Chronic care manages long-term, progressive conditions, while acute care addresses short-term conditions. Chronic conditions increasingly strain health systems, which are often unprepared for these demands. This study examines the burden of conditions requiring acute versus chronic care, including sequelae. Conditions and sequelae from the Global Burden of Diseases Study 2019 were classified into acute or chronic care categories. Data were analysed by age, sex, and socio-demographic index, presenting total numbers and contributions to burden metrics such as Disability-Adjusted Life Years (DALYs), Years Lived with Disability (YLD), and Years of Life Lost (YLL). Approximately 68% of DALYs were attributed to chronic care, while 27% were due to acute care. Chronic care needs increased with age, representing 86% of YLDs and 71% of YLLs, and accounting for 93% of YLDs from sequelae. These findings highlight that chronic care needs far exceed acute care needs globally, necessitating health systems to adapt accordingly. © 2025. The Author(s).

Correlation study of some physico-chemical parameters and benthic macroinvertebrates metrics on the ecological impacts of floriculture industries along Wedecha River, Debrezeit, Ethiopia

Objective: To assess the quality of water using biological and chemical information correlation study. Methods: A total of 6 sites, one reference site less impaired to represent natural conditions from upstream, and 5 impaired sites in the downstream were sampled for macroinvertebrates and physicochemical parameters. Results: In the polluted sites with high nutrient enrichment and organic loading, only organisms with special physiological and morphological adaptations were found, such as Hydrobiidae, Physidae and Viviparidae snails. Physicochemical parameters: electrical conductivity, chemical oxygen demand, biological oxygen demand, sulphate, orthophosphate and nitrate-nitrogen and metrics: percent of Mollusca, percent of Physidae and percent of dominant taxa were used for monitoring, and expected to have strong positive relation with increasing disturbance. Conclusions: Therefore, floriculture industries wastewater discharged to the nearby rivers has enormous effect on the degradation of the ecosystem. To sustain the ecological conditions of the nearby rivers, wastewater treatment and environmental audit were suggested. Environmental assessment and environmental audit enable the floriculturist to keep humans and the environment safe. Taking care of workers, soil, water and the environment has to be seen with great care and caution because it is difficult to maintain a healthy community and carry out development in a degraded environment.

Salting-Out Assisted Liquid-Liquid Extraction Combined with HPLC for Quantitative Extraction of Trace Multiclass Pesticide Residues from Environmental Waters

In this study, salting-out assisted liquid-liquid extraction combined with high performance liquid chromatography diode array detector (SALLE-HPLC-DAD) method was developed and validated for simultaneous analysis of carbaryl, atrazine, propazine, chlorothalonil, dimethametryn and terbutryn in environmental water samples. Parameters affecting the extraction efficiency such as type and volume of extraction solvent, sample volume, salt type and amount, centrifugation speed and time, and sample pH were optimized. Under the optimum extraction conditions the method was linear over the range of 10 - 100 μg/L (carbaryl), 8 - 100 μg/L (atarzine), 7 - 100 μg/L (propazine) and 9 - 100 μg/L (chlorothalonil, terbutryn and dimethametryn) with correlation coefficients (R2) between 0.99 and 0.999. Limits of detection and quantification ranged from 2.0 to 2.8 μg/L and 6.7 to 9.5 μg/L, respectively. The extraction recoveries obtained for ground, lake and river waters were in a range of 75.5% to 106.6%, with the intra-day and inter-day relative standard deviation lower than 3.4% for all the target analytes. All of the target analytes were not detected in these samples. Therefore, the proposed SALLE-HPLC-DAD method is simple, rapid, cheap and environmentally friendly for the determination of the aforementioned herbicides, insecticide and fungicide residues in environmental water samples.

Background: Enset Xanthomonas wilt (EXW) was first reported in 1939 and continues to threaten the sustainability of farming systems in south and southwestern parts of Ethiopia. The present study was conducted in the central zones of southern Ethiopia to assess farmers' knowledge and perception about EXW, its etiology and mode of transmission, and its implications for the management of EXW. Methods: A survey was conducted in 240 households across Hadiya, Kembata-Tembaro and Wolaita zones of southern Ethiopia using focus group discussions and a structured questionnaire to assess farmers' perceptions of causes and modes of EXW transmission, and their knowledge on symptom identification. In addition, EXW prevalence, incidence and severity were determined for each zone. Data were analyzed through descriptive statistics. Results: The results showed that a significant number of farmers are aware of EXW, its symptoms, etiology and transmission and spread, but they are not able to readily relate modes of spread to control methods. Since 2002, EXW became prominent in Hadiya, with the highest EXW incidence and severity, followed by Wolaita, and Kembata-Tembaro. Farmers identified EXW as the major cause for declining production and productivity of enset in the region. Conclusion: EXW has spread widely and rapidly in southern Ethiopia, with significant socioeconomic impacts in smallholders' livelihoods. There is a need for developing knowledge-based strategies and awareness-raising campaign for EXW management. © 2017 The Author(s).

