In recent years, treatment of chronic lymphocytic leukemia(CLL) has changed considerably, in favor of a tailored and chemo-free approach, with the introduction of target drugs, such as BTK inhibitors (BTKi). Continuous BTKi, thanks to a prolonged disease control and low hematological toxicities, improved therapeutic results and responses close to eradicationeven in patients with an unfavorable risk profile [del(17p), TP53 m, unmutated IGHV genes (uIGHV)]. Acalabrutinib, a covalent second-generation BTKi, demonstrated similar progression free survival (PFS) and overall survival (OS) of Ibrutinib, the first in class BTKi, in pts carrying del(17p) and/or del(11q), while showing a lower rate of treatment discontinuation for adverse events (AE) due to a more favorable toxicity profile. Despite these data, in clinical practice it's still unclear whether there's a difference of efficacy of Acalabrutinib in other high-risk (HR) subgroups ( TP53 m, uIGVH) and whether there might be a subgroup that could be a better candidate for treatment with Acalabrutinib. The primary aim of the study was to evaluate the efficacy of Acalabrutinib in pts with HR CLL and to identify which subgroup may benefit the most from its use. The secondary aims were to describe clinical-biological features of pts with HR CLL candidates to Acalabrutinib in real life and to assess its tolerability profile. A retrospective analysis was conducted on 129 pts with HR CLL [del(17p), TP53 m and/or uIGHV], who started Acalabrutinib between June 2021 and June 2023, from 14 Italian centers. Pts were divided into two groups: the treatment-naïve (TN) group (n = 98) and the relapsed/refractory (RR) group (n = 31). Median ages were 70 and 75 years for TN and RR pts, respectively. For both, we analyzed survival data by stratifying pts into further subgroups according to HR biological features [10 TN and 2 RR pts with del(17p) and/or TP53 m only, 68 TN and 18 RR with uIGHV only, 18 TN and 9 RR with uIGHV with del(17p)

A national survey of primary care physicians: Perceptions and practices of low-dose CT lung cancer screening

Soon after the National Lung Screening Trial, organizations began to endorse low-dose computed tomography (LCDT) screening for lung cancer in high-risk patients. Concerns about the risks versus benefits of screening, as well as the logistics of identifying and referring eligible patients, remained among physicians. This study aimed to examine primary care physicians' knowledge, attitudes, referral practices, and associated barriers regarding LDCT screening. We administered a national survey of primary care physicians in the United States between September 2016 and April 2017. Physicians received up to 3 mailings, 1 follow-up email, and received varying incentives to complete the survey. Overall, 293 physicians participated, for a response rate of 13%. We used weighted descriptive statistics to characterize participants and their responses. Over half of the respondents correctly reported that the US Preventive Services Task Force recommends LDCT screening for high-risk patients. Screening recommendations for patients not meeting high-risk criteria varied. Although 75% agreed that the benefits of LDCT screening outweigh the risks, fewer agreed that there is substantial evidence that screening reduces mortality (50%). The most commonly reported barriers to ordering screening included prior authorization requirements (57%), lack of insurance coverage (53%), and coverage denials (31%). The most frequently cited barrier to conducting LDCT screening shared decision making was patients' competing health priorities (42%). Given the impact of physician recommendations on cancer screening utilization, further understanding of physicians' LDCT screening attitudes and shared decision-making practices is needed. Clinical practice and policy changes are also needed to engage more patients in screening discussions. © 2018

Background: For severe asthma (SA) management, real-world evidence on the effects of biologic therapies in reducing the burden of oral corticosteroid (OCS) use is limited. Objective: To estimate the efficacy of biologic initiation on total OCS (TOCS) exposure in patients with SA from real-world specialist and primary care settings. Methods: From the International Severe Asthma Registry (ISAR, specialist care) and the Optimum Patient Care Research Database (OPCRD, primary care, United Kingdom), adult biologic initiators were identified and propensity score-matched with non-initiators (ISAR, 1:1;OPCRD, 1:2). The impact of biologic initiation on TOCS (including bursts for exacerbations) daily dose in the first- and second-year follow-up period was estimated using multivariable generalized linear models. Results: Among 5,663 patients (ISAR 48%, OPCRD 52%), the odds ratios (ORs) of biologic initiators achieving TOCS cessation in the first and second years of follow-up were 2.38 (95% CI, 1.87-3.04) and 2.11 (95% CI, 1.65-2.70), whereas the ORs of low (0- to 5-mg) TOCS intake were 1.62 (95% CI, 1.40-1.86) and 1.40 (95% CI, 1.21-1.61), respectively. Compared with non-initiators, biologic initiators had a substantially higher chance of achieving greater than 75% reduction from baseline (OR [95% CI] = 2.35 [2.06-2.68] and 1.53 [1.35-1.73] in first and second years, respectively). These findings remained persistent and robust when analyses were repeated with one country setting removed at a time. Conclusions: Biologic initiation in patients with SA led to substantial reduction in TOCS exposure, particularly in the first year. Future analyses will explore the impact on OCS-related adverse health events. © 2025 The Authors

