Background: The COVID-19 pandemic highlighted gaps in health surveillance systems, disease prevention, and treatment globally. Among the many factors that might have led to these gaps is the issue of the financing of national health systems, especially in low-income and middle-income countries (LMICs), as well as a robust global system for pandemic preparedness. We aimed to provide a comparative assessment of global health spending at the onset of the pandemic;characterise the amount of development assistance for pandemic preparedness and response disbursed in the first 2 years of the COVID-19 pandemic;and examine expectations for future health spending and put into context the expected need for investment in pandemic preparedness. Methods: In this analysis of global health spending between 1990 and 2021, and prediction from 2021 to 2026, we estimated four sources of health spending: development assistance for health (DAH), government spending, out-of-pocket spending, and prepaid private spending across 204 countries and territories. We used the Organisation for Economic Co-operation and Development (OECD)'s Creditor Reporting System (CRS) and the WHO Global Health Expenditure database (GHED) to estimate spending. We estimated development assistance for general health, COVID-19 response, and pandemic preparedness and response using a keyword search. Health spending estimates were combined with estimates of resources needed for pandemic prevention and preparedness to analyse future health spending patterns, relative to need. Findings: In 2019, at the onset of the COVID-19 pandemic, US$9·2 trillion (95% uncertainty interval [UI] 9·1–9·3) was spent on health worldwide. We found great disparities in the amount of resources devoted to health, with high-income countries spending $7·3 trillion (95% UI 7·2–7·4) in 2019;293·7 times the $24·8 billion (95% UI 24·3–25·3) spent by low-income countries in 2019. Background: Neck pain is a highly prevalent condition that leads to considerable pain, disability, and economic cost. We present the most current estimates of neck pain prevalence and years lived with disability (YLDs) from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) by age, sex, and location, with forecasted prevalence to 2050. Methods: Systematic reviews identified population-representative surveys used to estimate the prevalence of and YLDs from neck pain in 204 countries and territories, spanning from 1990 to 2020, with additional data from opportunistic review. Medical claims data from Taiwan (province of China) were also included. Input data were pooled using DisMod-MR 2.1, a Bayesian meta-regression tool. Prevalence was forecast to 2050 using a mixed-effects model using Socio-demographic Index as a predictor and multiplying by projected population estimates. We present 95% UIs for every metric based on the 2·5th and 97·5th percentiles of 100 draws of the posterior distribution. Findings: Globally, in 2020, neck pain affected 203 million (95% uncertainty interval [UI] 163–253) people. The global age-standardised prevalence rate of neck pain was estimated to be 2450 (1960–3040) per 100 000 population and global age-standardised YLD rate was estimated to be 244 (165–346) per 100 000. The age-standardised prevalence rate remained stable between 1990 and 2020 (percentage change 0·2% [–1·3 to 1·7]). Globally, females had a higher age-standardised prevalence rate (2890 [2330–3620] per 100 000) than males (2000 [1600–2480] per 100 000), with the prevalence peaking between 45 years and 74 years in male and female sexes. By 2050, the estimated global number of neck pain cases is projected to be 269 million (219–322), with an increase of 32·5% (23·9–42·3) from 2020 to 2050. Background: The causes for immune-mediated inflammatory diseases (IMIDs) are diverse and the incidence trends of IMIDs from specific causes are rarely studied. The study aims to investigate the pattern and trend of IMIDs from 1990 to 2019. Methods: We collected detailed information on six major causes of IMIDs, including asthma, inflammatory bowel disease, multiple sclerosis, rheumatoid arthritis, psoriasis, and atopic dermatitis, between 1990 and 2019, derived from the Global Burden of Disease study in 2019. The average annual percent change (AAPC) in number of incidents and age standardized incidence rate (ASR) on IMIDs, by sex, age, region, and causes, were calculated to quantify the temporal trends. Findings: In 2019, rheumatoid arthritis, atopic dermatitis, asthma, multiple sclerosis, psoriasis, inflammatory bowel disease accounted 1.59%, 36.17%, 54.71%, 0.09%, 6.84%, 0.60% of overall new IMIDs cases, respectively. The ASR of IMIDs showed substantial regional and global variation with the highest in High SDI region, High-income North America, and United States of America. Throughout human lifespan, the age distribution of incident cases from six IMIDs was quite different. Globally, incident cases of IMIDs increased with an AAPC of 0.68 and the ASR decreased with an AAPC of −0.34 from 1990 to 2019. The incident cases increased across six IMIDs, the ASR of rheumatoid arthritis increased (0.21, 95% CI 0.18, 0.25), while the ASR of asthma (AAPC = −0.41), inflammatory bowel disease (AAPC = −0.72), multiple sclerosis (AAPC = −0.26), psoriasis (AAPC = −0.77), and atopic dermatitis (AAPC = −0.15) decreased. The ASR of overall and six individual IMID increased with SDI at regional and global level. Countries with higher ASR in 1990 experienced a more rapid decrease in ASR. Interpretation: The incidence patterns of IMIDs varied considerably across the world. Background: The sustainable development goals (SDGs) aim to end HIV/AIDS as a public health threat by 2030. Understanding the current state of the HIV epidemic and its change over time is essential to this effort. This study assesses the current sex-specific HIV burden in 204 countries and territories and measures progress in the control of the epidemic. Methods: To estimate age-specific and sex-specific trends in 48 of 204 countries, we extended the Estimation and Projection Package Age-Sex Model to also implement the spectrum paediatric model. We used this model in cases where age and sex specific HIV-seroprevalence surveys and antenatal care-clinic sentinel surveillance data were available. For the remaining 156 of 204 locations, we developed a cohort-incidence bias adjustment to derive incidence as a function of cause-of-death data from vital registration systems. The incidence was input to a custom Spectrum model. To assess progress, we measured the percentage change in incident cases and deaths between 2010 and 2019 (threshold >75% decline), the ratio of incident cases to number of people living with HIV (incidence-to-prevalence ratio threshold <0·03), and the ratio of incident cases to deaths (incidence-to-mortality ratio threshold <1·0). Findings: In 2019, there were 36·8 million (95% uncertainty interval [UI] 35·1–38·9) people living with HIV worldwide. There were 0·84 males (95% UI 0·78–0·91) per female living with HIV in 2019, 0·99 male infections (0·91–1·10) for every female infection, and 1·02 male deaths (0·95–1·10) per female death. Global progress in incident cases and deaths between 2010 and 2019 was driven by sub-Saharan Africa (with a 28·52% decrease in incident cases, 95% UI 19·58–35·43, and a 39·66% decrease in deaths, 36·49–42·36). Elsewhere, the incidence remained stable or increased, whereas deaths generally decreased. In management systems, the focus is moving from managing the Quality of Service (QoS) of end-user service sessions to managing their Quality of Experience (QOE). To do this, the user experience and context of those sessions much be modelled, analysed, and optimised to ensure end user experience matches expectations as closely as possible. In this thesis, we introduce the Aesop approach, which uses semantic-based techniques to autonomically optimize end-user service delivery. The Aesop knowledge base models the end-user service management domain in a manner that is aware of the temporal properties of concepts. The autonomic Aesop Engine runs efficient semantic algorithms that implement the MAPE functions using those temporal properties to operate on small partitioned subsets of the knowledge base. The effectiveness and performance of the Aesop approach was evaluated on a HAN test bed. A significant improvement was observed on the compliance levels of high priority sessions in all experimental scenarios, with compliance levels more than doubled in some cases. A case study demonstrated that Aesop was also applicable in the Mobile Broadband Access domain.

