Summary Background Across low-income and middle-income countries (LMICs), one in ten deaths in children younger than 5 years is attributable to diarrhoea. The substantial between-country variation in both diarrhoea incidence and mortality is attributable to interventions that protect children, prevent infection, and treat disease. Identifying subnational regions with the highest burden and mapping associated risk factors can aid in reducing preventable childhood *** We used Bayesian model-based geostatistics and a geolocated dataset comprising 15 072 746 children younger than 5 years from 466 surveys in 94 LMICs, in combination with findings of the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2017, to estimate posterior distributions of diarrhoea prevalence, incidence, and mortality from 2000 to 2017. From these data, we estimated the burden of diarrhoea at varying subnational levels (termed units) by spatially aggregating draws, and we investigated the drivers of subnational patterns by creating aggregated risk factor *** The greatest declines in diarrhoeal mortality were seen in south and southeast Asia and South America, where 54·0% (95% uncertainty interval [UI] 38·1-65·8), 17·4% (7·7-28·4), and 59·5% (34·2-86·9) of units, respectively, recorded decreases in deaths from diarrhoea greater than 10%. Although children in much of Africa remain at high risk of death due to diarrhoea, regions with the most deaths were outside Africa, with the highest mortality units located in Pakistan. Indonesia showed the greatest within-country geographical inequality; some regions had mortality rates nearly four times the average country rate. Reductions in mortality were correlated to improvements in water, sanitation, and hygiene (WASH) or reductions in child growth failure (CGF). Despite substantial declines since 2000, lower respiratory infections (LRIs), diarrhoeal diseases, and malaria remain among the leading causes of nonfatal and fatal disease burden for children under 5 years of age (under 5), primarily in sub-Saharan Africa (SSA). The spatial burden of each of these diseases has been estimated subnationally across SSA, yet no prior analyses have examined the pattern of their combined burden. Here we synthesise subnational estimates of the burden of LRIs, diarrhoea, and malaria in children under-5 from 2000 to 2017 for 43 sub-Saharan countries. Some units faced a relatively equal burden from each of the three diseases, while others had one or two dominant sources of unit-level burden, with no consistent pattern geographically across the entire subcontinent. Using a subnational counterfactual analysis, we show that nearly 300 million DALYs could have been averted since 2000 by raising all units to their national average. Our findings are directly relevant for decision-makers in determining which and targeting where the most appropriate interventions are for increasing child survival. © 2022, The Author(s).

The Chaplygin gas as a model for modified teleparallel gravity?

This paper explores the possibility of treating the exotic Chaplygin-gas (CG) fluid model as some manifestation of an f (T) gravitation. To this end, we use the different cosmological CG equations of state, compare them with the equation of state for the modified teleparallel gravity and reconstruct the corresponding Lagrangian densities. We then explicitly derive the equation of state parameter of the torsion fluid wT and study its evolution for vacuum-torsion, radiation-torsion, dust-torsion, stiff fluid-torsion and radiation-dust-torsion multi-fluid systems. The obtained Lagrangians have, in general, matter dependence due to the matter-torsion coupling appearing in the energy density and pressure terms of the modified teleparallel gravity theory. For the simplest CG models, however, it is possibly to reconstruct f (T) Lagrangians that depend explicitly on the torsion scalar T only. The preliminary results show that, in addition to providing Chaplygin-gas-like solutions to the modified teleparallel gravitation, which naturally behave like dark matter and dark energy at early and late times respectively, the technique can be used to overcome some of the challenges attributed to the CG cosmological alternative. BACKGROUND:Plasmodium falciparum multidrug resistance transporter 1 (Pfmdr1) gene mutations are associated with altered response to artemisinin-based combination therapy (ACT), particularly the combinations containing the partner drugs lumefantrine and amodiaquine (i.e., artemether-lumefantrine [AL] and artesunate-amodiaquine [ASAQ]). Past studies of Pfmdr1 single nucleotide polymorphisms (SNPs) at codons 86, 184, and 1246 have shown different responses to AL and ASAQ. METHODS:To determine whether infection with parasites carrying specific Pfmdr1 SNPs leads to increased risk of recurrent parasitaemia (recrudescent or new infection), data from 3,915 samples from 16 therapeutic efficacy studies from 13 African countries between 2013 and 2019 were analysed. RESULTS:Patients treated with AL and infected with parasites carrying Pfmdr1 N86 were at greater risk of recurrent infection than those whose parasites carried 86Y. After treatment with ASAQ, individuals infected with parasites that carried Pfmdr1 86Y were more likely to experience a recurrent infection. CONCLUSIONS:These results support prior studies that suggested: (1) patients given AL and infected with parasites carrying N86 were more likely to experience a recurrent infection; (2) patients given ASAQ and infected with parasites carrying 86Y were more likely to experience a recurrent infection. These findings suggest that ACT and Pfmdr1 genotype may influence outcome after Plasmodium falciparum infection.

