To investigate the genetic basis of type 2 diabetes (T2D) to high resolution, the GoT2D and T2D-GENES consortia catalogued variation from whole-genome sequencing of 2,657 European individuals and exome sequencing of 12,940 individuals of multiple ancestries. Over 27M SNPs, indels, and structural variants were identified, including 99% of low-frequency (minor allele frequency [MAF] 0.1-5%) non-coding variants in the whole-genome sequenced individuals and 99.7% of low-frequency coding variants in the whole-exome sequenced individuals. Each variant was tested for association with T2D in the sequenced individuals, and, to increase power, most were tested in larger numbers of individuals (>80% of low-frequency coding variants in ~82 K Europeans via the exome chip, and ~90% of low-frequency non-coding variants in ~44 K Europeans via genotype imputation). The variants, genotypes, and association statistics from these analyses provide the largest reference to date of human genetic information relevant to T2D, for use in activities such as T2D-focused genotype imputation, functional characterization of variants or genes, and other novel analyses to detect associations between sequence variation and T2D.

Agents of human chromoblastomycosis,a skin disease almost exclusively caused by members of the order Chaetothyriales,are assumed to be traumatically inoculated into the skin with sharp environmental materials such as plant thorns or wooden splinters carrying the respective *** the supposition that such fungi should have their main habitat in the environment,we investigated the occurrence of black fungi in living areas of patients with *** South America Fonsecaea agents are prevalent as agents of the disease,while also related Cladophialophora species,known from other types of skin infections,are known from the *** environmental isolates were preliminarily selected as possible agents of chromoblastomycosis,based on *** from ITS sequence data isolates were attributed to the genera Cladophialophora,Cyphellophora,Exophiala,Fonsecaea,Phialophora,and Veronaea.A total of 45 fungi morphologically identified as Fonsecaea or Cladophialophora isolated from debris and thorns of living prickly plants in Brazil were processed for taxonomic *** analysis revealed that the isolates indeed belonged to the Chaetothyriales,but only rarely an agent of chromoblastomycosis was concerned;only two strains of *** and one *** were isolated from debris *** remaining isolates belonged to hitherto unknown molecular siblings of *** novel taxa are introduced.

Sentiment analysis to determine the impact of online messages on smokers' choices to use varenicline

Background: Social networks are a prominent component of online smoking cessation interventions. This study applied sentiment analysis-a data processing technique that codes textual data for emotional polarity-to examine how exposure to messages about the cessation drug varenicline affects smokers' decision making around its use. Methods: Data were from QuitNet, an online social network dedicated to smoking cessation and relapse prevention. Self-reported medication choice at registration and at 30 days was coded among new QuitNet registrants who participated in at least one forum discussion mentioning varenicline between January 31, 2005 and March 9, 2008. Commercially available software was used to code the sentiment of forum messages mentioning varenicline that occurred during this time frame. Logistic regression analyses examined whether forum message exposure predicted medication choice. Results: The sample of 2132 registrants comprised mostly women (78.3%), white participants (83.4%), averaged 41.2 years of age (SD = 10.9), and smoked on average 21.5 (SD = 9.7) cigarettes/day. After adjusting for potential confounders, as exposure to positive varenicline messages outweighed negative messages, the odds of switching to varenicline (odds ratio = 2.05, 95% confidence interval = 1.66 to 2.54) and continuing to use varenicline (odds ratio = 2.46, 95% confidence interval = 1.96 to 3.10) statistically significantly increased. Conclusions: Sentiment analysis is a useful tool for analyzing text-based data to examine their impact on behavior change. Greater exposure to positive sentiment in online conversations about varenicline is associated with a greater likelihood that smokers will choose to use varenicline in a quit attempt. © The Author 2013. Multilevel intervention research: Lessons learned and pathways forward

This summary reflects on this monograph regarding multilevel intervention (MLI) research to 1) assess its added value;2) discuss what has been learned to date about its challenges in cancer care delivery;and 3) identify specific ways to improve its scientific soundness, feasibility, policy relevance, and research agenda. The 12 submitted chapters, and discussion of them at the March 2011 multilevel meeting, were reviewed and discussed among the authors to elicit key findings and results addressing the questions raised at the outset of this effort. MLI research is underrepresented as an explicit focus in the cancer literature but may improve implementation of studies of cancer care delivery if they assess contextual, organizational, and environmental factors important to understanding behavioral and/or system-level interventions. The field lacks a single unifying theory, although several psychological or biological theories are useful, and an ecological model helps conceptualize and communicate interventions. MLI research designs are often complex, involving nonlinear and nonhierarchical relationships that may not be optimally studied in randomized designs. Simulation modeling and pilot studies may be necessary to evaluate MLI interventions. Measurement and evaluation of team and organizational interventions are especially needed in cancer care, as are attention to the context of health-care reform, eHealth technology, and genomics-based medicine. Future progress in MLI research requires greater attention to developing and supporting relevant metrics of level effects and interactions and evaluating MLI interventions. MLI research holds an unrealized promise for understanding how to improve cancer care delivery.
