary embolism was the most common thrombotic event (IR 45.34 [95% CI 44.85–45.84] to 93.77 [95% CI 93.46–94.08] per 100,000 person-years). The IR of myocarditis ranged from 1.60 [(95% CI 1.45–1.76) to 7.76 (95% CI 7.46–8.08) per 100,000 person-years. The IR of several AESI varied by site, healthcare setting, age and sex. The IR of some AESI were notably different in 2020 compared to 2015–19. Conclusion: Background incidence of AESIs exhibited some variability across study sites and between pre-pandemic and pandemic periods. These findings will contribute to global vaccine safety surveillance and research. © 2023
