Rabies postexposure prophylaxis (PEP) is administered for rabies prevention after a human exposure to a potentially rabid animal, such as a bite. Previous studies have reported that rabies PEP is often inappropriately administered. Health professional education was proposed as one potential solution to address inappropriate PEP use. We assessed baseline knowledge, knowledge gain, and knowledge retention among health professionals in Arizona of rabies epidemiology and appropriate PEP administration. Maricopa County department of Public Health created an online rabies PEP continuing education module and measured knowledge before and after module completion using a 10-question test. The same test was administered three times (pretest, posttest, and retention test at ≥3 months). To assess knowledge gain and retention, we compared median scores using nonparametric methods. A total of 302 respondents completed the pretest (median score, 60%) and posttest (median score, 90%;p <.001);98 respondents completed all three tests with median scores 60% (pretest), 90% (posttest, p <.01), and 80% (retention test and compared with pretest, p <.01). Sixty-nine (70%) respondents improved their pretest to retention test score by a mean of 2.4 points out of a total 10 points (median: 2 points;range: −5 to 7 points). Only 48% of pretest respondents correctly answered that PEP should not be administered immediately to anyone bitten by a healthy dog. However, 81% and 70% answered correctly on the posttest (p <.0001) and retention test (p =.002), respectively. Respondents demonstrated rabies epidemiology and PEP knowledge gain and ≥3-month knowledge retention after completing the online continuing education module. © 2018 Society for Public Health Education.

The association of exposure to several heavy metals with prostate cancer risk has been assessed in some studies, but the effects of complex interactions among heavy metals remain unclear. We aim to assess the association between metal mixtures exposure and prostate cancer risk in the European Prospective Investigation into cancer and Nutrition (EPIC) cohort. Fifteen heavy metals were measured in plasma samples of 62 prostate cancer cases and 242 non-cases from the EPIC-Granada cohort (south Spain) by ICP-MS-ORS. Bayesian kernel machine regression and distributed lag models (BKMR-DLM) were applied to assess the effect of mixtures of metals on prostate cancer risk, which allows accounting for nonlinear, interactive, joint effects and time-varying cumulative effects of mixture exposures. From these models we derived the excess prostate cancer risk for exposure to the mixture at the 75th percentile compared to the 25th percentile, keeping all other metals not involved in the mixture at the 50th percentile. The results demonstrated that four heavy metals (tungsten, copper, mercury, and vanadium) were significantly and positively associated with prostate cancer risk in the adjusted models for potential confounders . Using Bayesian kernel machine regression and distributed lag model (BKMR-DLM), the mixture vanadium+mercury+copper combined with another metal (arsenic, zinc, or cadmium) showed higher associations with prostate cancer risk (RR from 1.59 to 1.46) . The mixture vanadium+mercury had also a positive significant association in combination with cadmium, zinc, arsenic, or selenium. The overall mixture of 8 metals was also significantly associated with the prostate cancer risk (RR=1.45;95% CI: 1.21 to 1.68). In conclusion, we found positive associations between the plasma levels of four metals (tungsten, copper, mercury, and vanadium) and prostate cancer risk. Introduction: Longitudinal data on how acute respiratory illness (ARI) affects behavior, namely school or work participation, and nonpharmaceutical intervention (NPI) usage before and during the COVID-19 pandemic is limited. The authors assessed how ARIs and specific symptoms affected school, work, and health-related behaviors over time. Methods: From November 2019 to June 2021, participating households with children in King County, Washington, were remotely monitored for ARI symptoms weekly. Following ARIs, participants reported illness-related effects on school, work, and NPI use. Using logistic regression with generalized estimating equations, the authors examined associations between symptoms and behaviors. Results: Of 1,861 participants, 581 (31%) from 293 households reported 884 ARIs and completed one-week follow-up surveys. Compared with the prepandemic period, during the period of the pandemic pre–COVID-19 vaccine, ARI-related school (56% vs 10%, p<0.001) absenteeism decreased and masking increased (3% vs 28%, p<0.001). After vaccine authorization in December 2020, more ARIs resulted in masking (3% vs 48%, p<0.001), avoiding contact with non-household members (26% vs 58%, p<0.001), and staying home (37% vs 69%, p<0.001) compared with the prepandemic period. Constitutional symptoms such as fever were associated with work disruptions (OR=1.91;95% CI=1.06, 3.43), staying home (OR=1.55;95% CI=1.06, 2.27), and decreased contact with non-household members (OR=1.58;95% CI=1.05, 2.36). Conclusions: This remote household study permitted uninterrupted tracking of behavioral changes in families with children before and during the COVID-19 pandemic, identifying increased use of some NPIs when ill but no additional illness-associated work or school disruptions. © 2024 The Authors

