Comorbidity burden and risk of second primary non-breast cancer in breast cancer survivors

Background Breast cancer survivors have an increased risk of second primary cancers. However, the impact of comorbidity burden on risk of second primary non-breast cancer in breast cancer survivors is under-studied. We evaluated the association of comorbidity burden with risk of second primary non-breast cancer in breast cancer survivors. Methods In this retrospective cohort study, breast cancer patients aged ≥ 18 years diagnosed with in-situ, local, or regional first primary breast between 2008 and 2020 at Kaiser Permanente Southern California, were included. Patients were followed until 12/31/2021. Rates of second primary non-breast cancer were reported. Association of Elixhauser comorbidity index (ECI) and second primary cancer was reported using Cox proportional hazards regression. Results Of 37,222 female breast cancer survivors included, 1701 (4.6 %) survivors developed second primary non-breast cancers during a median (interquartile range) follow-up of 5.2 (2.8–8.6) years. Nearly one-third (32.3 %) of the survivors had ECI score 3 + ; 22.6 % had ECI score 1–2; rest 27.0 % had zero ECI score. Incidence rates of second cancers were 561, 739, and 1064 per 100,000 person-years in survivors with ECI= 0, 1–2, and 3 + , respectively. In multivariable analyses, compared to survivors with ECI= 0, survivors with ECI scores 1–2 and 3 + had higher risk of second primary non-breast cancer [unadjusted HR (95 % CI): 1.32 (1.16–1.50) and 1.88 (1.65–2.14), respectively]. Higher risk of second primary non-breast cancer persisted in survivors with ECI score 3 + in the adjusted model [adjusted HR (95 % CI): 1.24 (1.08–1.43)]. Conclusion Comorbidity burden appears to be correlated with an increased risk of second primary non-breast cancers in breast cancer survivors. Active surveillance and effective management of modifiable comorbidities may help in reducing second primary cancer risk in breast cancer survivors.

Background: Breast, cervical, and colorectal cancer screening are recommended and can reduce mortality from these cancers, yet information on screening prevalence in the US-Affiliated Pacific Islands (USAPI) is limited. Methods: We analyzed data from population-based cross-sectional surveys undertaken at different time points from 2016 to 2019 in American Samoa, the Commonwealth of the Northern Mariana Islands, the Republic of Palau, the Republic of the Marshall Islands, and the Federated States of Micronesia states of Pohnpei and Kosrae. We estimated the age-standardized percentage of never screened and up-to-date screening for breast, cervical, and colorectal cancer among eligible adults by select sociodemographic and health characteristics. Results: In the USAPI overall, 20.6 % (95 % confidence interval [CI]: 18.6, 22.8) of participants were up-to-date with breast cancer screening, 38.6 % (95 % CI: 37.1, 40.2) with cervical cancer screening, and 15.1 % (95 % CI: 13.8, 16.4) with colorectal cancer screening. Screening in the USAPI overall was lower for all three cancers among participants who reported having a high school education or less compared to those with more than a high school education. Cervical cancer screening was lower among participants with diabetes compared to those without diabetes, and colorectal cancer screening was lower among participants who reported tobacco use than among those without tobacco use. Conclusion: cancer screening was suboptimal across all three cancer types in the USAPI. Developing, implementing, or expanding culturally tailored and effective cancer screening strategies may address barriers to screening and improve access and utilization. © 2025 Elsevier Ltd

