1194. Carbapenem-Resistant Enterobacteriaceae in Kentucky: Initial 6 Months of Mechanism Testing

Background A global rise in carbapenem-resistant Enterobacteriaceae (CRE) has been noted over the past two decades. State and local data on CRE are necessary to better inform public health interventions. Methods Reporting of CRE (i.e., Enterobacteriaceae resistant to any carbapenem or shown to produce a carbapenemase) was mandated in Kentucky in 2015. Voluntary submission of isolates to the Antibiotic Resistance Laboratory Network regional laboratory for carbapenemase testing began September 2017. Demographic data collected as part of reporting included age, sex, county of residence, and inpatient/outpatient status. Descriptive and chi-square analyses were performed. Results Between September 1, 2017 and February 28, 2018, 149 CRE were reported to the Kentucky department for Public Health. Testing for presence of a carbapenemase was performed on 115 isolates (77.2%); 44 (38.3%) were carbapenemase producing (CP)-CRE and Klebsiella pneumoniae carbapenemase (KPC) was identified from 38 (86.4%). Also identified were Verona integron-encoded metallo-β-lactamase (VIM; 5, 11.4%) and New Delhi metallo-β-lactamase (NDM; 1, 2.3%). Identification of carbapenemase varied among genera: Citrobacter (3/4, 75%), Klebsiella (21/40, 52.5%), Serratia (2/5, 40%), Escherichia (6/20, 30%), Enterobacter (11/41, 26.8%), Proteus (0/4, 0%), other genera (1/2, 50%). CRE isolates from urban or suburban areas were more likely CP-CRE than were those from rural areas (30/65, 46.2% vs. 14/50, 28%, P = 0.047). Carbapenemase was identified more often among CRE isolates from currently hospitalized patients than from patients whose cultures were collected outside of an acute care hospital (37/70, 52.8% vs. 7/45, 15.6%; P < 0.001). Conclusion The percentage of CRE that were CP-CRE in Kentucky was comparable with that reported for the United States (38 vs. 32%). Tertiary care cancer centers can act as reference units for prevention and early detection programs. The purpose of this study was to describe the profile of the cancer prevention Campaign (CPC) participants at the A.C. Camargo cancer Center in Sao Paulo, Brazil. A cross-sectional, quantitative study was carried out to analyze data of the CPC participants at the A.C. Camargo cancer Center. Participants were recruited from the general public during lectures delivered by CPC members in selected parts of the city of São Paulo. Data included gender analysis, number of cancer cases diagnosed, topography and tumor/node/metastasis (TNM) stage, in the period 2008–2012. Sixty-two thousand nine hundred one people participated in CPC during the 5-years period (2008–2012). A total of 65 cancer cases were diagnosed, which represents 0,1% of detection rate: 25 cases were identified in men and 40 in women. The most frequent neoplasms in men were thyroid, prostate and oral cavity cancers while in women, thyroid and breast cancers were more frequent. cancer prevention activities in tertiary care cancer centers raise awareness in its employees and in the general population, increasing early diagnosis.

Background:Leptospirosis morbidity and mortality rates in China have decreased since the *** analyses of the spatiotemporal and demographic changes occurring in the last decade and its implication on estimates of disease burden are required to inform intervention *** this study,we quantified the epidemiological shift and geographical heterogeneity in the burden of leptospirosis during 2005-2015 in ***:We used reported leptospirosis case data from 1st January 2005 to 31st of December 2015 that routinely collected by the China Information System for Disease control and prevention(CISDCP)to analyze the epidemiological trend and estimate the burden in terms of disability-adjusted life-years(DALYs)over space,time,and demographical ***:A total of 7763 cases were reported during *** which,2403(31%)cases were the laboratoryconfirmed *** 2005,the notified incidence rate was gradually decreased(P<0.05)and it was relatively stable during 2011-2015(P>0.05).During 2005-2015,we estimated a total of 10313 DALYs were lost due to leptospirosis comprising a total of 1804 years-lived with disability(YLDs)and 8509 years-life lost(YLLs).Males had the highest burden of disease(7149 DALYs)compared to females(3164 DALYs).The highest burden estimate was attributed to younger individuals aged 10-19 years who lived in southern provinces of *** 2005-2015,this age group contributed to approximately 3078 DALYs corresponding to 30%of the total DALYs lost in ***,our analysis indicated a declining trend in burden estimates(P<0.001)since 2005 and remained relatively low during *** burden estimates have been identified in the endemic regions where infections principally *** of the changes in DALY estimates were driven by changes in ***:In the last 11-years,the burden estimates of leptospirosis have shown a declining trend across the country;however,leptospirosis should not be neglected as it re

Cox regression analysys with missing covariates via nonparametric multiple imputation

We consider the situation of estimating Cox regression in which some covariates are subject to missing, and there exists additional informa-tion (including observed event time, censoring indicator and fully observed covariates) which may be predictive of the missing covariates. We propose to use two working regression models: One for predicting the missing co-variates and the other for predicting the missing probabilities. For each missing covariate observation, these two working models are used to define a nearest neighbor imputing set. This set is then used to nonparametrically impute covariate values for the missing observation. Upon the completion of imputation, Cox regression is performed on the multiply imputed datasets to estimate the regression coefficients. In a simulation study, we compare the nonparametric multiple imputation approach with the augmented in-verse probability weighted (AIPW) method, which directly incorporates the two working models into estimation of Cox regression. We show that all approaches can reduce bias due to non-ignorable missing mechanism. The proposed nonparametric imputation method is robust to mis-specification of either one of the two working models and robust to mis-specification of the link function of the two working models. In contrast, the AIPW method is not robust to mis-specification of the link function of the two working mod-els and is sensitive to the selection probability. Genome-wide association studies (GWAS) have identified >250 loci for body mass index (BMI), implicating pathways related to neuronal biology. Most GWAS loci represent clusters of common, noncoding variants from which pinpointing causal genes remains challenging. Here we combined data from 718,734 individuals to discover rare and low-frequency (minor allele frequency (MAF) < 5%) coding variants associated with BMI. We identified 14 coding variants in 13 genes, of which 8 variants were in genes (ZBTB7B, ACHE, RAPGEF3, RAB21, ZFHX3, ENTPD6, ZFR2 and ZNF169) newly implicated in human obesity, 2 variants were in genes (MC4R and KSR2) previously observed to be mutated in extreme obesity and 2 variants were in GIPR. The effect sizes of rare variants are ~10 times larger than those of common variants, with the largest effect observed in carriers of an MC4R mutation introducing a stop codon (***35Ter, MAF = 0.01%), who weighed ~7 kg more than non-carriers. Pathway analyses based on the variants associated with BMI confirm enrichment of neuronal genes and provide new evidence for adipocyte and energy expenditure biology, widening the potential of genetically supported therapeutic targets in obesity.