BackgroundTreatment-related sexual dysfunction affects the majority of hematopoietic stem cell transplant (HSCT) survivors. Data regarding the relationship between sexual health domains and patient-reported quality of life (QOL) and psychological distress are limited, particularly in this population. MethodsWe conducted a secondary analysis of a cross-sectional study that included 125 autologous and allogeneic HSCT survivors. All were at least 3 months post-HSCT and endorsed sexual dysfunction causing distress. This was based on the two-question approach from the 2015 National Comprehensive cancer Network (NCCN) survivorship guidelines assessing 1) problems with sexual functioning and 2) whether these problems were causing distress. We assessed global satisfaction with sex and interest in sex (PROMIS), QOL (Functional Assessment of cancer Therapy-Bone Marrow Transplant [FACT-BMT]), and psychological distress (Hospital Anxiety and Depression-Scale [HADS]). We used multivariate regression analyses, adjusting for age, race, and gender, to examine the association between the sexual health domains with QOL and psychological distress. ResultsStudy participants had a median age of 55.0 (SD 14), and the majority were male (67.2%; 84/125). Female sex was associated with lower global satisfaction with sex (B= -2.14, P=0.019) and lower interest in sex (B= - 1.69, P<0.001). In multivariable regression analyses, global satisfaction with sex was associated with better QOL (B=0.86, P=0.015) but not with depression (B= -0.12, P=0.056) or anxiety (B= -0.10, P=0.180) symptoms. Patient's interest in sex was associated with their QOL (B=3.28, P<0.001), depression (B= -0.51, P=0.002), and anxiety (B= -0.39, P=0.032) symptoms. ConclusionsSexual health domains including global satisfaction with sex and interest in sex were associated with QOL and psychological distress at baseline in HSCT survivors. Obesity, Dietary inflammation, and Frailty among Older Adults: Evidence from the National Health and Nutrition Examination Survey

Knowledge related to the relationship between obesity and frailty is limited. This study aimed to investigate associations between obesity, dietary inflammation, and frailty among older adults. Study data came from National Health and Nutrition Examination Survey (2007–2014) examinations of adults age ≥60 years (n = 7182). Dietary inflammatory potential was determined using the Dietary Inflammatory Index (DII ^® ) derived from 24-h dietary recall. We analyzed independent and joint associations of obesity and DII with frailty to evaluate interaction. Multivariable logistic regression revealed that both obesity (Odds Ratio [OR] = 2.24, 95% CI: 1.68, 2.99) and moderately pro-inflammatory DII (OR = 1.68, 95% CI: 1.10, 2.58) were independently associated with greater frailty prevalence. A negative multiplicative interaction between obesity and highest pro-inflammatory diet also was found (adjusted odds in non-obese and obese were 2.07 and 2.37, respectively;p = 0.046). Results indicate the importance of considering obesity and dietary inflammatory potential when screening for frailty or developing treatments. © 2019, © 2019 Taylor & Francis Group, LLC.

IntroductionAllogeneic hematopoietic cell transplantation (HCT) may leave recipients with long-term immune dysfunction. About one in four HCT recipients who contract SARS-CoV-2 develops severe Coronavirus disease (COVID-19), and one in six dies. In the general population, short leukocyte telomere length (LTL) is associated with severe COVID-19. ObjectiveIn this study, we examined the relationship between donor LTL and COVID-19 severity in HCT recipients, who contracted SARS-CoV-2 after HCT. MethodsWe studied 87 HCT recipients who were hospitalized for COVID-19. The Telomere Shortest Length Assay (TeSLA) was used to measure LTL of recipients and their matched donors in pre-HCT blood samples from the CIBMTR Biorepository. Study outcomes were: 1) COVID-19 severity defined as mild (no oxygen needed); moderate (oxygen needed, but not mechanical ventilation); or severe (mechanical ventilation needed); and 2) HCT recipient survival four months after hospitalization for COVID-19. We used multinomial logistic regression and Cox regression to test the relationship between two TeSLA parameters— mean LTL and proportion of leukocyte telomeres < 3 kilobases (kb)- and COVID-19 outcomes in the HCT recipient, controlling for recipient and donor age, and recipient sex. ResultsRecipients’ mean age at HCT was 46 years (SD=19) and mean time since HCT was 3.8 years (SD=3). About 64% had a history of grade III/IV acute graft versus host disease (GVHD) and/or extensive chronic GVHD. Ten recipients developed severe COVID-19 at an average of 4.7 years (SD= 3.5) post-HCT. There was no difference in patient or HCT characteristics between the three COVID-19 severity groups, except for mean patient age at HCT (mild= 41, moderate= 57, severe= 57 years; P=0.005). Having severe COVID-19, expressed as respiratory distress, was associated with longer LTL (relative risk [RR]= 148.5, P= 0.005) and lower proportion of leukocyte telomeres < 3 kb in donors (RR= 0.8, P= 0.01), compared to having mild COVID

