We propose the application of a novel sub-ontology extractionmethodology for achieving interoperability and improving the semantic validity of information retrieval in the medical information systems (MIS) domain. The system offers advanced profiling of a user’s field of specialization by exploiting the concept of sub-ontology extraction, i.e., each sub-ontology may subsequently represent a particular user profile. Semantic profiling of a user’s field of specialization or interest is necessary functionality in any medical domain information retrieval system;this is because the (structural and semantic) extent of information sources is massive and individual users are only likely to be interested in specific parts of the overall knowledge documents on the basis of their area of specialization. The prototypical system, OntoMOVE, has been specifically designed for application in themedical information systems domain. OntoMOVE utilizes semantic web standards like RDF(S) and OWL in addition to medical domain standards and vocabularies encompassed by the UMLS knowledge sources. © 2018, The Authors. Background Interest in standardizing descriptions of health outcomes is increasing. In a Health Outcome Descriptor (HOD), outcomes are systematically described covering four domains: Symptoms, Time horizon, Testing and Treatment, and Consequences. Given the lack of HODs for Rheumatoid Arthritis (RA), the aim of this study was to review published RA outcome descriptions and map them to the HOD framework. Methods We conducted a rapid review of patient-decision aids (PtDAs) and patient preference studies to assess how seven RA outcomes have been described in English to patients. These outcomes were selected by author consensus, from a living systematic review of RA drug therapy. After data extraction and a thematic content analysis, a narrative summary for each outcome was provided. Results We included 11 PtDAs and 27 patient preference studies. Overall, the descriptions of the same health outcome varied widely across studies. Adverse events were described in most cases (N=26/38). For both PtDAs and preference studies, few provided a description for patient-important outcomes like remission (N=2/11 and N=1/27 respectively) and pain (N=2/11 and N=6/27 respectively). From an HOD perspective, the descriptions focused primarily on symptoms patients may experience (95%), and less on the other domains (18-39%). Conclusion There is wide variability in the content of the published RA outcome descriptions, as well as a lack of descriptions regarding common patient-important outcomes. As this study provides a detailed overview of existing descriptions, it may inform future development of HODs for RA.

Background: The COVID-19 pandemic highlighted gaps in health surveillance systems, disease prevention, and treatment globally. Among the many factors that might have led to these gaps is the issue of the financing of national health systems, especially in low-income and middle-income countries (LMICs), as well as a robust global system for pandemic preparedness. We aimed to provide a comparative assessment of global health spending at the onset of the pandemic;characterise the amount of development assistance for pandemic preparedness and response disbursed in the first 2 years of the COVID-19 pandemic;and examine expectations for future health spending and put into context the expected need for investment in pandemic preparedness. Methods: In this analysis of global health spending between 1990 and 2021, and prediction from 2021 to 2026, we estimated four sources of health spending: development assistance for health (DAH), government spending, out-of-pocket spending, and prepaid private spending across 204 countries and territories. We used the Organisation for Economic Co-operation and Development (OECD)'s Creditor Reporting System (CRS) and the WHO Global Health Expenditure database (GHED) to estimate spending. We estimated development assistance for general health, COVID-19 response, and pandemic preparedness and response using a keyword search. Health spending estimates were combined with estimates of resources needed for pandemic prevention and preparedness to analyse future health spending patterns, relative to need. Findings: In 2019, at the onset of the COVID-19 pandemic, US$9·2 trillion (95% uncertainty interval [UI] 9·1–9·3) was spent on health worldwide. We found great disparities in the amount of resources devoted to health, with high-income countries spending $7·3 trillion (95% UI 7·2–7·4) in 2019;293·7 times the $24·8 billion (95% UI 24·3–25·3) spent by low-income countries in 2019. Background: Neck pain is a highly prevalent condition that leads to considerable pain, disability, and economic cost. We present the most current estimates of neck pain prevalence and years lived with disability (YLDs) from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) by age, sex, and location, with forecasted prevalence to 2050. Methods: Systematic reviews identified population-representative surveys used to estimate the prevalence of and YLDs from neck pain in 204 countries and territories, spanning from 1990 to 2020, with additional data from opportunistic review. Medical claims data from Taiwan (province of China) were also included. Input data were pooled using DisMod-MR 2.1, a Bayesian meta-regression tool. Prevalence was forecast to 2050 using a mixed-effects model using Socio-demographic Index as a predictor and multiplying by projected population estimates. We present 95% UIs for every metric based on the 2·5th and 97·5th percentiles of 100 draws of the posterior distribution. Findings: Globally, in 2020, neck pain affected 203 million (95% uncertainty interval [UI] 163–253) people. The global age-standardised prevalence rate of neck pain was estimated to be 2450 (1960–3040) per 100 000 population and global age-standardised YLD rate was estimated to be 244 (165–346) per 100 000. The age-standardised prevalence rate remained stable between 1990 and 2020 (percentage