Background: Diabetes is one of the leading causes of death and disability worldwide, and affects people regardless of country, age group, or sex. Using the most recent evidentiary and analytical framework from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD), we produced location-specific, age-specific, and sex-specific estimates of diabetes prevalence and burden from 1990 to 2021, the proportion of type 1 and type 2 diabetes in 2021, the proportion of the type 2 diabetes burden attributable to selected risk factors, and projections of diabetes prevalence through 2050. Methods: Estimates of diabetes prevalence and burden were computed in 204 countries and territories, across 25 age groups, for males and females separately and combined;these estimates comprised lost years of healthy life, measured in disability-adjusted life-years (DALYs;defined as the sum of years of life lost [YLLs] and years lived with disability [YLDs]). We used the Cause of Death Ensemble model (CODEm) approach to estimate deaths due to diabetes, incorporating 25 666 location-years of data from vital registration and verbal autopsy reports in separate total (including both type 1 and type 2 diabetes) and type-specific models. Other forms of diabetes, including gestational and monogenic diabetes, were not explicitly modelled. Total and type 1 diabetes prevalence was estimated by use of a Bayesian meta-regression modelling tool, DisMod-MR 2.1, to analyse 1527 location-years of data from the scientific literature, survey microdata, and insurance claims;type 2 diabetes estimates were computed by subtracting type 1 diabetes from total estimates. Mortality and prevalence estimates, along with standard life expectancy and disability weights, were used to calculate YLLs, YLDs, and DALYs. We assessed the association between a genome-wide polygenic risk score (PRS) developed for lung adenocarcinoma (LUAD) risk and mutation on the epidermal growth factor receptor (EGFR) gene in 998 East Asian never-smoking female LUAD cases (518 EGFR-positive; 480 EGFR-negative) and 4,544 never-smoking controls using case-case and multinomial regression analyses. We found that the PRS was more strongly associated with EGFR-positive LUAD compared to EGFR-negative LUAD, where the association between the fourth quartile of the PRS and EGFR-positive LUAD (OR=8.63, 95% CI:5.67, 13.14) was significantly higher than the association between the fourth quartile of the PRS with EGFR-negative LUAD (OR=3.50, 95% CI: 2.44, 5.00) (p-heterogeneity=3.66x10). Our findings suggest that germline genetic susceptibility may be differentially associated with LUAD in never-smoking female East Asian patients depending on the cancer's mutation status, which may have important public health and clinical implications.

This technical note focuses on the control of a quadrotor unmanned aerial vehicle (UAV) tethered to the ground. The control objective is to stabilize the UAV to the desired position while ensuring that the cable remains taut at all times. A cascade control scheme is proposed. The inner loop controls the attitude of the UAV. The outer loop gives the attitude reference to the inner loop, and is designed so that (i) the gravity force is compensated, (ii) the cable is taut at all times, and (iii) the trajectory of the UAV follows the geodesic path. To prove asymptotic stability, small gain arguments are used. The control scheme is augmented with a reference governor to enforce constraints. Copyright © 2019, The Authors. All rights reserved.

Recent research on joint source-channel coding (JSCC) for wireless communications has achieved great success owing to the employment of deep learning (DL). However, the existing work on DL based JSCC usually trains the designed network to operate under a specific signal-to-noise ratio (SNR) regime, without taking into account that the SNR level during the deployment stage may differ from that during the training stage. A number of networks are required to cover the scenario with a broad range of SNRs, which is computational inefficiency (in the training stage) and requires large storage. To overcome these drawbacks our paper proposes a novel method called Attention DL based JSCC (ADJSCC) that can successfully operate with different SNR levels during transmission. This design is inspired by the resource assignment strategy in traditional JSCC, which dynamically adjusts the compression ratio in source coding and the channel coding rate according to the channel SNR. This is achieved by resorting to attention mechanisms because these are able to allocate computing resources to more critical tasks. Instead of applying the resource allocation strategy in traditional JSCC, the ADJSCC uses the channel-wise soft attention to scaling features according to SNR conditions. We compare the ADJSCC method with the state-of-the-art DL based JSCC method through extensive experiments to demonstrate its adaptability, robustness and versatility. Secure Dynamic State Estimation by Decomposing Kalman Filter

