Frequency of HLA-A*03 associates with HIV-1 infection in a Chinese cohort

During the early mid-1990s, a number of rural farmers across central China were employed to the unregulated plasma- selling-activity and many of them were infected by HIV-1. However, aids progression in the former blood donors （FBDs） is various. The aim of this study is to assess human leukocyte antigen （HLA） class I allele distribution in FBDs and evaluate its association with HIV-1 infection and disease progression. A total of 353 FBDs were enrolled in the cohort including 294 ART na＇fve HIV-1 seropositive and 59 HIV-1 seronegative age-matched subjects. The viral load and CD4/CD8 T cell counts were assessed in all subjects. Compared with HIV-seropositive group, the frequency of HLA-A＊03 in control was significantly higher. After classifying the HLA-B alleles of the subjects according to the presence of Bw4/Bw6 serological epitopes, detri- mental effect of HLA Bw6/Bw6 homozygosity was also confirmed in the HIV-seropositive subjects. This study provides nov- el evidence on HLA class I allele distribution and association of HLA-A＊03 frequency with HIV-1 infection and viremia in the HIV-1 infected FBDs, which may throw light on intervention strategy for the HIV-1 infection and our understanding how host immunity and genetic background affect HIV infection and aids progression.

Antiviral treatment among pregnant women with chronic hepatitis B

Objective. To describe the antiviral treatment patterns for chronic hepatitis B (CHB) among pregnant and nonpregnant women.;Methods. Using 2011 MarketScan claims, we calculated the rates of antiviral treatment among women (aged 10-50 years) with CHB. We described the pattern of antiviral treatment during pregnancy and ≥1 month after delivery.;Results. We identified 6274 women with CHB during 2011. Among these, 64 of 507 (12.6%) pregnant women and 1151 of 5767 (20.0%) nonpregnant women received antiviral treatment (P < 0.01). Pregnant women were most commonly prescribed tenofovir (73.4%) and lamivudine (21.9%);nonpregnant women were most commonly prescribed tenofovir (50.2%) and entecavir (41.3%) (P < 0.01). Among 48 treated pregnant women with an identifiable delivery date, 16 (33.3%) were prescribed an antiviral before pregnancy and continued treatment for at least one month after delivery;14 (29.2%) started treatment during the third trimester and continued at least one month after delivery.;Conclusion. Among this insured population, pregnant women with CHB received an antiviral significantly less often than nonpregnant women. The most common antiviral prescribed for pregnant women was tenofovir. These data provide a baseline for assessing changes in treatment patterns with anticipated increased use of antivirals to prevent breakthrough perinatal hepatitis B virus infection. © 2014 Lin Fan et al.

Impact of HIV/sexually transmitted infection testing on risky sexual behaviors among men who have sex with men in Langfang, China

Background Men who have sex with men （MSM） in China remain at high risk for HIV infection, the proportion of reported HIV/aids cases that occurred among MSM rose greatly from 2005 to 2011. HIV testing and counseling is a critical HIV prevention strategy among HIV related high-risk population, including MSM in China. This article aimed to assess the association between receiving HIV testing and high-risk sexual behaviors among MSM in Langfang, Hebei Province, China. Methods Between September and November 2007, 233 MSM were recruited to receive an HIV testing intervention. Face-to-face interviews were conducted before HIV testing and 3 months later HIV-related risk behaviors were assessed. Serological testing for HIV and other sexually transmitted infections （STIs） was performed. Results Of the recruited 233 MSM, 200 completed follow-up. Baseline prevalence was 7.8% for HIV, 21.0% for syphilis, 15.8% for gonorrhea, and 5.0% for chlamydia. Multivariate analysis indicated that inconsistent condom use （OR=7.9, 95% CI： 0.9-66.7, P=0.059） and bleeding during anal sex （OR=5.9, 95% Cl： 1.3-26.2, P=0.019） were risk factors for HIV infection, and group sex （OR=6.6, 95% Cl： 2.2-19.7, P=0.001） was a risk factor for syphilis infection at baseline. At 3 months follow-up, among STI-positive MSM, self-reported anal sex fell from 73.1% to 38.5% （P 〈0.001）; group sex fell from 19.2% to 5.8% （P 〈0.001）; and bleeding during anal sex fell from 23.1% to 5.8% （P 〈0.001）. Among STI-negative MSM, the frequency of one-night stands fell from 32.5% to 17.2% （P 〈0.001）, and oral sex rose from 57% to 78.5% （P 〈0.001）. STI-positive MSM were less likely to engage in anal sex compared to STI-negative MSM （X^2=5.189, P=0.023）. Conclusions HIV testing is an important intervention strategy among MSM. HIV testing services among MSM need to be scaled up, along with comprehensive, tailored interventions including condom promotion and STI treatment.

