Background: Pathogenic variants (PVs) in ATM, BRCA1, BRCA2, CHEK2, and PALB2 are associated with increased breast cancer risk. It is unknown, however, whether this risk differs by PV type or location in carriers ascertained from the general population. Patients and methods: To evaluate breast cancer risks associated with PV type and location in ATM, BRCA1, BRCA2, CHEK2, and PALB2, we carried out age-adjusted case-control association analysis in 32 247 women with and 32 544 age-matched women without breast cancer from the CARRIERS Consortium. PVs were grouped by type and location within genes and assessed for risks of breast cancer [odds ratios (OR), 95% confidence intervals (CI), and P values] using logistic regression. Results: Compared with women carrying BRCA2 exon 11 protein truncating variants (PTVs) in the CARRIERS population-based study, women with BRCA2 ex1-10 PTVs (OR = 13.5, 95% CI 6.0-38.7, P < 0.001) and ex13-27 PTVs (OR = 9.0, 95% CI 4.9-18.5, P < 0.001) had higher breast cancer risks, lower rates of estrogen receptor (ER)-negative breast cancer (ex13-27 OR = 0.5, 95% CI 0.2-0.9, P = 0.035;ex1-10 OR = 0.5, 95% CI 0.1-1.0, P = 0.065), and earlier age at breast cancer diagnosis (ex13-27 5.5 years, P < 0.001;ex1-10 2.4 years, P = 0.169). These associations with ER-negative breast cancer and age were replicated in a high-risk clinical cohort from Ambry Genetics and the population-based UK Biobank cohort. No differences in risk by gene region were observed for PTVs in other predisposition genes. Conclusions: Population-based and clinical high-risk cohorts establish that PTVs in exon 11 of BRCA2 are associated with reduced breast cancer risk, later age at diagnosis, and greater risk of ER-negative disease. These differential risks may improve individualized risk prediction and clinical management for women carrying BRCA2 PTVs. © 2025 The Authors

Background The World Health Organization has confirmed that Helicobacter pylori is a carcinogen associated with gastric cancer (GC). Although a recent increasing trend in the prevalence of the H. pylori -naïve status has been reported, the precise values of each generation have not yet been specified. This study aimed to confirm the generational H. pylori -naïve prevalence in Japan. Materials and Methods Scientific articles available in PubMed and EMBASE between January 2014 and June 2024 were systematically searched. Publications that specified the generational H. pylori- naïve prevalence were included in the analysis. Publications that did not describe the prevalence of H. pylori -naïve individuals and the definition of H. pylori -eradicated individuals were excluded. A meta-regression analysis with a logistic mixed effect model was conducted to predict the generational prevalence of the H. pylori -naïve status. In this model, birth year was incorporated as a spline function to model non-linear trends, and study differences were included as random effects. Results A total of 899 publications were identified. After screening, eight publications were included in the final analysis. Nine different study groups comprising 46,704 individuals included in those publications were identified and analyzed. The calculated H. pylori -naïve prevalence rates among the average cohort were 21.1% for births in 1930, 47.2% for births in 1950, 75.3% for births in 1970, 91.7% for births in 1990, and 97.7% for births in 2010. Conclusions The H. pylori -naïve prevalence increased dramatically with more recent birth years and reached a plateau of over 95%, especially after 2000. The current GC screening strategy can be modified based on this finding.

IntroductionLeukocyte telomere length (LTL) significantly shortens after hematopoietic cell transplant (HCT) because of hematopoietic cell proliferation to achieve engraftment and immune reconstitution. The magnitude of LTL shortening is associated with patient outcomes after HCT. Limited LTL shortening is associated with increased relapse risk, and excess LTL shortening is associated with higher risk of non-relapse mortality. ObjectiveThis study aimed to identify factors associated with LTL shortening in the first three-months after HCT. MethodsWe studied 916 HCT recipients from the Blood & Marrow Transplant Clinical Trial Network 1202 Protocol who had donor blood samples available in the CIBMTR Biorepository. Southern blotting was used to measure LTL in donor pre-HCT and recipient post-HCT samples (average of 90 days). LTL shortening was calculated as donor mean LTL – recipient mean LTL. We examined LTL as a continuous outcome and in categories (limited LTL shortening: <25 th percentile vs ≥25 th percentile; excess LTL shortening: >75 th percentile vs ≤75 th percentile). We explored bivariate associations between LTL and donor, recipient, and clinical and transplant characteristics and included variables with P<0.1 in final multivariable linear and logistic regression models. ResultsMultiple linear regression models found an association between less LTL shortening and older donor age (P<0.001) and peripheral blood (PB) vs bone marrow (BM) graft (P=0.001). In logistic regression models, older donor age (P< 0.001); PB vs BM graft (P=0.003); female vs male donors (P=0.019); and lymphoid vs myeloid disease (P=0.039) were significantly associated with a higher likelihood of limited LTL shortening. Older recipient age (P=0.001); receiving Busulfan-Cyclophosphamide (Bu/Cy; P=0.046) and total body irradiation (TBI; P<0.001) conditioning regimens vs Busulfan-Fludarabine (Bu/Flu); and receiving antithymocyte globulin (ATG) or Campath (P=0.01) vs not receiving ATG or Campath