In order to evaluate the efficacy of new research approaches as well as existing systems for management of end user service sessions, it is necessary to set up controlled sessions in a realistic, measurable, and repeatable manner. While service delivery clients support automation of end user sessions and expose mechanisms for retrieval of end user service experience and context, there is no single manner for collection of service experience and context measurements, or for collation and delivery of those measurements to a management system. The Service Experience and Context COllection (SECCO) framework was developed to fulfil three roles. Firstly, it allows end user service sessions to be run in a controlled manner. Secondly, it records end user service experience and context for running sessions. Thirdly, it compiles terminal reports for end user sessions and forwards them to Aesop for processing. This paper outlines the design and implementation of SECCO and describes three separate cases where SECCO was applied.

The presence of emergent semantics in social annotation systems has been reported in numerous studies. Two important problems in this context are the induction of semantic relations among tags and the discovery of different senses of a given tag. While a number of approaches for discovering tag senses exist, little is known about which factors influence the discovery process. In this paper, we analyze the influence of user pragmatic factors. We divide taggers into different pragmatic distinctions. Based on these distinctions, we identify subsets of users whose annotations allow for a more precise and complete discovery of tag senses. Our results provide evidence for a link between tagging pragmatics and semantics and provide another argument for including pragmatic factors in semantic extraction methods. Our work is relevant for improving search, retrieval and browsing in social annotation systems, as well as for optimizing ontology learning algorithms based on tagging data. © 2013 Springer-Verlag.

This paper presents research carried out in order to elicit user needs for the design and development of a digital library and research platform intended to enhance user engagement with cultural heritage collections. It outlines a range of user constituencies for this digital library. The data required for automatic optimization of user services usually exists in current systems, but that data is not modelled or linked in a way that facilitates automation. knowledge engineering is a promising approach for managing the disparate communication service quality management information data sets and the links across those data sets. Once a knowledge base is in place, semantic techniques can be used to analyse and suggest optimizations to service quality. This paper describes our work in building, populating and evaluating a knowledge base for an IPTV service in Home Area Networks. Population of the knowledge base was implemented using terminal reports. The characteristics of the approach were evaluated through experimentation and the evaluation results are presented in this paper.

Recent research has demonstrated how the widespread adoption of collaborative tagging systems yields emergent semantics. In recent years, much has been learned about how to harvest the data produced by taggers for engineering light-weight ontologies. For example, existing measures of tag similarity and tag relatedness have proven crucial step stones for making latent semantic relations in tagging systems explicit. However, little progress has been made on other issues, such as understanding the different levels of tag generality (or tagabstrcatsness), which is essential for, among others, identifying hierarchical relationships between concepts. In this paper we aim to address this gap. Starting from a review of linguistic definitions of wordabstrcatness, we first use several large-scale ontologies and taxonomies as grounded measures of word generality, including Yago, Wordnet, DMOZ and Wikitaxonomy. Then, we introduce and apply several folksonomy-based methods to measure the level of generality of given tags. We evaluate these methods by comparing them with the grounded measures. Our results suggest that the generality of tags in social tagging systems can be approximated with simple measures. Our work has implications for a number of problems related to social tagging systems, including search, tag recommendation, and the acquisition of light-weight ontologies from tagging data.
