Randomized trial of a community-based, culturally tailored intervention: High engagement among Latina breast cancer survivors

BACKGROUND:A preliminary randomized controlled trial was conducted to evaluate the impact of a culturally tailored, group-based intervention for Latina breast cancer survivors (BCSs) and their caregivers on survivors' quality of life (QOL) compared to usual care services provided by community-based organizations. METHODS:This study randomized 136 Latina BCSs and 136 caregivers in Washington, DC; New York, New York; and San Jose, California to an eight-session culturally tailored coping and communication intervention (n = 70 dyads) or usual care support services via community-based organizations (n = 66 dyads). QOL domains (anxiety, depression, fatigue, physical functioning, and social functioning) were measured at baseline, immediately post-intervention, and 6 months post-intervention. RESULTS:Among the 70 dyads in the intervention, 71.4% (n = 50) attended five or more of the eight sessions, which demonstrated high engagement; 82% of participants were retained at 6 months. Results indicated clinically significant improvements in Latina BCSs' reports of anxiety and fatigue at 6 months postintervention compared to BCSs in usual care; however, after adjustment for multiple comparisons, results were not statistically significant. CONCLUSIONS:A culturally tailored, group-based intervention for Latina BCSs and their caregivers led to clinically relevant but not statistically significant improvements in survivors' anxiety and fatigue compared to usual services provided by community-based organizations. Interventions conducted in partnership with community-based organizations yield high engagement and retention; these types of collaborative approaches to psychosocial services have the potential to positively affect Latina BCSs' QOL.

BACKGROUND: A high prevalence of excess weight in children younger than 5 years suggests the involvement of early-life risk factors. The preconception and pregnancy periods are crucial stages for the implementation of interventions to prevent childhood obesity. Most studies so far have evaluated the effects of early-life factors separately, with only a few investigating the combined effect of parental lifestyle factors. Our objective was to fill the literature gap regarding parental lifestyle factors in the preconception and pregnancy periods and to study their association with the risk of overweight in children after the age of 5 years. METHODS: We harmonised and interpreted data from four European mother-offspring cohorts (EDEN [comprising 1900 families], Elfe [comprising 18 000 families], Lifeways [comprising 1100 families], and Generation R [comprising 9500 families]). Written informed consent was obtained from parents of all involved children. Lifestyle factor data collected through questionnaires comprised parental smoking, BMI, gestational weight gain, diet, physical activity, and sedentary behaviour. We applied principal component analyses to identify multiple lifestyle patterns in preconception and pregnancy. Their association with child BMI z-score and risk of overweight (including obesity, overweight and obesity, as defined by the International Task Force reference) between the ages of 5 and 12 years were assessed using cohort-specific multivariable linear and logistic regression models (adjusted for confounders including parental age, education level, employment status, geographic origin, parity, and household income). FINDINGS: Among the various lifestyle patterns identified in all cohorts, the two that better explained variance were high parental smoking plus low maternal diet quality or high maternal sedentary behaviour, and high parental BMI plus low gestational weight gain. Background: Age-related disparities in outcomes for pediatric and adolescent leukemia are multifactorial. While the prevalence of pre-existing medical morbidities is low in children and adolescents, cancer-associated organ dysfunction leading to increased acuity of illness at initial presentation has been noted in up to 24% of those with pediatric leukemia. We have noted an association between adolescent and young adult (AYA) age group and acuity of illness at initial presentation (Jain et al, Blood , 2023). The current study aims to examine (1) the association between acuity of illness and overall survival, and (2) whether acuity of illness mediates age-related differences in overall survival. Methods: We conducted a retrospective analysis of 688 patients aged 1-21 years who presented to Children's Healthcare of Atlanta between 2010-2018 with any newly diagnosed leukemia, including B-cell acute lymphoblastic leukemia (ALL), T-cell ALL, acute myeloid leukemia, and other leukemias (chronic myeloid leukemia, juvenile myelomonocytic leukemia, Burkitt's leukemia). We excluded patients who lacked information in the first 72 hours following initial presentation to the hospital due to incomplete records or initial treatment at another facility. Based on manual chart review, high acuity of illness (yes/no) was defined as any ICU admission or ICU-level resource utilization by organ system (i.e., cardiovascular, respiratory, renal, hematologic, and neurologic), regardless of physical location in the hospital. We assessed overall survival with Kaplan-Meier analysis, with censoring defined by date at last follow-up, 5 years post-diagnosis, or December 31, 2020, whichever came first; the analysis was stratified by acuity of illness (high versus low) at initial presentation. Background: Chronic inflammation is associated with numerous chronic diseases and can be managed with diet. Aim: The purpose of this study was to examine differences in baseline characteristics and plasma inflammation levels between two groups of participants that participated in an intensive, lifestyle intervention or a remotely delivered intervention. This work also assessed the association between Dietary Inflammatory Index (DII)^® scores and participants’ inflammatory and metabolic biomarkers at baseline. Method: Ninety-five participants (61 intervention, 34 control) chose to enroll in either a 12-month intervention consisting of a face-to-face nutrition, physical activity, and stress management intervention or a remotely-delivered intervention (control group) focusing on general cancer prevention. The intervention group met at the University of South Carolina for classes and the control group had materials emailed to them. A quantile regression was used to compare participants’ high-sensitivity C-reactive protein and interleukin-6 levels. Multiple linear regression was used to determine the association between DII scores and biomarkers. Results: There were significant differences in age, body mass index, body fat percentage, and blood pressure between groups, but there were no differences in levels of inflammatory biomarkers. Values of interleukin-6 at the 90th percentile of its distribution were 8.31 pg/ml higher among those in DII quartile 4 compared with quartile 1 (p = 0.02). All other outcomes were not significant. Conclusion: Given similar levels of inflammatory biomarkers, participants opting for the control group would also have benefited from a more intensive lifestyle intervention focusing on reducing inflammation. © The Author(s) 2018.