Background: Esophageal cancer is associated with substantial disease burden in China, and data on the economic burden are fundamental for setting priorities in cancer interventions. The medical expenditure for the diagnosis and treatment of esophageal cancer in China has not been fully quantified. This study aimed to examine the medical expenditure of Chinese patients with esophageal cancer and the associated ***: From 2012 to 2014, a hospital-based multicenter retrospective survey was conducted in 37 hospitals in 13 provinces/municipalities across China as a part of the cancer Screening program of Urban China. For each esophageal cancer patient diagnosed between 2002 and 2011, clinical information and expense data were extracted by using structured questionnaires. All expense data were reported in Chinese Yuan(CNY; 1 CNY = 0.155 USD) based on the2011 value and inflated using the year-specific health care consumer price index for ***: A total of 14,967 esophageal cancer patients were included in the analysis. It was estimated that the overall average expenditure per patient was 38,666 CNY, and an average annual increase of 6.27% was observed from 2002(25,111 CNY) to 2011(46,124 CNY). The average expenditures were 34,460 CNY for stage Ⅰ,39,302 CNY for stage Ⅱ,40,353 CNY for stage Ⅲ, and 37,432 CNY for stage IV diseases(P < 0.01). The expenditure also differed by the therapy type, which was 38,492 CNY for surgery, 27,933 CNY for radiotherapy, and 27,805 CNY for chemotherapy(P < 0.05).Drugs contributed to 45.02% of the overall ***: These conservative estimates suggested that medical expenditures for esophageal cancer in China substantially increased in the last 10 years, treatment for early-stage esophageal cancer costs less than that for advanced cases, and spending on drugs continued to account for a considerable proportion of the overall expenditure.

Use and Predictors of Expectant Management of Low and Intermediate Risk Localized Prostate cancer in Elderly Men

Introduction Expectant management reduces overtreatment in low risk but not intermediate risk localized prostate cancer. We assessed the use and predictors of expectant management to understand its uptake in practice in the United States. Methods Using the SEER (Surveillance, epidemiology and End Results)-Medicare database, we conducted a retrospective cohort study of men 66 years or older diagnosed with low (25,506 patients) or intermediate risk (25,597) localized prostate cancer between 2004 and 2011 and followed through December 31, 2012. We defined expectant management as 1) no definitive therapy and at least 1 prostate specific antigen test or repeat biopsy 4 to 12 months after diagnosis or 2) receiving definitive therapy after prostate specific antigen testing or repeat biopsy 7 to 12 months after diagnosis. We performed separate analyses for low and intermediate risk groups using multiple logistic regressions. Results For men diagnosed with prostate cancer in 2004 to 2011 expectant management increased from 22% to 43% in the low risk group and from 15% to 18% in the intermediate risk group. In the low risk group expectant management increased with age (adjusted OR 1.26 for age 71 to 75 years, 2.21 for 76 to 80 years, 6.33 for greater than 80 years;all p <0.0001 compared to age 66 to 70 years). Expectant management uptake was higher among men with comorbidities (OR 1.29) and those residing in the Pacific region (adjusted OR 0.56 compared to East Coast). Conclusions In United States practice utilization of expectant management has steadily increased in low risk prostate cancer and remained low in intermediate risk prostate cancer through time. While men with comorbidities or advanced age were more likely to undergo expectant management, its use varied substantially by geographic region. Our findings bring attention to the presence of multiple barriers to expectant management implementation. © 2017 American Urological Association Education and Research, Inc.