BackgroundTreatment-related sexual dysfunction affects the majority of hematopoietic stem cell transplant (HSCT) survivors. Data regarding the relationship between sexual health domains and patient-reported quality of life (QOL) and psychological distress are limited, particularly in this population. MethodsWe conducted a secondary analysis of a cross-sectional study that included 125 autologous and allogeneic HSCT survivors. All were at least 3 months post-HSCT and endorsed sexual dysfunction causing distress. This was based on the two-question approach from the 2015 National Comprehensive cancer Network (NCCN) survivorship guidelines assessing 1) problems with sexual functioning and 2) whether these problems were causing distress. We assessed global satisfaction with sex and interest in sex (PROMIS), QOL (Functional Assessment of cancer Therapy-Bone Marrow Transplant [FACT-BMT]), and psychological distress (Hospital Anxiety and Depression-Scale [HADS]). We used multivariate regression analyses, adjusting for age, race, and gender, to examine the association between the sexual health domains with QOL and psychological distress. ResultsStudy participants had a median age of 55.0 (SD 14), and the majority were male (67.2%; 84/125). Female sex was associated with lower global satisfaction with sex (B= -2.14, P=0.019) and lower interest in sex (B= - 1.69, P<0.001). In multivariable regression analyses, global satisfaction with sex was associated with better QOL (B=0.86, P=0.015) but not with depression (B= -0.12, P=0.056) or anxiety (B= -0.10, P=0.180) symptoms. Patient's interest in sex was associated with their QOL (B=3.28, P<0.001), depression (B= -0.51, P=0.002), and anxiety (B= -0.39, P=0.032) symptoms. ConclusionsSexual health domains including global satisfaction with sex and interest in sex were associated with QOL and psychological distress at baseline in HSCT survivors. An increased risk of ALL second primary malignancy (SPM) has been observed with lenalidomide maintenance therapy after autologous HCT (len-post-autoHCT) for MM. Lenalidomide exerts its therapeutic effects via cereblon-mediated downregulation of the transcription factors Ikaros ( IKZF1) and Aiolos ( IKZF3). Inherited susceptibility variants in IKZF1 have been described for de novo ALL. We performed a case-control germline targeted sequencing study of the entire IKZF1 and IKZF3 genes with mean coverage ∼2,200X to investigate inherited susceptibility to ALL SPM risk. All cases and controls had a primary diagnosis of MM and received len-post-autoHCT with melphalan; sequencing was performed on an aliquot of the apheresis product or peripheral blood sample that was collected before autoHCT for MM. Cases of ALL SPMs were contributed by 4 individual centers and one national trial (BMT CTN 0702); each case had a maximum of 4 controls per case, sampled with replacement and matched on age, sex, race/ethnicity and duration of lenalidomide exposure (controls had longer duration of lenalidomide maintenance than cases). An additional control group of Acute Myeloid Leukemia/Myelodysplastic Syndrome (AML/MDS) SPMs after len-post-autoHCT were contributed by one center (Table). Multivariate logistic regression of case-control status, adjusted by age at HCT, sex, race/ethnicity, duration of lenalidomide maintenance, was performed for: 1) ALL SPM, 2) AML/MDS SPM, 3) ALL and AML/MDS SPM. We found the risk allele A of rs62447181 in IKZF1 had an OR=2.51 P=9.53 × 10 −3 for ALL SPM, OR=4.18 P=0.09 for AML/MDS SPM and OR=2.57 P=4.98 × 10 −3 for ALL&AML/MDS SPM. rs62447181 is a cis-expression quantitative trait loci (eQTL) of IKZF1 in blood (p=3.88 × 10 −55 in eQTLGen Consortium data) and resides in a promoter-proximal enhancer predicted by the ENCODE project (Figure A). Purpose: This case-control study aimed to examine the association between the inflammation potential of the diet and depression. Patients and Methods: Ninety-six patients with major depression disorder were matched with 96 age-and sex-matched healthy controls. Energy-adjusted DII (E-DII™) scores were calculated based on the participants’ responses to a detailed nutritional assessment using a quantitative food frequency questionnaire. Descriptive results were reported. Logistic regression was used to estimate odds ratios and 95% confidence intervals associated with E-DII scores and depression adjusting for covariates including age, sex, body mass index, comorbidities, level of education, and employment. Results: The mean E-DII score of participants in the study was 1.32±1.08, indicating a generally pro-inflammatory diet. Patients with depression had elevated E-DII scores compared to age-and sex-matched controls with E-DII scores of 1.72± 1.09 vs 0.90±0.90, respectively (P=0.001). Results of logistic regression analysis revealed that patients with depression were nearly three times more likely to be in the extreme ends of pro-inflammatory diet (OR 2.75;95% CI 1.82–4.15). Conclusion: Adult Bahraini patients with depression have higher dietary inflammation potential compared to the general population controls. Further research is needed to confirm these results and provide effective interventions to reduce the burden of this dietary inflammation. © 2021 Chen et al.

BACKGROUND:The risks of breast cancer in African American (AA) women associated with inherited mutations in breast cancer predisposition genes are not well defined. Thus, whether multigene germline hereditary cancer testing panels are applicable to this population is unknown. We assessed associations between mutations in panel-based genes and breast cancer risk in 5054 AA women with breast cancer and 4993 unaffected AA women drawn from 10 epidemiologic studies. METHODS:Germline DNA samples were sequenced for mutations in 23 cancer predisposition genes using a QIAseq multiplex amplicon panel. Prevalence of mutations and odds ratios (ORs) for associations with breast cancer risk were estimated with adjustment for study design, age, and family history of breast cancer. RESULTS:Pathogenic mutations were identified in 10.3% of women with estrogen receptor (ER)-negative breast cancer, 5.2% of women with ER-positive breast cancer, and 2.3% of unaffected women. Mutations in BRCA1, BRCA2, and PALB2 were associated with high risks of breast cancer (OR = 47.55, 95% confidence interval [CI] = 10.43 to >100; OR = 7.25, 95% CI = 4.07 to 14.12; OR = 8.54, 95% CI = 3.67 to 24.95, respectively). RAD51D mutations were associated with high risk of ER-negative disease (OR = 7.82, 95% CI = 1.61 to 57.42). Moderate risks were observed for CHEK2, ATM, ERCC3, and FANCC mutations with ER-positive cancer, and RECQL mutations with all breast cancer. CONCLUSIONS:The study identifies genes that predispose to breast cancer in the AA population, demonstrates the validity of current breast cancer testing panels for use in AA women, and provides a basis for increased referral of AA patients for cancer genetic testing.