Background: Previous attempts to characterise the burden of chronic respiratory diseases have focused only on specific disease conditions, such as chronic obstructive pulmonary disease (COPD) or asthma. In this study, we aimed to characterise the burden of chronic respiratory diseases globally, providing a comprehensive and up-to-date analysis on geographical and time trends from 1990 to 2017. Methods: Using data from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2017, we estimated the prevalence, morbidity, and mortality attributable to chronic respiratory diseases through an analysis of deaths, disability-adjusted life-years (DALYs), and years of life lost (YLL) by GBD super-region, from 1990 to 2017, stratified by age and sex. Specific diseases analysed included asthma, COPD, interstitial lung disease and pulmonary sarcoidosis, pneumoconiosis, and other chronic respiratory diseases. We also assessed the contribution of risk factors (smoking, second-hand smoke, ambient particulate matter and ozone pollution, household air pollution from solid fuels, and occupational risks) to chronic respiratory disease-attributable DALYs. Findings: In 2017, 544·9 million people (95% uncertainty interval [UI] 506·9–584·8) worldwide had a chronic respiratory disease, representing an increase of 39·8% compared with 1990. Chronic respiratory disease prevalence showed wide variability across GBD super-regions, with the highest prevalence among both males and females in high-income regions, and the lowest prevalence in sub-Saharan Africa and south Asia. The age-sex-specific prevalence of each chronic respiratory disease in 2017 was also highly variable geographically. Chronic respiratory diseases were the third leading cause of death in 2017 (7·0% [95% UI 6·8–7·2] of all deaths), behind cardiovascular diseases and neoplasms. Background: The COVID-19 pandemic highlighted gaps in health surveillance systems, disease prevention, and treatment globally. Among the many factors that might have led to these gaps is the issue of the financing of national health systems, especially in low-income and middle-income countries (LMICs), as well as a robust global system for pandemic preparedness. We aimed to provide a comparative assessment of global health spending at the onset of the pandemic;characterise the amount of development assistance for pandemic preparedness and response disbursed in the first 2 years of the COVID-19 pandemic;and examine expectations for future health spending and put into context the expected need for investment in pandemic preparedness. Methods: In this analysis of global health spending between 1990 and 2021, and prediction from 2021 to 2026, we estimated four sources of health spending: development assistance for health (DAH), government spending, out-of-pocket spending, and prepaid private spending across 204 countries and territories. We used the Organisation for Economic Co-operation and Development (OECD)'s Creditor Reporting System (CRS) and the WHO Global Health Expenditure Database (GHED) to estimate spending. We estimated development assistance for general health, COVID-19 response, and pandemic preparedness and response using a keyword search. Health spending estimates were combined with estimates of resources needed for pandemic prevention and preparedness to analyse future health spending patterns, relative to need. Findings: In 2019, at the onset of the COVID-19 pandemic, US$9·2 trillion (95% uncertainty interval [UI] 9·1–9·3) was spent on health worldwide. We found great disparities in the amount of resources devoted to health, with high-income countries spending $7·3 trillion (95% UI 7·2–7·4) in 2019;293·7 times the $24·8 billion (95% UI 24·3–25·3) spent by low-income countries in 2019. Background: The causes for immune-mediated inflammatory diseases (IMIDs) are diverse and the incidence trends of IMIDs from specific causes are rarely studied. The study aims to investigate the pattern and trend of IMIDs from 1990 to 2019. Methods: We collected detailed information on six major causes of IMIDs, including asthma, inflammatory bowel disease, multiple sclerosis, rheumatoid arthritis, psoriasis, and atopic dermatitis, between 1990 and 2019, derived from the Global Burden of Disease study in 2019. The average annual percent change (AAPC) in number of incidents and age standardized incidence rate (ASR) on IMIDs, by sex, age, region, and causes, were calculated to quantify the temporal trends. Findings: In 2019, rheumatoid arthritis, atopic dermatitis, asthma, multiple sclerosis, psoriasis, inflammatory bowel disease accounted 1.59%, 36.17%, 54.71%, 0.09%, 6.84%, 0.60% of overall new IMIDs cases, respectively. The ASR of IMIDs showed substantial regional and global variation with the highest in High SDI region, High-income North America, and United States of America. Throughout human lifespan, the age distribution of incident cases from six IMIDs was quite different. Globally, incident cases of IMIDs increased with an AAPC of 0.68 and the ASR decreased with an AAPC of −0.34 from 1990 to 2019. The incident cases increased across six IMIDs, the ASR of rheumatoid arthritis increased (0.21, 95% CI 0.18, 0.25), while the ASR of asthma (AAPC = −0.41), inflammatory bowel disease (AAPC = −0.72), multiple sclerosis (AAPC = −0.26), psoriasis (AAPC = −0.77), and atopic dermatitis (AAPC = −0.15) decreased. The ASR of overall and six individual IMID increased with SDI at regional and global level. Countries with higher ASR in 1990 experienced a more rapid decrease in ASR. Incompressible Flow and Heat Transfer over a Plate: A Hybrid Integral Domain-Discretized Numerical Procedure

This work deals with incompressible two-dimensional viscous flow over a semi-infinite plate ac-cording to the approximations resulting from Prandtl boundary layer theory. The governing non-linear coupled partial differential equations describing laminar flow are converted to a self-simi- lar type third order ordinary differential equation known as the Falkner-Skan equation. For the purposes of a numerical solution, the Falkner-Skan equation is converted to a system of first order ordinary differential equations. These are numerically addressed by the conventional shooting and bisection methods coupled with the Runge-Kutta technique. However the accompanying energy equation lends itself to a hybrid numerical finite element-boundary integral application. An appropriate complementary differential equation as well as the Green second identity paves the way for the integral representation of the energy equation. This is followed by a finite element-type discretization of the problem domain. Based on the quality of the results obtained herein, a strong case is made for a hybrid numerical scheme as a useful approach for the numerical resolution of boundary layer flows and species transport. Thanks to the sparsity of the resulting coefficient matrix, the solution profiles not only agree with those of similar problems in literature but also are in consonance with the physics they represent.
