An increased risk of ALL second primary malignancy (SPM) has been observed with lenalidomide maintenance therapy after autologous HCT (len-post-autoHCT) for MM. Lenalidomide exerts its therapeutic effects via cereblon-mediated downregulation of the transcription factors Ikaros ( IKZF1) and Aiolos ( IKZF3). Inherited susceptibility variants in IKZF1 have been described for de novo ALL. We performed a case-control germline targeted sequencing study of the entire IKZF1 and IKZF3 genes with mean coverage ∼2,200X to investigate inherited susceptibility to ALL SPM risk. All cases and controls had a primary diagnosis of MM and received len-post-autoHCT with melphalan; sequencing was performed on an aliquot of the apheresis product or peripheral blood sample that was collected before autoHCT for MM. Cases of ALL SPMs were contributed by 4 individual centers and one national trial (BMT CTN 0702); each case had a maximum of 4 controls per case, sampled with replacement and matched on age, sex, race/ethnicity and duration of lenalidomide exposure (controls had longer duration of lenalidomide maintenance than cases). An additional control group of Acute Myeloid Leukemia/Myelodysplastic Syndrome (AML/MDS) SPMs after len-post-autoHCT were contributed by one center (Table). Multivariate logistic regression of case-control status, adjusted by age at HCT, sex, race/ethnicity, duration of lenalidomide maintenance, was performed for: 1) ALL SPM, 2) AML/MDS SPM, 3) ALL and AML/MDS SPM. We found the risk allele A of rs62447181 in IKZF1 had an OR=2.51 P=9.53 × 10 −3 for ALL SPM, OR=4.18 P=0.09 for AML/MDS SPM and OR=2.57 P=4.98 × 10 −3 for ALL&AML/MDS SPM. rs62447181 is a cis-expression quantitative trait loci (eQTL) of IKZF1 in blood (p=3.88 × 10 −55 in eQTLGen Consortium data) and resides in a promoter-proximal enhancer predicted by the ENCODE project (Figure A). Background People living with HIV (PLHIV) with viral hepatitis C (HCV) co-infection have higher risk of liver failure and mortality. Globally, >2 million people globally have HIV/HCV co-infections. Central Asia is one of the regions most affected by HCV. Since 2018, Kyrgystan has provided HCV treatment free of charge to 427 PLHIV. We analyzed factors associated with HCV treatment success among PLHIV in Kyrgystan.

Metabolic disturbance plays a critical role in the initiation of colorectal cancer (CRC), yet the identification of metabolites that are useful for early detection of CRC and its precursor lesions remains elusive. We conducted an untargeted plasma metabolomic profiling by liquid chromatography-mass spectrometry in a two-stage case-control study, including 219 CRC cases, 164 colorectal adenoma (CRA) cases, and 219 normal controls (NC) as a training set, and 91 CRC, 115 CRA, and 109 NC as a validation set. Among 891 named metabolites, 239 were significantly altered in CRC versus NC, 26 in CRA versus NC, and 88 in CRC versus CRA within the training set. The results were stable when adjusting for potential confounders. A panel of 10 metabolites, including six lipid species, one benzenoid, one organoheterocyclic compound, one organic acid derivative, and one organic oxygen compound, showed optimal performance in discriminating CRC from NC (AUC = 0.81) in the validation. Moreover, a panel of seven metabolites exhibited optimal performance in discriminating CRA from NC, with an AUC of 0.89. Our findings provide novel evidence supporting specific plasma metabolites, particularly those implicated in lipid metabolism, as promising biomarkers for the early detection of CRC.

Understanding sex disparities in modifiable risk factors across the lifespan is essential for crafting individualized intervention *** aim to investigate age-related sex disparity in cardiometabolic phenotypes in a large nationwide Chinese cohort.A total of 254,670 adults aged 40 years or older were selected from a population-based cohort in *** sex disparities in the prevalence of metabolic diseases were observed across different age strata,particularly for dyslipidemia and its com*** additive models were employed to characterize phenotype features,elucidating how gender differences evolve with advancing *** of the 16 phenotypes consistently exhibited no sex differences,while four(high-density lipoprotein[HDL]cholesterol,apolipoprotein A1,diastolic blood pressure,and fasting insulin)displayed significant sex differences across all age ***,apolipoprotein B,non-HDL cholesterol,and total cholesterol demonstrated significant age-dependent sex dis***,premenopausal females exhibited significant age-related differences in lipid levels around the age of 40-50 years,contrasting with the relatively stable associations observed in males and postmenopausal *** played an important but not sole role in age-related sex differences in blood *** duration also had an age-and sex-dependent impact on *** analysis and K-means clustering further revealed that 58.6%of the 263 measured lipids varied with sex and age,with sphingomyelins,cholesteryl esters,and triacylglycerols being the most profoundly influenced lipid species by the combined effects of age,sex,and their *** findings underscore the importance of age consideration when addressing gender disparities in metabolic diseases and advocate for personalized,age-specific prevention and management.

