No evidence-based interventions exist for anxiety related to pelvic exams after cancer treatment. A 3-session cognitive behavioral intervention, Pelvic Examination and Anxiety Coping Skills for Empowerment (PEACE), was developed. We conducted a proof-of concept pilot study (N=16) to evaluate intervention acceptability and feasibility and changes in anxiety and pain symptoms. Participants completed 3 assessments (baseline, post-intervention, post-pelvic exam) and post-exam interviews. Exploratory outcomes included exam-related distress and pain, and intervention mechanisms of change. The mixed methods findings indicated preliminary acceptability (e.g., 71% endorsed PEACE “made things a lot better”) and feasibility (e.g., 87.5% completed the protocol). PEACE shows promise for reducing procedural distress, which may ultimately improve care engagement. © 2024 Society for Menstrual Cycle Research.

Changes in Perceptions of the Near-Home Walking Environment Among Us Adults—2015 and 2020 National Health Interview Survey

The built environments in which we work, live, and play can influence physical activity behaviors, and perceptions of these environments are associated with walking behavior. This study’s objective is to compare national-level data on perceptions of the near-home walking environment from the 2015 and 2020 National Health Interview Survey. Adults in 2015 (n = 30,811) and 2020 (n = 29,636) reported perceptions of walkable supports (roads, sidewalks, paths, or trails;sidewalks on most streets), destinations (shops, stores, or markets;bus or transit stops;movies, libraries, or churches;and places that help you relax, clear your mind, and reduce stress), and barriers to walking (traffic, crime, and animals). Age-adjusted prevalence estimates, prevalence differences, and 95% confidence intervals were calculated overall and by demographic characteristics. The reported prevalence of roads, sidewalks, paths, or trails for walking increased overall (85.3% in 2015 to 88.0% in 2020) and for many subgroups. Perceived places to walk to for relaxation, to clear your mind, and to reduce stress increased overall (72.1% in 2015 to 77.1% in 2020) and for all subgroups. Perceptions of crime as a barrier to walking decreased overall (12.5% in 2015 to 11.2% in 2020) and for some subgroups. From 2015 to 2020, perceptions of the near-home walking environment did not change, although perceptions increased regarding having roads, sidewalks, paths, or trails, places to relax, and crime. Continuing to monitor perceptions of the walking environment could contribute to progress toward national walking and walkability goals in the United States. © 2023, The Authors. Quantile Differences in the Age-Related Decline in Cardiorespiratory Fitness Between Sexes in Adults Without Type 2 Diabetes Mellitus in the United States

Objective: To comprehensively assess the extent to which the decline in cardiorespiratory fitness (CRF) with age differs between sexes. Participants and Methods: This study used data from the Aerobics Center Longitudinal Study, conducted between September 1974 and August 2006, consisting primarily of White adults from middle-to-upper socioeconomic strata restricted to adults without type 2 diabetes mellitus (33,742 men and 9,415 women). Quantile regression models were used to estimate the differences in age-associated changes in CRF between the sexes, estimated using a maximal treadmill test. Results: For adults aged up to 45 years, significant differences in slopes relating to age and CRF between men and women were observed for all adjusted percentiles of CRF other than the 90th percentile;women reported significantly greater declines in CRF per year. For those aged 45-60 years and those older than 60 years, no significant differences in age-related declines in CRF were observed between the sexes. Conclusion: This study found that compared with men, the onset of decline in CRF was found to occur earlier and at lower CRF percentiles in women. This is of particular concern, given that compared with men, women already tend to have lower CRF levels. These findings suggest that maintaining the levels of physical activity sufficient to maintain moderate-to-high levels of fitness is particularly important for women earlier during adulthood. © 2022 The Authors

During the coronavirus disease 2019(COVID-19)pandemic,the Japanese government implemented several infection control measures,such as states of emergency(SoE)and school closures,and encouraged universal masking and hand *** activity restrictions and improved hygiene were found to be associated with declines in the incidence of infuenza during the 2019–2020 winter season[1].Prior studies have found that the incidence of seven pediatric infections,such as pharyngoconjunctivitis and varicella,decreased during and after the school closures in Japan[2].As activity restrictions continue to be lifted,there is growing concern that pediatric infections may suddenly return given that the pool of susceptible children increased during the pandemic[3].In this study,we use national infectious disease sentinel data from January 2012 to October 2021 to assess changes in the reported number of pediatric infections in Japan during the COVID-19 *** hypothesize that the reporting of many infectious diseases decreased during the early stages of the pandemic but ultimately returned to normal when activity restrictions were lifted.