Purpose. To examine perceptions of phase-I human immunodeficiency virus (HIV) vaccine trial participation among African-Americans and Hispanics in San Francisco, California.;Design. Qualitative, semistructured interviews.;Setting. San Francisco Department of Health.;Participants. Thirty-six African-American and Hispanic men and women, 18 to 50 years of age, residing in the San Francisco Bay Area.;Method. Purposive sampling using advertisements, community-based organization rosters, and snowball referrals. Thematic analysis of transcripts identified salient themes and patterns.;Conclusion. Historically, African-Americans and Hispanics in the United States have had limited participation in HIV trials. Understanding their perceptions of HIV biomedical research, identifying facilitators and barriers to participation, addressing misinformation about HIV, distorted risk perceptions, HIV stigma, and providing accessible opportunities to participate are imperative to ensure health equity and generalizability of findings. Participants viewed participation in HIV research as important;however, they held that HIV was not a health priority given limited awareness about HIV research or beliefs that only infected or high-risk persons were eligible for participation. Altruism and personal gain, trustworthy trial staff, convenient schedules and facilities, and involvement of trusted community groups in recruitment were perceived to motivate participants. Concerns about the social consequences of participating in HIV research and product-related side effects were seen as discouraging participation. Limitations include the possibility that participants in interview research have more favorable views of biomedical research than those who refuse to participate. The preventative effects of antiretroviral therapy for people with HIV have been debated since they were first raised. Models commenced studying the preventive effects of treatment in the 1990s, prior to initial public reports. However, the outcomes of the preventive effects of antiretroviral use were not consistent. Some outcomes of dynamic models were based on unfeasible assumptions, such as no consideration of drug resistance, behavior disinhibition, or economic inputs in poor countries, and unrealistic input variables, for example, overstated initiation time, adherence, coverage, and efficacy of treatment. This paper reviewed dynamic mathematical models to ascertain the complex effects of ART on HIV *** review discusses more conservative inputs and outcomes relative to antiretroviral use in HIV infections in dynamic mathematical models. Heterosexual transmission of HIV and related risk factors among serodiscordant couples in Henan province, China

Infection with human immunodeficiency virus （HIV） disrupts the balance among yT cell subsets, with increasing Vo1＋ cells and substantial depletion of circulating Vo2＋ cells. Depletion is an indirect effect of HIV in CD4-negative Vo2 cells, but is specific for phosphoantigen-responsive subpopulations identified by the Vy2-Jy1.2 （also called Vy9-JyP） T cell receptor rearrangement. The extent of cell loss and recovery is related closely to clinical status, with highest levels of functional V cells present in virus controllers （undetectable viremia in the absence of antiretroviral therapy）. We review the mechanisms and clinical consequences for V cell depletion in HIV disease. We address the question of whether HIV-mediated V cell depletion, despite being an indirect effect of infection, is an important part of the immune evasion strategy for this virus. The important roles for V cells, as effectors and immune regulators, identify key mechanisms affected by HIV and show the strong relationships between V62 cell loss and immunodeficiency disease. This field is moving toward immune therapies based on targeting V cells and we now have clear goals and expectations to Ruide interventional clinical trials.
