Despite substantial declines since 2000, lower respiratory infections (LRIs), diarrhoeal diseases, and malaria remain among the leading causes of nonfatal and fatal disease burden for children under 5 years of age (under 5), primarily in sub-Saharan Africa (SSA). The spatial burden of each of these diseases has been estimated subnationally across SSA, yet no prior analyses have examined the pattern of their combined burden. Here we synthesise subnational estimates of the burden of LRIs, diarrhoea, and malaria in children under-5 from 2000 to 2017 for 43 sub-Saharan countries. Some units faced a relatively equal burden from each of the three diseases, while others had one or two dominant sources of unit-level burden, with no consistent pattern geographically across the entire subcontinent. Using a subnational counterfactual analysis, we show that nearly 300 million DALYs could have been averted since 2000 by raising all units to their national average. Our findings are directly relevant for decision-makers in determining which and targeting where the most appropriate interventions are for increasing child survival. © 2022, The Author(s).

Background: To our knowledge, no previous study has examined the inter-relationship between frailty, dysglycaemia, and mortality in frail older adults with type 2 diabetes who are on insulin therapy. We used continuous glucose monitors (CGMs) to profile this patient population and determine the prognostic value of CGM metrics. We hypothesised that incremental frailty was associated with increased hypoglycaemia or time below range (TBR). Methods: HARE was a multicentre, prospective, observational cohort study with mortality hazard analysis carried out in four hospitals in Hong Kong. Eligible participants were community-living adults aged 70 years and older;had had type 2 diabetes for 5 years or more;were on insulin therapy;were frail;and were not hospitalised at the time of frailty assessment and CGM recording. Glucose control was characterised according to the Advanced Technologies and Treatments for Diabetes 2019 international consensus clinical targets. Frailty index was computed, and comprehensive frailty assessments and targeted serum metabolic profiling were performed. The Jonckheere-Terpstra test for trend was used to analyse frailty index tertiles and variables. Inter-relationships between CGM metrics and frailty, glycated haemoglobin A_1c (HbA_1c), and serum albumin were characterised using adjusted regression models. Survival analysis and Cox proportional hazard modelling were performed. Findings: Between July 25, 2018, and Sept 27, 2019, 225 participants were recruited, 222 of whom had CGMs fitted and 215 of whom had analysable CGM data (190 were frail, 25 were not frail). Incremental frailty was associated with older age, greater HbA_1c, worse renal function, and history of stroke. Eight of 11 CGM metrics were significantly associated with frailty. Decreased time in range (TIR;glucose concentration 3·9–10·0 mmol/L) and increased time above range (TAR) metrics were strongly correlated with increased frailty and hyperglycaemia,

Effects of nongaussian diffusion on "isotropic diffusion"measurements: An ex-vivo microimaging and simulation study

Designing novel diffusion-weighted pulse sequences to probe tissue microstructure beyond the conventional Stejskal-Tanner family is currently of broad interest. One such technique, multidimensional diffusion MRI, has been recently proposed to afford model-free decomposition of diffusion signal kurtosis into terms originating from either ensemble variance of isotropic diffusivity or microscopic diffusion anisotropy. This ability rests on the assumption that diffusion can be described as a sum of multiple Gaussian compartments, but this is often not strictly fulfilled. The effects of nongaussian diffusion on single shot isotropic diffusion sequences were first considered in detail by de Swiet and Mitra in 1996. They showed theoretically that anisotropic compartments lead to anisotropic time dependence of the diffusion tensors, which causes the measured isotropic diffusivity to depend on gradient frame orientation. Here we show how such deviations from the multiple Gaussian compartments assumption conflates orientation dispersion with ensemble variance in isotropic diffusivity. Second, we consider additional contributions to the apparent variance in isotropic diffusivity arising due to intracompartmental kurtosis. These will likewise depend on gradient frame orientation. We illustrate the potential importance of these confounds with analytical expressions, numerical simulations in simple model geometries, and microimaging experiments in fixed spinal cord using isotropic diffusion encoding waveforms with 7.5 ms duration and 3000 mT/m maximum amplitude. Despite the crucial role of interfacial perpendicular magnetic anisotropy in a Co(Fe)/MgO-based magnetic tunnel junction, the underlying mechanism is still being debated. Here, we report an anatomical study of the oxygen and Mg effect on Pt/Co bilayers through repeated in situ anomalous Hall effect measurements, controlled oxygen exposure, and Mg deposition in an ultrahigh vacuum system. We found that chemisorbed oxygen not only quenches the effective magnetic moment of the Co surface layer, but also softens its magnetic anisotropy. However, a subsequent Mg dusting on the oxygen preexposed Pt/Co surface can recover the magnetic anisotropy. The ab initio calculations on the exchange splitting and orbital hybridization near the Fermi level give a clear physical explanation of the experimental observations. Our results suggest that the Co(Fe)-O-M bond plays a more important role than the widely perceived Co(Fe)-O bond does in realizing interfacial perpendicular magnetic anisotropy in Co(Fe)/MgO heterostructures.

The proliferation of relatively affordable off-the-shelf drones offers great opportunities for wildlife monitoring and conservation. Similarly the recent reduction in cost of thermal infrared cameras also offers new promise in this field, as they have the advantage over conventional RGB cameras of being able to distinguish animals based on their body heat and being able to detect animals at night. However, the use of drone-mounted thermal infrared cameras comes with several technical challenges. In this paper we address some of these issues, namely thermal contrast problems due to heat from the ground, absorption and emission of thermal infrared radiation by the atmosphere, obscuration by vegetation, and optimizing the flying height of drones for a best balance between covering a large area and being able to accurately image and identify animals of interest. We demonstrate the application of these methods with a case study using field data, and make the first ever detection of the critically endangered riverine rabbit (Bunolagus monticularis) in thermal infrared data.