Background: Land use/land cover (LU/LC) change is the challenging and continuous drivers of environment change. Understanding the rate and process of change is, therefore, basic for managing the environment. This study was intended to analyze the LU/LC changes from 1985 to 2015 periods, and predict the situation to 2030 and 2045 in the Andassa watershed of Blue Nile basin, Ethiopia. The hybrid classification technique for extracting thematic information from satellite images and CA-Markov model for prediction of LU/LC were employed. Results: Cultivated land was expanding from 62.7% in 1985 to 73.1% in 2000 and to 76.8% in 2015. The area of built-up also slightly increased (0.1–1.1%) between 1985 and 2015 periods. In contrast, forest, shrubland and grassland were reduced from 3.5 to 1.9%, 26.2 to 15.3% and 7.6 to 4.9% in 1985 and 2015 periods, respectively. The increase of cultivated land and built-up area, and the withdrawing of forest, shrubland and grassland were further continued in 2030 and 2045 periods. Conclusion: Significant amount of LU/LC conversions had occurred in the watershed from 1985 to 2015 periods, and expected to continue in 2030 and 2045 periods. Thus, appropriate interventions to revert the trends are very much critical. © The Author(s) 2017.

Mycobacterium tuberculosis in central Ethiopia: drug sensitivity patterns and association with genotype

Drug resistance tuberculosis (TB) and the emergence of multidrug resistant (MDR) isolates are significant concerns regarding TB control programs in several countries. This study was undertaken to evaluate the drug sensitivity of Mycobacterium tuberculosis and to assess its association with strains and lineages of M. tuberculosis. A total of 279 M. tuberculosis strains isolated from Central Ethiopia were tested for their drug sensitivity patterns to first line TB drugs using the conventional proportion method on Löwenstein Jensen media. The association between drug sensitivity and strain type was assessed on 263 isolates of the 279 isolates. Of the 268 M. tuberculosis isolates obtained from new cases, 209 (78%) were susceptible to first line TB drugs, and 59 (22.2%) bacterial isolates were resistant to at least one of the first line drugs. The highest mono-resistance (7.5%) pertained to streptomycin (STM). Remarkably, seven of eleven isolates (63.6%) previous treatment for TB were resistant to at least one of the first line drugs. The prevalence of MDR-TB was 1.5% (4/268) for newly identified TB cases, all of which were members of the Euro-American Lineage. There was no statistically significant association (P > 0.05) between drug sensitivity, and either strains, sub-lineages or main lineages of M. tuberculosis. A significant proportion of M. tuberculosis was resistant to at least one first line anti-TB drug. Moreover, the frequencies of resistance to either isoniazid or rifampicin were high compared to data that were previously reported in some part of the country. © 2017 The Authors

Land use/land cover analysis and ecosystem services valuation in the central highlands of Ethiopia

We assessed the land use/land cover (LULC) dynamics and its associated changes in ecosystem service values for the Toke Kutaye district (ca 72,700 ha) in the central highlands of Ethiopia. Four satellite images of the study area, distributed between 1973 and 2014 were analyzed using Arc GIS software to assess the LULC changes of the area. In addition, we carried out an ecosystem services valuation, based on the identified LULC types and on the ecosystem services values database. Six LULC types were identified in the study area (forest, shrub/bush land, grassland, cultivated land, settlement, and bare land). Forest cover decreased 83% between 1973 and 2014. Settlement and cultivated land increased by 471 and 222%, respectively. The overall ecosystem services value reduced by 68%, and this was mainly due to deforestation. The use of LULC analysis combined to ecosystem services valuation improved our understanding of deforestation trends in the study area. We recommend the integration of LULC analysis and ecosystem service valuation as a tool that could better inform land managers and policy-makers in their decision-making processes. © 2016 Informa UK Limited, trading as Taylor & Francis Group.

Alternative beneficiation of tantalite and removal of radioactive oxides from Ethiopian Kenticha pegmatite–spodumene ores

The beneficiation methods for Ethiopian Kenticha pegmatite–spodumene ores were assessed through mineralogical and quantitative analyses with X-ray diffraction（XRD） and energy-dispersive X-ray fluorescence（EDXRF）. The tantalite in the upper zone of the Kenticha pegmatite–spodumene deposit is 58.7wt% higher than that in the inner zone. XRD analysis revealed that the upper zone is dominated by manganocolumbite, whereas the inner zone is predominantly tantalite-Mn. Repeated cleaning and beneficiation of the upper-zone ore resulted in concentrate compositions of 57.34wt% of Ta_2O_5 and 5.41wt% of Nb_2O_5. Washing the tantalite concentrates using 1vol% KOH and 1 M H_2SO_4 led to the removal of thorium and uranium radioactive oxides from the concentrate. The findings of this study suggest that the beneficiation and alkaline washing of Kenticha pegmatite–spodumene ores produce a high-grade export-quality tantalite concentrate with negligible radioactive oxides.