We consider the problem of estimating the state of a linear time-invariant Gaussian system in the presence of sparse integrity attacks. The attacker can control p out of m sensors and arbitrarily change the measurements. Under mild assumptions, we can decompose the optimal Kalman estimate as a weighted sum of local state estimates, each of which is derived using only the measurements from a single sensor. Furthermore, we propose a convex optimization based approach, instead of the weighted sum approach, to combine the local estimate into a more secure state estimate. It is shown that our proposed estimator coincides with the Kalman estimator with certain probability when all sensors are benign, and we provide a sufficient condition under which the estimator is stable against the (p, m)-sparse attack when p sensors are compromised. A numerical example is provided to illustrate the performance of the proposed state estimation scheme. © 2017

Chronic care manages long-term, progressive conditions, while acute care addresses short-term conditions. Chronic conditions increasingly strain health systems, which are often unprepared for these demands. This study examines the burden of conditions requiring acute versus chronic care, including sequelae. Conditions and sequelae from the Global Burden of Diseases Study 2019 were classified into acute or chronic care categories. Data were analysed by age, sex, and socio-demographic index, presenting total numbers and contributions to burden metrics such as Disability-Adjusted Life Years (DALYs), Years Lived with Disability (YLD), and Years of Life Lost (YLL). Approximately 68% of DALYs were attributed to chronic care, while 27% were due to acute care. Chronic care needs increased with age, representing 86% of YLDs and 71% of YLLs, and accounting for 93% of YLDs from sequelae. These findings highlight that chronic care needs far exceed acute care needs globally, necessitating health systems to adapt accordingly. © 2025. The Author(s).

This paper covers in depth functioning essence of the balancing electricity market in the Russian Federation in accordance with the current economic stimuli. Significant role of the prediction process for energy resources consumption has been noted, which has a direct influence on a pricing policy within the marked trade area. analysis of different factors, that impact execution accuracy of the one day in advance model, was conducted. Problem of the weakly defined and unknown parameters, that influence the performance of the balancing electricity market, is suggested to resolve by means of fuzzy neural controller. The paper presents its structure and settings, and possibility to scale it to use not only as an analyzer but as an executive element in the subsystems of electric power systems. High accuracy performance of the fuzzy neural controller was shown - around (0,5-3) %, that is much higher than methods already in use (for instance, based on a coefficient of decline/growth of power consumption).

The paper examines a possibility to optimize complex technological processes of energy resources transfer and consumption. Notes the importance of unidentified and weakly formalized factors consideration, which influence quality of the described object classes functioning. Suggests use of fuzzy neural networks and controllers on their basis for implementation of technological processes optimization of energy resources transfer and consumption at the expense of its dynamic accuracy forecast improvement. Presents algorithmic realization, that later on configured in the MATLAB simulated environment. Quantitative and qualitative parameters, which characterize fuzzy neural controller's usage quality, were received. Later, software environment elements are being described, which constructed on the basis of the suggested adaptive approach for complex technological processes optimization. Points out a possibility to conduct comparative study, and also simulation modeling implementation by way of presented solutions.

Background: The COVID-19 pandemic highlighted gaps in health surveillance systems, disease prevention, and treatment globally. Among the many factors that might have led to these gaps is the issue of the financing of national health systems, especially in low-income and middle-income countries (LMICs), as well as a robust global system for pandemic preparedness. We aimed to provide a comparative assessment of global health spending at the onset of the pandemic;characterise the amount of development assistance for pandemic preparedness and response disbursed in the first 2 years of the COVID-19 pandemic;and examine expectations for future health spending and put into context the expected need for investment in pandemic preparedness. Methods: In this analysis of global health spending between 1990 and 2021, and prediction from 2021 to 2026, we estimated four sources of health spending: development assistance for health (DAH), government spending, out-of-pocket spending, and prepaid private spending across 204 countries and territories. We used the Organisation for Economic Co-operation and Development (OECD)'s Creditor Reporting system (CRS) and the WHO Global Health Expenditure Database (GHED) to estimate spending. We estimated development assistance for general health, COVID-19 response, and pandemic preparedness and response using a keyword search. Health spending estimates were combined with estimates of resources needed for pandemic prevention and preparedness to analyse future health spending patterns, relative to need. Findings: In 2019, at the onset of the COVID-19 pandemic, US$9·2 trillion (95% uncertainty interval [UI] 9·1–9·3) was spent on health worldwide. We found great disparities in the amount of resources devoted to health, with high-income countries spending $7·3 trillion (95% UI 7·2–7·4) in 2019;293·7 times the $24·8 billion (95% UI 24·3–25·3) spent by low-income countries in 2019.