Introduction: Few healthy eating, school-based interventions have been rigorously evaluated in American Indian communities. Gardening and healthy eating are priorities in the Navajo Nation. Collaborations between researchers and local partners supported the design and implementation of this project. Design: The Yéego! Healthy Eating and Gardening Study was a group-randomized controlled trial to evaluate a school-based healthy eating and gardening intervention in 6 schools in the Navajo Nation. Schools were randomized 1:2 to intervention or comparison. Setting/participants: The Shiprock and Tsaile/Chinle areas in the Navajo Nation were selected. Elementary schools were screened for eligibility. All students in third and fourth grades were invited to participate in the assessments. Intervention: Delivered during 1 school year in the intervention schools, the intervention included a culturally relevant nutrition and gardening curriculum and a school garden. Main outcome measures: Student self-efficacy for eating fruits and vegetables, student self-efficacy for gardening, and student healthy foods score from a modified Alternative Healthy Eating Index were assessed in third and fourth graders at the beginning and end of a school year affected by the COVID-19 pandemic. Primary analyses used repeated measures linear mixed models accounting for students nested within schools to estimate the intervention effect and 95% CIs. Results: Students in the intervention schools had self-efficacy scores for eating fruits and vegetables that were 0.22 points greater (95% CI=0.04, 0.41) than those in the comparison schools, although the student healthy foods score increased in the intervention schools by 2.0 (95% CI=0.4, 3.6);the differential change was modest at 1.7 (95% CI=–0.3, 3.7). Background: Diabetes is one of the leading causes of death and disability worldwide, and affects people regardless of country, age group, or sex. Using the most recent evidentiary and analytical framework from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD), we produced location-specific, age-specific, and sex-specific estimates of diabetes prevalence and burden from 1990 to 2021, the proportion of type 1 and type 2 diabetes in 2021, the proportion of the type 2 diabetes burden attributable to selected risk factors, and projections of diabetes prevalence through 2050. Methods: Estimates of diabetes prevalence and burden were computed in 204 countries and territories, across 25 age groups, for males and females separately and combined;these estimates comprised lost years of healthy life, measured in disability-adjusted life-years (DALYs;defined as the sum of years of life lost [YLLs] and years lived with disability [YLDs]). We used the Cause of Death Ensemble model (CODEm) approach to estimate deaths due to diabetes, incorporating 25 666 location-years of data from vital registration and verbal autopsy reports in separate total (including both type 1 and type 2 diabetes) and type-specific models. Other forms of diabetes, including gestational and monogenic diabetes, were not explicitly modelled. Total and type 1 diabetes prevalence was estimated by use of a Bayesian meta-regression modelling tool, DisMod-MR 2.1, to analyse 1527 location-years of data from the scientific literature, survey microdata, and insurance claims;type 2 diabetes estimates were computed by subtracting type 1 diabetes from total estimates. Mortality and prevalence estimates, along with standard life expectancy and disability weights, were used to calculate YLLs, YLDs, and DALYs. Secular trends in incidence of lung cancer by histological type in Beijing, China, 2000-2016

Objective: The objective of this study was to characterize secular trends in the sex-specific, age-standardized incidence of lung cancer by histological type in Beijing, China, from 2000 to 2016 based on data from a populationbased cancer ***: Data on the incidence of cancer from 2000 to 2016 were obtained from the Beijing cancer *** examined trends in the sex-specific, age-standardized incidence of lung cancer by histological type using a Joinpoint regression ***: A total of 117,409 cases of lung cancer were diagnosed from 2000 to 2016. Overall, 73,062(62.23%)patients were males. The most common histological type among both sexes was adenocarcinoma; however, the proportion of adenocarcinoma differed significantly between males and females(45.36% vs. 77.14%, respectively,P<0.0001). The age-standardized incidence of total lung cancer increased from 2000 to 2010 with an annual percent change(APC) of 2.2% [95% confidence interval(95% CI), 1.5% to 2.9%] and stabilized thereafter. Among males, the incidence of total lung cancer peaked in 2008 and then decreased slightly, with an APC of-1.1%(95%CI,-2.1% to-0.1%). Among females, the incidence increased continuously during the study period, with an APC of 1.4%(95% CI, 0.9% to 1.9%). The incidence of squamous cell carcinoma decreased significantly in recent years among both sexes, with APCs of-2.6%(95% CI,-4.5% to-0.6%) from 2007 to 2016 for males and-5.4%(95%CI,-7.2% to-3.6%) from 2004 to 2016 for females. In contrast, the incidence of adenocarcinoma increased continuously throughout the study period, by APCs of 4.0%(95% CI, 2.6% to 5.4%) for males and 6.2%(95% CI,4.8% to 7.6%) for females. The incidence of small cell carcinoma peaked in 2007 and stabilized thereafter among males, whereas it peaked in 2012 and then decreased with an APC of-14.7%(95% CI,-25.3% to-2.6%) among females.