BACKGROUND: Populations of African American or Black women have persistently higher breast cancer mortality than the overall US population, despite having slightly lower age-adjusted incidence. METHODS: Three cancer Intervention and Surveillance Modeling Network simulation teams modeled cancer mortality disparities between Black female populations and the overall US population. Model inputs used racial group-specific data from clinical trials, national registries, nationally representative surveys, and observational studies. Analyses began with cancer mortality in the overall population and sequentially replaced parameters for Black populations to quantify the percentage of modeled breast cancer morality disparities attributable to differences in demographics, incidence, access to screening and treatment, and variation in tumor biology and response to therapy. RESULTS: Results were similar across the 3 models. In 2019, racial differences in incidence and competing mortality accounted for a net ‒1% of mortality disparities, while tumor subtype and stage distributions accounted for a mean of 20% (range across models = 13%-24%), and screening accounted for a mean of 3% (range = 3%-4%) of the modeled mortality disparities. Treatment parameters accounted for the majority of modeled mortality disparities: mean = 17% (range = 16%-19%) for treatment initiation and mean = 61% (range = 57%-63%) for real-world effectiveness. CONCLUSION: Our model results suggest that changes in policies that target improvements in treatment access could increase breast cancer equity. The findings also highlight that efforts must extend beyond policies targeting equity in treatment initiation to include high-quality treatment completion. This research will facilitate future modeling to test the effects of different specific policy changes on mortality disparities. © The Author(s) 2023. Published by Oxford University Press. Aims The national incidence, risk factors, and associated mortality of atrial fibrillation (AF) in breast cancer patients are unknown. Methods and results Using the Surveillance, Epidemiology, and End Results-Medicare-linked database, we identified females, ≥66 years old, with a new primary diagnosis of breast cancer from 2007 through 2014. These patients were individually matched 1:1 to Medicare enrolees without cancer, and each pair was followed for 1 year to identify a primary outcome of AF. Cumulative incidence was calculated using competing risk survival statistics. Following this, identifying risk factors of AF among breast cancer patients was conducted using the adjusted Cox proportional hazards model. Finally, Kaplan–Meier methods and adjusted Cox proportional hazards modelling were performed to estimate mortality in breast cancer patients with incident and prevalent AF. This study included 85 423 breast cancer patients. Among these 9425 (11.0%) had AF diagnosis prior to the breast cancer diagnosis. New-onset AF was diagnosed in 2993 (3.9%) patients in a 1-year period after the breast cancer diagnosis [incidence 3.3%, 95% confidence interval (CI) 3.0–3.5%, at 1 year; higher rate in the first 60 days (0.6%/month)]. Comparatively, the incidence of new-onset AF in matched non-cancer controls was 1.8% (95% CI 1.6–2.0%). Apart from traditional demographic and cardiovascular risk factors, breast cancer stage was strongly associated with the development of AF [American Joint Committee on cancer (AJCC) Stage II/III/IV vs. I: adjusted hazard ratio (aHR) 1.51/2.63/4.21, respectively]. New-onset AF after breast cancer diagnosis (aHR 3.00) is associated with increased 1-year cardiovascular mortality. Clinical Validation of a Personalized NHL-TME Model System: One-on-One Comparison between Modeled Drug Responses and Patients' Clinical Responses

