Background: The COVID-19 pandemic highlighted gaps in health surveillance systems, disease prevention, and treatment globally. Among the many factors that might have led to these gaps is the issue of the financing of national health systems, especially in low-income and middle-income countries (LMICs), as well as a robust global system for pandemic preparedness. We aimed to provide a comparative assessment of global health spending at the onset of the pandemic;characterise the amount of development assistance for pandemic preparedness and response disbursed in the first 2 years of the COVID-19 pandemic;and examine expectations for future health spending and put into context the expected need for investment in pandemic preparedness. Methods: In this analysis of global health spending between 1990 and 2021, and prediction from 2021 to 2026, we estimated four sources of health spending: development assistance for health (DAH), government spending, out-of-pocket spending, and prepaid private spending across 204 countries and territories. We used the Organisation for Economic Co-operation and Development (OECD)'s Creditor Reporting System (CRS) and the WHO Global Health Expenditure database (GHED) to estimate spending. We estimated development assistance for general health, COVID-19 response, and pandemic preparedness and response using a keyword search. Health spending estimates were combined with estimates of resources needed for pandemic prevention and preparedness to analyse future health spending patterns, relative to need. Findings: In 2019, at the onset of the COVID-19 pandemic, US$9·2 trillion (95% uncertainty interval [UI] 9·1–9·3) was spent on health worldwide. We found great disparities in the amount of resources devoted to health, with high-income countries spending $7·3 trillion (95% UI 7·2–7·4) in 2019;293·7 times the $24·8 billion (95% UI 24·3–25·3) spent by low-income countries in 2019. Background: The causes for immune-mediated inflammatory diseases (IMIDs) are diverse and the incidence trends of IMIDs from specific causes are rarely studied. The study aims to investigate the pattern and trend of IMIDs from 1990 to 2019. Methods: We collected detailed information on six major causes of IMIDs, including asthma, inflammatory bowel disease, multiple sclerosis, rheumatoid arthritis, psoriasis, and atopic dermatitis, between 1990 and 2019, derived from the Global Burden of Disease study in 2019. The average annual percent change (AAPC) in number of incidents and age standardized incidence rate (ASR) on IMIDs, by sex, age, region, and causes, were calculated to quantify the temporal trends. Findings: In 2019, rheumatoid arthritis, atopic dermatitis, asthma, multiple sclerosis, psoriasis, inflammatory bowel disease accounted 1.59%, 36.17%, 54.71%, 0.09%, 6.84%, 0.60% of overall new IMIDs cases, respectively. The ASR of IMIDs showed substantial regional and global variation with the highest in High SDI region, High-income North America, and united States of America. Throughout human lifespan, the age distribution of incident cases from six IMIDs was quite different. Globally, incident cases of IMIDs increased with an AAPC of 0.68 and the ASR decreased with an AAPC of −0.34 from 1990 to 2019. The incident cases increased across six IMIDs, the ASR of rheumatoid arthritis increased (0.21, 95% CI 0.18, 0.25), while the ASR of asthma (AAPC = −0.41), inflammatory bowel disease (AAPC = −0.72), multiple sclerosis (AAPC = −0.26), psoriasis (AAPC = −0.77), and atopic dermatitis (AAPC = −0.15) decreased. The ASR of overall and six individual IMID increased with SDI at regional and global level. Countries with higher ASR in 1990 experienced a more rapid decrease in ASR. Interpretation: The incidence patterns of IMIDs varied considerably across the world. Background: The sustainable development goals (SDGs) aim to end HIV/AIDS as a public health threat by 2030. Understanding the current state of the HIV epidemic and its change over time is essential to this effort. This study assesses the current sex-specific HIV burden in 204 countries and territories and measures progress in the control of the epidemic. Methods: To estimate age-specific and sex-specific trends in 48 of 204 countries, we extended the Estimation and Projection Package Age-Sex Model to also implement the spectrum paediatric model. We used this model in cases where age and sex specific HIV-seroprevalence surveys and antenatal care-clinic sentinel surveillance data were available. For the remaining 156 of 204 locations, we developed a cohort-incidence bias adjustment to derive incidence as a function of cause-of-death data from vital registration systems. The incidence was input to a custom Spectrum model. To assess progress, we measured the percentage change in incident cases and deaths between 2010 and 2019 (threshold >75% decline), the ratio of incident cases to number of people living with HIV (incidence-to-prevalence ratio threshold <0·03), and the ratio of incident cases to deaths (incidence-to-mortality ratio threshold <1·0). Findings: In 2019, there were 36·8 million (95% uncertainty interval [UI] 35·1–38·9) people living with HIV worldwide. There were 0·84 males (95% UI 0·78–0·91) per female living with HIV in 2019, 0·99 male infections (0·91–1·10) for every female infection, and 1·02 male deaths (0·95–1·10) per female death. Global progress in incident cases and deaths between 2010 and 2019 was driven by sub-Saharan Africa (with a 28·52% decrease in incident cases, 95% UI 19·58–35·43, and a 39·66% decrease in deaths, 36·49–42·36). Modern healthcare organizations generate huge amount of electronic data stored in heterogeneous databases, Even though these data are a valuable resource for mining useful knowledge to support scientific decision making, a representational heterogeneity of such databases requires a customized and flexible method to mine actionable knowledge. In this paper we propose a medical decision support system (MDSS) based on a logical framework. The proposed MDSS includes a knowledge induction component to induce knowledge from clinical data repositories and the induced knowledge can be deployed to pre-treatment data from other sources. The implementation of knowledge induction engine has been presented to express the power of higher-order programming of logic-based language. The flexibility of our mining engine is obtained through the pattern matching and meta-programming facilities provided by logic-based language.