change 0·2% [–1·3 to 1·7]). Globally, females had a higher age-standardised prevalence rate (2890 [2330–3620] per 100 000) than males (2000 [1600–2480] per 100 000), with the prevalence peaking between 45 years and 74 years in male and female sexes. By 2050, the estimated global number of neck pain cases is projected to be 269 million (219–322), with an increase of 32·5% (23·9–42·3) from 2020 to 2050. Background: The causes for immune-mediated inflammatory diseases (IMIDs) are diverse and the incidence trends of IMIDs from specific causes are rarely studied. The study aims to investigate the pattern and trend of IMIDs from 1990 to 2019. Methods: We collected detailed information on six major causes of IMIDs, including asthma, inflammatory bowel disease, multiple sclerosis, rheumatoid arthritis, psoriasis, and atopic dermatitis, between 1990 and 2019, derived from the Global Burden of Disease study in 2019. The average annual percent change (AAPC) in number of incidents and age standardized incidence rate (ASR) on IMIDs, by sex, age, region, and causes, were calculated to quantify the temporal trends. Findings: In 2019, rheumatoid arthritis, atopic dermatitis, asthma, multiple sclerosis, psoriasis, inflammatory bowel disease accounted 1.59%, 36.17%, 54.71%, 0.09%, 6.84%, 0.60% of overall new IMIDs cases, respectively. The ASR of IMIDs showed substantial regional and global variation with the highest in High SDI region, High-income North America, and United States of America. Throughout human lifespan, the age distribution of incident cases from six IMIDs was quite different. Globally, incident cases of IMIDs increased with an AAPC of 0.68 and the ASR decreased with an AAPC of −0.34 from 1990 to 2019. The incident cases increased across six IMIDs, the ASR of rheumatoid arthritis increased (0.21, 95% CI 0.18, 0.25), while the ASR of asthma (AAPC = −0.41), inflammatory bowel disease (AAPC = −0.72), multiple sclerosis (AAPC = −0.26), psoriasis (AAPC = −0.77), and atopic dermatitis (AAPC = −0.15) decreased. The ASR of overall and six individual IMID increased with SDI at regional and global level. Countries with higher ASR in 1990 experienced a more rapid decrease in ASR. Interpretation: The incidence patterns of IMIDs varied considerably across the world. Background: The sustainable development goals (SDGs) aim to end HIV/AIDS as a public health threat by 2030. Understanding the current state of the HIV epidemic and its change over time is essential to this effort. This study assesses the current sex-specific HIV burden in 204 countries and territories and measures progress in the control of the epidemic. Methods: To estimate age-specific and sex-specific trends in 48 of 204 countries, we extended the Estimation and Projection Package Age-Sex Model to also implement the spectrum paediatric model. We used this model in cases where age and sex specific HIV-seroprevalence surveys and antenatal care-clinic sentinel surveillance data were available. For the remaining 156 of 204 locations, we developed a cohort-incidence bias adjustment to derive incidence as a function of cause-of-death data from vital registration systems. The incidence was input to a custom Spectrum model. To assess progress, we measured the percentage change in incident cases and deaths between 2010 and 2019 (threshold >75% decline), the ratio of incident cases to number of people living with HIV (incidence-to-prevalence ratio threshold <0·03), and the ratio of incident cases to deaths (incidence-to-mortality ratio threshold <1·0). Findings: In 2019, there were 36·8 million (95% uncertainty interval [UI] 35·1–38·9) people living with HIV worldwide. There were 0·84 males (95% UI 0·78–0·91) per female living with HIV in 2019, 0·99 male infections (0·91–1·10) for every female infection, and 1·02 male deaths (0·95–1·10) per female death. Global progress in incident cases and deaths between 2010 and 2019 was driven by sub-Saharan Africa (with a 28·52% decrease in incident cases, 95% UI 19·58–35·43, and a 39·66% decrease in deaths, 36·49–42·36). Elsewhere, the incidence remained stable or increased, whereas deaths generally decreased. A crucial piece of semantic web development is the creation of viable ontology matching approaches to ensure interoperability in a wide range of applications such as information integration and semantic multimedia. In this paper, a new approach for ontology matching called IROM (Information Retrieval-based Ontology Matching) is presented. This approach derives the different components of an information retrieval (IR) framework based on the information provided by the input ontologies and supported by ontology similarity measures. Subsequently, a retrieval algorithm is applied to determine the correspondences between the matched ontologies. IROM was tested with ontology pairs taken from two resources for reference ontologies, OAEI and FOAM. The evaluation shows that IROM is competitive with top-ranked matchers on the benchmark test at OAEI campaign of 2009. © 2011 Springer-Verlag.

Finding correct semantic correspondences between heterogeneous ontologies is one of the most challenging problems in the area of semantic web technologies. As manually constructing such mappings is not feasible in realistic scenarios, a number of automatic matching tools have been developed that propose mappings based on general heuristics. As these heuristics often produce incorrect results, a manual revision is inevitable in order to guarantee the quality of generated mappings. Experiences with benchmarking matching systems revealed that the manual revision of mappings is still a very difficult problem because it has to take the semantics of the ontologies as well as interactions between mappings into account. In this article, we propose methods for supporting human experts in the task of revising automatically created mappings. In particular, we present non-standard reasoning methods for detecting and propagating implications of expert decisions on the correctness of a mapping.