Background Clinical outcomes for patients with non-Hodgkin lymphoma (NHL) are highly variable depending on patient characteristics, pathologic features, and genomic abnormalities. Despite efforts to personalize treatment decisions using next-generation sequencing, success has been limited due to the substantial genomic heterogeneity observed even among tumors of the same histologic subtype. To address the unmet needs of personalized treatment of NHL, we established ex vivo tumor microenvironment (TME) models by co-culturing bone marrow fibroblasts (BMF) plus B-cell growth factors with tumor cells from individual CLL patients (Lu et al. Blood cancer J 2021 ). Proliferation centers in the lymph nodes are the major histological sites for CLL pathogenesis, progression, and transformation (Soma et al. Hum Pathol, 2006 ). Our ex vivo TME models, with their ability to promote CLL proliferation, recapitulate this most salient cell behavior observed in vivo. In our initial validation with a cohort of 31 CLL patients, ex vivo drug responses ( EVR ) demonstrated strong correlation with individual patients' clinical responses to ibrutinib. Sensitive patients' cells exhibited sensitivity in the system, while resistant patients' cells behaved resistant. In our current study, we present additional validations of this model system, incorporating genomic and cytogenetic analyses, along with clinical correlations in patients with other NHL histologies, including Richter-transformed CLL and MCL. Methods & Results We found in our previous study (Lu et al) that the co-cultured CLL cells show characteristics typical of lymph node-resident CLL cells, including larger cell size and decreased cell surface CXCR4. Single cell RNA sequencing technology confirmed the downregulation of CXCR4 at the mRNA level. In addition, the co-cultured CLL cells exhibited increased MYC and E2F gene signatures, further supporting the fidelity of the ex vivo system in mimicking the lymph node TME. Additionally,

BackgroundThe COVID-19 pandemic disrupted routine cancer screening; however, it is unclear whether the public’s cancer screening information-seeking behaviors changed. Our study examined cancer screening information-seeking before and after pandemic onset using data from the National cancer Institute’s cancer Information *** analyzed screening inquiries from general public users before (3/27/19–3/26/20) and after (3/27/20–3/26/21) pandemic onset. We examined point of access, subjects of interaction, and *** were more general public cancer screening inquiries made post-pandemic onset (n= 1069; 56.1%) compared to before (n= 837, 43.9%). Although the proportion of breast cancer screening inquiries increased after pandemic onset, inquiries for cervical, colorectal, and other cancers decreased. Telephone inquiries increased, whereas email and instant chat inquiries decreased. Inquiries regarding finding healthcare services and managing costs increased, whereas screening tests and other subjects decreased. Referrals increased for the National Breast and Cervical cancer Early Detection program and other national/community organizations. All differences were significant atp≤ .*** found potentially important changes in cancer screening information-seeking after pandemic onset, including increasing interest in breast cancer screening, yet decreasing interest in other cancer screening tests. Future work should evaluate how public health crises affect information seeking and influence intentions to screen for cancer.

In March 2020, there were no treatment options for COVID-19. Passive immune therapy including anti-SARS-CoV-2 hyperimmune globulin (hIVIG) was a logical candidate for COVID-19 therapeutic trials, given past success treating emerging pathogens with endogenous neutralizing antibodies. We established a plasma collection protocol for persons recovered from COVID-19. To speed recruitment in the first U.S. hotspot, Seattle, Washington, federal and state public health agencies collaborated with Bloodworks Northwest to collect convalescent plasma (CP) for manufacturing hIVIG. During March–December 2020, we identified and recruited prospective CP donors via letters to persons recovered from COVID-19 with laboratory-confirmed SARS-CoV-2 infection. Prospective donors were pre-screened and administered a medical history survey. Anti-SARS-CoV-2 neutralizing antibody (NAb) titers were classified as qualifying (≥1:80) or non-qualifying (<1:80) for enrollment based on a live virus neutralization assay. Generalized estimating equations were used to identify characteristics of donors associated with qualifying versus nonqualifying NAb titers. Overall, 21,359 letters resulted in 3207 inquiries, 2159 prescreenings with laboratory-confirmed SARS-CoV-2 infection, and 573 donors (27% of all pre-screenings with confirmed infection) who provided a screening plasma donation. Of 573 donors screened, 254 (44%) provided plasma with qualifying NAb titers, resulting in 1284 units for hIVIG manufacture. In a multivariable model, after adjusting for other factors, time (60 days) from COVID-19 symptom onset to screening was associated with lower odds of qualifying NAb (adjusted odds ratio = 0.67, 95% CI: 0.48–0.94). The collaboration facilitated a rapid response to develop and provide hIVIG for clinical trials and CP for transfusion. Only 1 in 12 donor inquiries resulted in a qualifying plasma donation.