We propose a knowledge mining system as an integrated environment storing a repertoire of tools for discovering strong and useful knowledge. We demonstrate the usefulness aspect through the semi-automatic trigger creation for the medical database. A rapid prototyping of association mining engine is also presented in the paper.

Inductive databases can be viewed as a natural extension of traditional databases to contain not only persistent data but also the generalization of stored data, which are called patterns. The idea of inductive databases has been proposed originally as a support system for the knowledge discovery or data mining process. Many SQL-like languages have been designed and implemented to include mining operators in the SQL primitives. We percept the concept of inductive databases in a different angle. In stead of designing yet another inductive database system, we are looking for the deployment of an existing inductive query language and environment to support the database tasks. We focus on the task of query answering which has a high potential of being a beneficiary of the stored patterns in inductive databases. Our experimental results of query rewriting technique using induced patterns as a semantic knowledge confirm this advantage.

Density estimation is an important pre-processing step in the problem of data stream classification in which the number of data is overwhelming and the exact data distribution is unknown. We simplify the problem by employing a statistical sampling technique to obtain an approximate solution. With the proposed method, an unbounded large data set can be sampled in a number of random configurations, and that data can be used to describe the data set as a whole. The efficiency of the method depends largely on the ability to draw samples effectively which in turn depends on how close we can estimate the target density. We use finite mixture models to represent the probability density functions of the data stream. Then, we apply the EM algorithm twice to learn the model parameters. The efficiency of our estimation technique has been shown in the experimental results

Projective clustering is a clustering technique for high dimensional data with the inherent sparsity of the data points. To overcome the unreliable measure of similarity among data points in high dimensions, all data points are projected to a lower dimensional sub-space. Principal component analysis (PCA) is an efficient method to dimensionality reduction by projecting all points to a lower dimensional subspace so that the information loss is minimized. However, PCA does not handle well the situation that different clusters are formed in different subspaces. We propose a method of multiple principal component analysis for iteratively computing projective clusters. The objective function is designed to determine the subspace associated with each cluster. Some experiments have been carried out to show the effectiveness of the proposed method.

Clustering is a task of grouping data based on similarity. A popular k-means algorithm groups data by firstly assigning all data points to the closest clusters, then determining the cluster means. The algorithm repeats these two steps until it has converged. We propose a variation called weighted k-means to improve the clustering scalability. To speed up the clustering process, we develop the reservoir-biased sampling as an efficient data reduction technique since it performs a single scan over a data set. Our algorithm has been designed to group data of mixture models. We present an experimental evaluation of the proposed method.