Background Increasing evidence suggests that conventional adenomas (CAs) and serrated polyps (SPs) represent two distinct groups of precursor lesions for colorectal cancer (CRC). The influence of common genetic variants on risk of CAs and SPs remain largely *** Among 27 426 participants within three prospective cohort studies, we created a weighted genetic risk score (GRS) based on 40 CRC-related single nucleotide polymorphisms (SNPs) identified in previous genome-wide association studies; and we examined the association of GRS (per one standard deviation increment) with risk of CAs, SPs and synchronous CAs and SPs, by multivariable logistic regression. We also analysed individual variants in the secondary *** During 18-20 years of follow-up, we documented 2952 CAs, 1585 SPs and 794 synchronous CAs and SPs. Higher GRS was associated with increased risk of CAs [odds ratio (OR) = 1.17, 95% confidence interval (CI): 1.12-1.21] and SPs (OR = 1.09, 95% CI: 1.03-1.14), with a stronger association for CAs than SPs (Pheterogeneity=0.01). An even stronger association was found for patients with synchronous CAs and SPs (OR = 1.32), advanced CAs (OR = 1.22) and multiple CAs (OR = 1.25). Different sets of variants were associated with CAs and SPs, with a Spearman correlation coefficient of 0.02 between the ORs associating the 40 SNPs with the two lesions. After correcting for multiple testing, three variants were associated with CAs (rs3802842, rs6983267 and rs7136702) and two with SPs (rs16892766 and rs4779584).Conclusions Common genetic variants play a potential role in the conventional and serrated pathways of CRC. Different sets of variants are identified for the two pathways, further supporting the aetiological heterogeneity of CRC.

Accurate discrimination between patients with and without cancer from cfDNA is crucial for early cancer diagnosis. Herein, we develop and validate a deep-learning-based model entitled end-motif inspection via transformer (EMIT) for discriminating individuals with and without cancer by learning feature representations from cfDNA end-motifs. EMIT is a self-supervised learning approach that models rankings of cfDNA end-motifs. We include 4606 samples subjected to different types of cfDNA sequencing to develop EIMIT, and subsequently evaluate classification performance of linear projections of EMIT on six datasets and an additional inhouse testing set encopassing whole-genome, whole-genome bisulfite and 5-hydroxymethylcytosine sequencing. The linear projection of representations from EMIT achieved area under the receiver operating curve (AUROC) values ranged from 0.895 (0.835-0.955) to 0.996 (0.994-0.997) across these six datasets, outperforming its baseline by significant margins. Additionally, we showed that linear projection of EMIT representations can achieve an AUROC of 0.962 (0.914-1.0) in identification of lung cancer on an independent testing set subjected to whole-exome sequencing. The findings of this study indicate that a transformer-based deep learning model can learn cancer-discrimative representations from cfDNA end-motifs. The representations of this deep learning model can be exploited for discriminating patients with and without cancer.

BACKGROUND: A high prevalence of excess weight in children younger than 5 years suggests the involvement of early-life risk factors. The preconception and pregnancy periods are crucial stages for the implementation of interventions to prevent childhood obesity. Most studies so far have evaluated the effects of early-life factors separately, with only a few investigating the combined effect of parental lifestyle factors. Our objective was to fill the literature gap regarding parental lifestyle factors in the preconception and pregnancy periods and to study their association with the risk of overweight in children after the age of 5 years. METHODS: We harmonised and interpreted data from four European mother-offspring cohorts (EDEN [comprising 1900 families], Elfe [comprising 18 000 families], Lifeways [comprising 1100 families], and Generation R [comprising 9500 families]). Written informed consent was obtained from parents of all involved children. Lifestyle factor data collected through questionnaires comprised parental smoking, BMI, gestational weight gain, diet, physical activity, and sedentary behaviour. We applied principal component analyses to identify multiple lifestyle patterns in preconception and pregnancy. Their association with child BMI z-score and risk of overweight (including obesity, overweight and obesity, as defined by the International Task Force reference) between the ages of 5 and 12 years were assessed using cohort-specific multivariable linear and logistic regression models (adjusted for confounders including parental age, education level, employment status, geographic origin, parity, and household income). FINDINGS: Among the various lifestyle patterns identified in all cohorts, the two that better explained variance were high parental smoking plus low maternal diet quality or high maternal sedentary behaviour, and high parental BMI plus low gestational weight gain.
