An integral part of understanding and then designing programs to reduce childhood cancer inequities includes adequate representation of people with cancer in research, including children. A scoping review was carried out to understand how cancer research is oriented toward inequities and to identify who has participated in childhood qualitative cancer research. A systematic search identified 119 qualitative studies that met inclusion criteria, with most studies taking place in high-income countries (n=84). Overall, data were lacking on social determinants of health at multiple levels—structural, household, child, and guardian. Only 29 studies reported on race and/or ethnicity, with the majority of those including predominantly or all white children. Six articles included socioeconomic information, and across most articles, attention was absent to the financial ramifications of cancer care. Limited reporting of sociodemographics highlights a broader issue of neglecting key demographics and social factors that contribute to inequities. © 2024 The Authors

Estimation of the risk of lung cancer in women participating in a population-based breast cancer screening program

Introduction Lung cancer mortality is increasing in women. In Spain, estimates suggest that lung cancer mortality may soon surpass breast cancer mortality, the main cause of cancer mortality among women. The aim of this study was to estimate the proportion of women at high risk of developing lung cancer in a group of participants in a population-based breast cancer screening program. Methods Cross-sectional study in a sample of women who participated in a population-based breast cancer screening program in 2016 in Hospitalet de Llobregat n = 1,601. High risk of lung cancer was defined according to the inclusion criteria of the National Lung Screening Trial (NLST) and the Dutch-Belgian randomized lung cancer screening trial (NELSON). Results Around 20% of smokers according to NLST and 40% of smokers according to NELSON criteria, and around 20% of former smokers according to both criteria, are at high risk of developing lung cancer. A positive and statistically significant trend is observed between the proportion of women at high risk and nicotine dependence measured with the brief Fagerström Test. Conclusions A high proportion of participants in this breast cancer screening program have a high risk of developing lung cancer and would be eligible to participate in a lung cancer screening program. BACKGROUND:Breast, cervical, and colorectal cancer screening are recommended and can reduce mortality from these cancers, yet information on screening prevalence in the US-Affiliated Pacific Islands (USAPI) is limited. METHODS:We analyzed data from population-based cross-sectional surveys undertaken at different time points from 2016 to 2019 in American Samoa, the Commonwealth of the Northern Mariana Islands, the Republic of Palau, the Republic of the Marshall Islands, and the Federated States of Micronesia states of Pohnpei and Kosrae. We estimated the age-standardized percentage of never screened and up-to-date screening for breast, cervical, and colorectal cancer among eligible adults by select sociodemographic and health characteristics. RESULTS:In the USAPI overall, 20.6 % (95 % confidence interval [CI]: 18.6, 22.8) of participants were up-to-date with breast cancer screening, 38.6 % (95 % CI: 37.1, 40.2) with cervical cancer screening, and 15.1 % (95 % CI: 13.8, 16.4) with colorectal cancer screening. Screening in the USAPI overall was lower for all three cancers among participants who reported having a high school education or less compared to those with more than a high school education. Cervical cancer screening was lower among participants with diabetes compared to those without diabetes, and colorectal cancer screening was lower among participants who reported tobacco use than among those without tobacco use. CONCLUSION:cancer screening was suboptimal across all three cancer types in the USAPI. Developing, implementing, or expanding culturally tailored and effective cancer screening strategies may address barriers to screening and improve access and utilization.

