Background: The sustainable development goals (SDGs) aim to end HIV/AIDS as a public health threat by 2030. Understanding the current state of the HIV epidemic and its change over time is essential to this effort. This study assesses the current sex-specific HIV burden in 204 countries and territories and measures progress in the control of the epidemic. Methods: To estimate age-specific and sex-specific trends in 48 of 204 countries, we extended the Estimation and Projection Package Age-Sex Model to also implement the spectrum paediatric model. We used this model in cases where age and sex specific HIV-seroprevalence surveys and antenatal care-clinic sentinel surveillance data were available. For the remaining 156 of 204 locations, we developed a cohort-incidence bias adjustment to derive incidence as a function of cause-of-death data from vital registration systems. The incidence was input to a custom Spectrum model. To assess progress, we measured the percentage change in incident cases and deaths between 2010 and 2019 (threshold >75% decline), the ratio of incident cases to number of people living with HIV (incidence-to-prevalence ratio threshold <0·03), and the ratio of incident cases to deaths (incidence-to-mortality ratio threshold <1·0). Findings: In 2019, there were 36·8 million (95% uncertainty interval [UI] 35·1–38·9) people living with HIV worldwide. There were 0·84 males (95% UI 0·78–0·91) per female living with HIV in 2019, 0·99 male infections (0·91–1·10) for every female infection, and 1·02 male deaths (0·95–1·10) per female death. Global progress in incident cases and deaths between 2010 and 2019 was driven by sub-Saharan Africa (with a 28·52% decrease in incident cases, 95% UI 19·58–35·43, and a 39·66% decrease in deaths, 36·49–42·36). Elsewhere, the incidence remained stable or increased, whereas deaths generally decreased. Cox regression analysys with missing covariates via nonparametric multiple imputation

We consider the situation of estimating Cox regression in which some covariates are subject to missing, and there exists additional informa-tion (including observed event time, censoring indicator and fully observed covariates) which may be predictive of the missing covariates. We propose to use two working regression models: One for predicting the missing co-variates and the other for predicting the missing probabilities. For each missing covariate observation, these two working models are used to define a nearest neighbor imputing set. This set is then used to nonparametrically impute covariate values for the missing observation. Upon the completion of imputation, Cox regression is performed on the multiply imputed datasets to estimate the regression coefficients. In a simulation study, we compare the nonparametric multiple imputation approach with the augmented in-verse probability weighted (AIPW) method, which directly incorporates the two working models into estimation of Cox regression. We show that all approaches can reduce bias due to non-ignorable missing mechanism. The proposed nonparametric imputation method is robust to mis-specification of either one of the two working models and robust to mis-specification of the link function of the two working models. In contrast, the AIPW method is not robust to mis-specification of the link function of the two working mod-els and is sensitive to the selection probability. Genome-wide association studies (GWAS) have identified >250 loci for body mass index (BMI), implicating pathways related to neuronal biology. Most GWAS loci represent clusters of common, noncoding variants from which pinpointing causal genes remains challenging. Here we combined data from 718,734 individuals to discover rare and low-frequency (minor allele frequency (MAF) < 5%) coding variants associated with BMI. We identified 14 coding variants in 13 genes, of which 8 variants were in genes (ZBTB7B, ACHE, RAPGEF3, RAB21, ZFHX3, ENTPD6, ZFR2 and ZNF169) newly implicated in human obesity, 2 variants were in genes (MC4R and KSR2) previously observed to be mutated in extreme obesity and 2 variants were in GIPR. The effect sizes of rare variants are ~10 times larger than those of common variants, with the largest effect observed in carriers of an MC4R mutation introducing a stop codon (***35Ter, MAF = 0.01%), who weighed ~7 kg more than non-carriers. Pathway analyses based on the variants associated with BMI confirm enrichment of neuronal genes and provide new evidence for adipocyte and energy expenditure biology, widening the potential of genetically supported therapeutic targets in obesity.

Background Several studies have emerged in the past two decades investigating the role of human papillomavirus infection in colorectal cancer development. To better inform global prevention strategies for colorectal cancer, we reviewed available evidence on the associations between human papillomavirus infection and colorectal cancer. Methods In this systematic review and meta-analysis, we searched PubMed, Embase, Cochrane Library, Web of Science, and websites of international conferences for studies published before Dec 31, 2019. Interventional or observational studies containing original quantitative data describing associations between human papillomavirus and incidence or prevalence of colorectal cancer were included. We calculated pooled odds ratios (ORs) and their 95% CIs using random-effect models. We assessed risk of bias using the Newcastle-Ottawa scale. Findings We identified 20 observational studies including 1424 cases of colorectal cancer or adenoma and 1363 controls. The prevalence of human papillomavirus was 34·5% in the group with colorectal cancer or adenoma and 17% in the control group. Human papillomavirus was associated with a three-fold increase in colorectal cancer overall (OR 3·05, 95% CI 2·01–4·62; heterogeneity I 2 63·5%). In the subgroup analysis, human papillomavirus was associated with increased colorectal cancer prevalence in controls (2·96, 1·16–7·61; I 2 75·3%) and in the group with adjacent normal tissues (3·05, 2·12–4·40; I 2 31·2%). Interpretation We found evidence that human papillomavirus is likely to be associated with increased colorectal cancer prevalence. prevention of human papillomavirus infection could be considered for inclusion in campaigns promoting prevention strategies for colorectal cancer.