High Spicy Food Intake and Risk of cancer： A Meta-analysis of Case-control Studies

Background：Studies on the association between spicy food intake and cancer risk have reported inconsistent *** quantitatively assessed this association by conducting a meta-analysis based on evidence from case-control ***：PubMed,EMBASE,and the Cochrane Library were searched for eligible *** odds ratios （ORs） with their 95% confidence interval （CI） were calculated using a random-or fixed-effects *** methodological quality of the included articles was assessed using the Newcastle-Ottawa scale （NOS）.All data were analyzed using STATA 1 1.0 software （version 11.0;StataCorp.,College Station,TX,USA）.Subgroup analyses were also performed with stratification by region,sex,number of cases,cancer subtype,source of the control group,and NOS ***：A total 39 studies from 28 articles fulfilled the inclusion criteria for the meta-analysis （7884 patients with cancer and 10,142 controls）.Comparison of the highest versus lowest exposure category in each study revealed a significant OR of 1.76 （95% CI =1.35-2.29） in spite of significant heterogeneity （P 〈 0.00l）.In the subgroup analyses,this positive correlation was still found for gastric cancer,different regions,different numbers of cases,different sources of the control group,and high-quality articles （NOS score of≥ 7）.However,no statistically significant association was observed for women,esophageal cancer,gallbladder cancer,or low-quality articles （NOS score of〈7）.No evidence of publication bias was ***：Evidence from case-control studies suggested that a higher level of spicy food intake may be associated with an increased incidence of cancer despite significant *** studies are warranted to clarify our understanding of the association between high spicy food intake and the risk of cancer.

Summary Being the second-largest country in the Middle East, Iran has a long history of civilisation during which several dynasties have been overthrown and established and health-related structures have been reorganised. Iran has had the replacement of traditional practices with modern medical treatments, emergence of multiple pioneer scientists and physicians with great contributions to the advancement of science, environmental and ecological changes in addition to large-scale natural disasters, epidemics of multiple communicable diseases, and the shift towards non-communicable diseases in recent decades. Given the lessons learnt from political instabilities in the past centuries and the approaches undertaken to overcome health challenges at the time, Iran has emerged as it is today. Iran is now a country with a population exceeding 80 million, mainly inhabiting urban regions, and has an increasing burden of non-communicable diseases, including cardiovascular diseases, hypertension, diabetes, malignancies, mental disorders, substance abuse, and road injuries.

Association between dietary inflammatory index and high-sensitive C-reactive protein levels in cancer screening in Japanese

Introduction Inflammation is related to the development of many cancers. Diet plays an important role in the regulation of inflammation. The dietary inflammatory index (DII ® ) is a literature-derived dietary index designed to assess the impact of diet on individual inflammation status. However, most validation studies of DII conducted to date have been done in Western countries, indicating the need for evaluation in Asian populations. The aim of this study was to validate DII scores derived from a food frequency questionnaire (FFQ) with high-sensitive C-reactive protein (hs-CRP) levels in participants undergoing cancer screening in Japan. Methods In cancer screenings conducted from 2009 to 2013 in the National cancer Center in Japan, 7919 participants aged 40–70 years answered the FFQ and received blood analysis for hs-CRP. Of these, we excluded participants with a body mass index (BMI) of < 14 kg/m 2 or > 40 kg/m 2 ( n = 13); missing data on smoking status ( n = 1) or metabolic equivalents (Mets, n = 1); history of cancer, stroke, or myocardial infarction ( n = 877); extreme energy intake (upper and lower 2.5 percentiles, n = 349); missing data on alcohol consumption ( n = 120); and missing data of hs-CRP, or hs-CRP > 1.0 mg/dL ( n = 84). Finally, 6474 participants (3825 for men and 2649 for women) were included in this analysis. DII was calculated from crude intake from the FFQ and adjusted for energy using the density method. Adjusted geometric mean levels of hs-CRP were calculated using a multivariable linear regression model by quartiles of DII score. P for trend across quartiles of DII score were calculated using DII quartiles as continuous variables. Further, logistic regression analysis was also applied to estimate odds ratio (OR) [95% confidence interval (95% CI)] of hs-CRP > 0.3 mg/dL across quartiles of DII score.