Objective:Dental clinic oropharyngeal human papillomavirus (HPV) patient education remains understudied. Using descriptive analysis, this study aimed to understand Appalachian Ohio dental clinics’ provider knowledge on HPV, structural compositions for education, patient education approaches, and ***:Surveyed Appalachian Ohio general and pediatric dentists responded to HPV education attitude statements and estimated their HPV vaccine recommendation frequency. Staff completed environmental scans about staff composition, structural capacity for general education, and HPV educational efforts and materials. Descriptive analyses were compared by clinic ***:All dentists (n= 14) believed patients were at-risk for HPV-related cancers and most (≥64%) agreed that the HPV vaccine was safe and effective. Most felt unprepared to educate patients on HPV (64%) and few frequently recommended the HPV vaccine (14%). No clinic shared materials with patients despite most having sufficient display space (75% for ≥2 material types). Older dentist clinics were more actively engaged in HPV education, made more vaccine recommendations, and had more capacity for additional education; younger dentist clinics had more personnel that were more confident in their communication skills and the HPV ***:Appalachian dentists rarely educated patients about HPV. Targeted educational materials and training are needed to support providers in providing patient education.

Background:Health insurance education could mitigate financial toxicity experienced by young adult (YA) cancer survivors by increasing confidence when navigating cancer care costs. This paper describes the protocol in a randomized controlled trial (RCT) to test a virtual patient navigation program designed to help YA cancer survivors understand their health ***:This is a two-arm, multi-site (Huntsman cancer Institute, Intermountain Health) RCT wherein intervention participants receive four sessions with a patient navigator and a booklet on insurance; usual care receives the booklet. We will enroll 300 YA cancer survivors (n= 200 intervention;n= 100 usual care) diagnosed with breast, testicular, lymphoma, sarcoma, colorectal, melanoma, or thyroid cancer between the ages of 26 and 39, who have completed treatment in the past two years. All participants will complete three surveys: enrollment, 6 months, and 12 months; medical records/insurance claims data will be collected out to 18-month follow-up. The primary outcomes include improvement in health insurance literacy and financial toxicity at 6 months. Secondary outcomes include adherence to cancer surveillance guidelines at 18 months. In an effort to understand the difference between melanomas diagnosed in Australia (New South Wales) and Canada, where the incidence in New South Wales is almost three times greater than in Canada, and mortality is twice as high although survival is slightly more favorable, we had one pathologist review 1,271 melanomas from British Columbia and Ontario, Canada, to compare these to melanomas in New South Wales, Australia. We hypothesized that histopathologic characteristics might provide insight into divergent pathways to melanoma development. We found a number of differences in risk factors and tumor characteristics between the two geographic areas. There were higher mole counts and darker phenotypes in the Canadian patients, while the Australian patients had greater solar elastosis, more lentigo maligna melanomas, and more tumor infiltrating lymphocytes. We hypothesize that the differences observed may illustrate different etiologies – the cumulative exposure pathway among Australian patients and the nevus pathway among Canadian patients. This is one of the largest studies investigating the divergent pathway hypothesis and is particularly robust due to the evaluation of all lesions by one dermatopathologist. © 2021 The Authors

Background: Stillbirths are a major public health issue and a sensitive marker of the quality of care around pregnancy and birth. The UN Global Strategy for Women's, Children's and Adolescents’ Health (2016–30) and the Every Newborn Action Plan (led by UNICEF and WHO) call for an end to preventable stillbirths. A first step to prevent stillbirths is obtaining standardised measurement of stillbirth rates across countries. We estimated stillbirth rates and their trends for 195 countries from 2000 to 2019 and assessed progress over time. Methods: For a systematic assessment, we created a dataset of 2833 country-year datapoints from 171 countries relevant to stillbirth rates, including data from registration and health information systems, household-based surveys, and population-based studies. After data quality assessment and exclusions, we used 1531 datapoints to estimate country-specific stillbirth rates for 195 countries from 2000 to 2019 using a Bayesian hierarchical temporal sparse regression model, according to a definition of stillbirth of at least 28 weeks’ gestational age. Our model combined covariates with a temporal smoothing process such that estimates were informed by data for country-periods with high quality data, while being based on covariates for country-periods with little or no data on stillbirth rates. Bias and additional uncertainty associated with observations based on alternative stillbirth definitions and source types, and observations that were subject to non-sampling errors, were included in the model. We compared the estimated stillbirth rates and trends to previously reported mortality estimates in children younger than 5 years. Findings: Globally in 2019, an estimated 2·0 million babies (90% uncertainty interval [UI] 1·9–2·2) were stillborn at 28 weeks or more of gestation, with a global stillbirth rate of 13·9 stillbirths (90% UI 13·5–15·4) per 1000 total births. Background:Research on calcium intake as well as variants in the calcium sensor receptor ( gene and their interaction in relation to CRC survival is still limited. Methods:Data from 18,952 CRC patients, were included. Associations between primarily pre-diagnostic dietary ( = 13.085), supplemental ( = 11,837), total calcium intake ( = 5970) as well as 325 single nucleotide polymorphisms (SNPs) of the gene ( = 15,734) in relation to CRC-specific and all-cause mortality were assessed using Cox proportional hazard models. Also interactions between calcium intake and variants in the gene were assessed. Results:During a median follow-up of 4.8 years (IQR 2.4-8.4), 6801 deaths occurred, of which 4194 related to CRC. For all-cause mortality, no associations were observed for the highest compared to the lowest sex- and study-specific quartile of dietary (HR 1.00, 95%CI 0.92-1.09), supplemental (HR 0.97, 95%CI 0.89-1.06) and total calcium intake (HR 0.99, 95%CI 0.88-1.11). No associations with CRC-specific mortality were observed either. Interactions were observed between supplemental calcium intake and several SNPs of the gene. Conclusion:Calcium intake was not associated with all-cause or CRC-specific mortality in CRC patients. The association between supplemental calcium intake and all-cause and CRC-specific mortality may be modified by genetic variants in the gene.