IntroductionSexual dysfunction is a common and persistent complication following hematopoietic stem cell transplant (HSCT). We developed and tested a multimodal sexual dysfunction intervention consisting of three monthly visits with trained transplant clinicians focused on addressing biopsychosocial causes of sexual dysfunction. We previously demonstrated that this intervention led to improvements in sexual function and quality of life (QOL) and reductions in anxiety and depression symptoms. ObjectivesOur aim for the present study was to examine the degree to which the effects of the sexual dysfunction intervention on patient-reported QOL and mood symptoms were accounted for by changes in sexual function and satisfaction. MethodsWe conducted a secondary analysis of a randomized clinical trial investigating a multimodal sexual dysfunction intervention versus enhanced standard care for HSCT survivors between 2/2019 and 2/2023. Participants completed self-report measures of sexual function (i.e., PROMIS SexFS domains of sexual satisfaction, interest, and orgasm), QOL (FACT-BMT), and anxiety and depression symptoms (HADS) at baseline and 3 months post-enrollment. We used causal mediation with bootstrapping to assess whether changes in sexual function domains mediated intervention effects on patient-reported QOL and mood symptoms at 3 months controlling for baseline values. ResultsIn total, 125 HSCT recipients ( M age=55 years, 33% female, 73% allogeneic HSCT) participated. Changes in sexual satisfaction, interest, and orgasm mediated the intervention effects on 3-month QOL (indirect effect = 7.20, 95%CI 2.74, 11.65; indirect effect = 4.21, 95%CI 0.67, 7.74, indirect effect = 5.92, 95%CI 2.20, 9.65, respectively) and depression symptoms (indirect effect = -1.03, 95%CI -1.79, -0.28; indirect effect = -0.65, 95%CI -1.23, -0.06; indirect effect = -0.72, 95%CI -1.38, -0.05, respectively). BackgroundSexual dysfunction is the most common complication affecting HSCT survivors and is associated with worse psychological distress and patient QOL. Yet, interventions to address sexual dysfunction in HSCT survivors are lacking. MethodsWe conducted a single center randomized clinical trial of a multimodal intervention to address sexual dysfunction in autologous and allogeneic HSCT survivors who were at least 3 months post-HSCT and endorsed sexual dysfunction causing distress. Patients were randomly assigned to the intervention or usual care. Intervention participants met with a trained HSCT clinician for three monthly visits that focused on 1) assessing the causes of their sexual dysfunction; 2) educating and empowering patients to address sexual health concerns; and 3) implementing therapeutic interventions (e.g., vaginal lubrication, medications, intimacy exercises, etc.). We assessed patient global satisfaction with sex, interest in sex, orgasm pleasure, erectile function, vaginal discomfort (PROMIS), QOL (Functional Assessment of cancer Therapy-Bone Marrow Transplant [FACT-BMT]), and psychological distress (Hospital Anxiety and Depression-Scale [HADS]) at baseline and 3 months after enrollment. The primary endpoint was to compare patient global satisfaction with sex at three months between the study groups. We used linear regression models, adjusting for baseline scores, to evaluate the intervention effects on study outcomes at three months. ResultsWe enrolled 74.0% (125/169) of eligible patients (mean age = 55.5 (SD=14.0), 67% male). Overall, 93.7% (60/64) of those randomized to the intervention attended all three intervention visits. At 3 months, patients randomized to the intervention reported improved global satisfaction with sex (B=4.7, P<0.001), interest in sex (B=1.3, P<0.001), orgasm pleasure (B=3.3, P<0.001), erectile function (B=10.9, P<0.001), and vaginal discomfort (B=-9.6, P=0.008) compared to those assigned to usual care. BackgroundSexual dysfunction is the most common complication affecting HCT survivors and is associated with worse patient-reported quality of life (QOL) and psychological distress. Yet, interventions to address sexual dysfunction in HCT survivors are lacking. MethodsWe conducted a pilot randomized trial of a multimodal mobile application (SHIFT) to address sexual dysfunction for patients with hematologic malignancies who were at least 3 months post autologous or allogeneic HCT and endorsed sexual dysfunction causing distress. Patients were randomly assigned to SHIFT or enhanced usual care. Intervention participants met with a trained HCT clinician for a brief clinical exam to address biological causes of sexual dysfunction and were then provided access to SHIFT for 8 weeks. SHIFT consists of five modules focused on educating and empowering patients to address their sexual health concerns and addressing the biologic, interpersonal, social, and psychological causes of sexual dysfunction. Patients assigned to enhanced usual care met with the trained HCT clinician for a brief clinical exam and they were not given access to SHIFT. The primary endpoint was feasibility, defined a priori as at least 60% of eligible patients enrolling and 60% of those enrolled completing at least 70% of the SHIFT modules. We assessed patient global satisfaction with sex, interest in sex, orgasm pleasure (PROMIS), QOL (Functional Assessment of cancer Therapy-Bone Marrow Transplant [FACT-BMT]), and psychological distress (Hospital Anxiety and Depression-Scale [HADS]) at baseline and 8 weeks. To assess the usability of SHIFT, we used the System Usability Scale (>80 indicates excellent usability). We a priori identified a p-value <0.25 as promising for preliminary efficacy in this pilot study. ResultsWe enrolled 64.2% (61/95) of eligible patients (mean age = 57.2 (SD=14.2), 60% male). Adverse Childhood Experiences are Associated With e-Cigarette Abstinence in a Vaping Cessation Randomized Clinical Trial Among Adolescents

