Background: Screening has become one of our best tools for early detection and prevention of cancer. The group-randomized trial is the most rigorous experimental design for evaluating multilevel interventions. However, identifying the proper sample size for a group-randomized trial requires reliable estimates of intraclass correlation (ICC) for screening outcomes, which are not available to researchers. We present crude and adjusted ICC estimates for cancer screening outcomes for various levels of aggregation (physician, clinic, and county) and provide an example of how these ICC estimates may be used in the design of a future trial. Methods: Investigators working in the area of cancer screening were contacted and asked to provide crude and adjusted ICC estimates using the analysis of variance method estimator. Results: Of the 29 investigators identified, estimates were obtained from 10 investigators who had relevant data. ICC estimates were calculated from 13 different studies, with more than half of the studies collecting information on colorectal screening. In the majority of cases, ICC estimates could be adjusted for age, education, and other demographic characteristics, leading to a reduction in the ICC. ICC estimates varied considerably by cancer site and level of aggregation of the groups. Conclusions: Previously, only two articles had published ICCs for cancer screening outcomes. We have complied more than 130 crude and adjusted ICC estimates covering breast, cervical, colon, and prostate screening and have detailed them by level of aggregation, screening measure, and study characteristics. We have also demonstrated their use in planning a future trial and the need for the evaluation of the proposed interval estimator for binary outcomes under conditions typically seen in GRTs. © The Author 2010. Atypical Work Hours and Metabolic Syndrome Among Police Officers

ABSTRACTABSTRACTThis study examined whether atypical work hours are associated with metabolic syndrome among a random sample of 98 police officers. Shift work and overtime data from daily payroll records and reported sleep duration were obtained. Metabolic syndrome was defined as elevated waist circumference and triglycerides, low HDL cholesterol, hypertension, and glucose intolerance. Multivariate analysis of variance and analysis of covariance models were used for analyses. Officers working midnight shifts were on average younger and had a slightly higher mean number of metabolic syndrome components. Stratification on sleep duration and overtime revealed significant associations between midnight shifts and the mean number of metabolic syndrome components among officers with less sleep (p= .013) and more overtime (p= .007). Results suggest shorter sleep duration and more overtime combined with midnight shift work may be important contributors to the metabolic syndrome.

Dr. Altman has published in the areas of community health promotion and disease prevention most recently on issues related to tobacco control. His current research examines: a) relationships between community participation psychological empowerment and health behavior b) the relationship between tobacco access and tobacco use among youth c) the extent to which tobacco farmers can be encouraged to engage in crop diversification d) strategies for increasing physical activity among the elderly and e) strategies for public health intervention. DOUGLAS W. LEVINE is an Assistant Professor in the Department of Public Health Sciences Section on Social Sciences and Health Policy at the Bowman Gray School of Medicine Wake Forest University. His areas of expertise are related to statistics and quantitative methods. He is also conducting research on the subsistence activities of the Alaskan native residents in the Bristol Bay Region of Alaska and with David Altman a four-year study with tobacco farmers in North Carolina. GEORGE HOWARD Ph.D. is a Professor of Biostatistics and Epidemiology in the Department of Public Health Sciences of Wake Forest University. He received his Dr.P.H. from the University of North Carolina at Chapel Hill in 1987 and has published extensively on cardiovascular and cerebrovascular epidemiology risk factors including the effects of active and passive cigarette smoking on atherosclerosis. The tobacco industry remains a formidable influence in thwarting tobacco control efforts, in part because industry leaders have raised policymaker and public concern about how tobacco control might negatively impact tobacco farmers. In this study, we interviewed a random, stratified sample of 528 tobacco farmers in the Southeast and 991 U.S. residents about issues related to tobacco farmers. We found that a majority of the U.S. public expressed the belief that U.S. taxpayers subsidized tobacco farmers. This belief, combined with the strong opposition to tobacco farmer subsidies (15 percent thought there should be subsidies), likely play a role in public attitudes toward tobacco farmers. At the same time, however, we found reasonably strong public support for efforts to help tobacco farmers transition to new enterprises, although the public underestimates tobacco farmer interest in diversification. For example, two of three Americans believe that the government should help farmers make a living other than growing tobacco and over half thought that taxpayers should help pay for such programs. These data illustrate potential common ground for elected officials to consider as they attempt to concomitantly address the needs of farmers and the interests of the public. Ultimately, if the interests of tobacco farmers can be made clearly to the general public, and vice versa, pro- health and pro-farmer policies might well become a reality.

Cigarette smoking is negatively associated with family average income among urban and rural men in Regional Mainland China

Socio-economic status (SES) has a strong influence on cigarette smoking behaviour. However, as a more sensitive and realistic index of SES, family average income (FAI) has little studied regarding its association with smoking. With a response rate of 90.1%, a cross-sectional study was conducted among randomly selected urban-rural participants (n=29,353) between October of 2000 and March of 2001 in Nanjing, China. The proportion of male participants who were current smokers was 54.7%;for females it was 2.2%. After adjustment for possible confounding variables (area of residence, age, education, occupation) males in the middle (OR 0.76;95% CI 0.69-0.84) and higher (OR 0.64;95% CI 0.57-0.71) FAI tertiles had lower odds of being smokers than did males in the lower FAI tertile. There were no differences by FAI category in the odds of being an ex-smoker. Therefore, current smoking among adult males is inversely associated with family average income in a regional Chinese population. FAI may inform the targeting of campaigns or other initiatives, particularly in populations where material prosperity is low in some social groups. © 2006 Springer Verlag.

The article provides information on a workshop on the development of imaging systems for cancer prevention and preemption convened by the U.S. National cancer Institute (NCI) cancer Imaging program held in Gaithersburg, Maryland. Doctor Katherine Ferrara of the University of California discussed the function of ultrasound in breast tumor imaging. Ways to close the gap between development and application of imaging approaches were tackled in a roundtable discussion on cervix and skin cancer.