PURPOSE:Adverse childhood experiences (ACEs) are linked to e-cigarette use. To date, no studies have examined the association between ACEs and e-cigarette cessation. This study examined the relationship between ACEs and vaping cessation outcomes among adolescent e-cigarette users. METHODS:Participants were n = 1,248 adolescent (13-17 years) e-cigarette users recruited nationally to a randomized clinical trial of vaping cessation who completed the PEdiatric ACEs and Related Life Events Screener at baseline. The primary outcome was 30-day point prevalence abstinence from nicotine vaping at 7 months (missingness = vaping). Analyses examined associations between vaping cessation and individual ACEs items, risk strata (low: 0 ACEs; intermediate: 1-3 ACEs; high: ≥4 ACEs), and domains of adversity using inverse probability of retention weighted logistic regression adjusted for baseline confounders. RESULTS:Most participants (90.1%) reported at least 1 ACE, with 55.8% classified as high risk (≥4 ACEs). High-risk participants showed 20% lower abstinence rates compared to combined low or intermediate-risk groups (risk ratio = 0.79, 95% confidence interval = 0.69-0.91, p < .001). Each additional abuse experience was associated with a 7% decrease in abstinence rates (risk ratio= 0.93, 95% confidence interval = 0.87-0.99, p = .028), while neglect and household dysfunction domains showed no significant associations with cessation outcomes. DISCUSSION:This study provides the first evidence of an inverse relationship between ACEs and vaping cessation success among adolescents. The results suggest that trauma-informed approaches may be valuable additions to youth vaping cessation interventions.

cancer patients often want information from "peers" with the same diagnosis or treatment. To increase access to this valuable resource, we developed a website to deliver written peer support to cancer patients undergoing stem cell transplant. Because little evidence describes how to optimize benefits or reduce potential harms of written peer support, we gathered multiple forms of stakeholder feedback to inform the website's ethical approach, personalization, design, function, and content: a Community Advisory Board; a longitudinal study of patients' written peer support needs and motivations; focus groups; semi-structured interviews; and usability testing. Findings provide a rich foundation for website development.

BackgroundTreatment-related sexual dysfunction affects the majority of hematopoietic stem cell transplant (HSCT) survivors. Data regarding the relationship between sexual health domains and patient-reported quality of life (QOL) and psychological distress are limited, particularly in this population. MethodsWe conducted a secondary analysis of a cross-sectional study that included 125 autologous and allogeneic HSCT survivors. All were at least 3 months post-HSCT and endorsed sexual dysfunction causing distress. This was based on the two-question approach from the 2015 National Comprehensive cancer Network (NCCN) survivorship guidelines assessing 1) problems with sexual functioning and 2) whether these problems were causing distress. We assessed global satisfaction with sex and interest in sex (PROMIS), QOL (Functional Assessment of cancer Therapy-Bone Marrow Transplant [FACT-BMT]), and psychological distress (Hospital Anxiety and Depression-Scale [HADS]). We used multivariate regression analyses, adjusting for age, race, and gender, to examine the association between the sexual health domains with QOL and psychological distress. ResultsStudy participants had a median age of 55.0 (SD 14), and the majority were male (67.2%; 84/125). Female sex was associated with lower global satisfaction with sex (B= -2.14, P=0.019) and lower interest in sex (B= - 1.69, P<0.001). In multivariable regression analyses, global satisfaction with sex was associated with better QOL (B=0.86, P=0.015) but not with depression (B= -0.12, P=0.056) or anxiety (B= -0.10, P=0.180) symptoms. Patient's interest in sex was associated with their QOL (B=3.28, P<0.001), depression (B= -0.51, P=0.002), and anxiety (B= -0.39, P=0.032) symptoms. ConclusionsSexual health domains including global satisfaction with sex and interest in sex were associated with QOL and psychological distress at baseline in HSCT survivors.
