Health Economics Research in Primary prevention of cancer: Assessment, Current Challenges, and Future Directions

In the past 2 decades, the demand for information on health economics research to guide health care decision making has substantially increased. Studies have provided evidence that eliminating or reducing tobacco use;eating a healthy diet, including fruit and vegetables;being physically active;reducing alcohol consumption;avoiding ultraviolet radiation;and minimizing exposure to environmental and occupational carcinogenic agents should substantially reduce cancer incidence in the population. The benefits of these primary prevention measures in reducing cancer incidence are not instantaneous. Therefore, health economics research has an important role to play in providing credible information to decision makers on the health and economic benefits of primary prevention. This article provides an overview of health economics research related to primary prevention of cancer. We addressed the following questions: 1) What are the gaps and unmet needs for performing health economics research focused on primary prevention of cancer? 2) What are the challenges and opportunities to conducting health economics research to evaluate primary prevention of cancer? and 3) What are the future directions for enhancing health economics research on primary prevention of cancer? Modeling primary prevention of cancer is often difficult given data limitations, long delays before the policy or intervention is effective, possible unintended effects of the policy or intervention, and the necessity of outside expertise to understand key inputs or outputs to the modeling. Despite these challenges, health economics research has an important role to play in providing credible information to decision makers on the health and economic benefits of primary prevention of cancer. © 2022 Published by Oxford University Press.

Background: Breast cancer (BC) incidence and mortality vary significantly across countries, highlighting the need to update the global burden of female BC, including current trends and future projections. Methods: Data were sourced from GLOBOCAN 2022, including estimated new cases and deaths from BC across 21 United Nation (UN) regions and 185 countries, the age-standardized incidence rate (ASIR) and mortality rate (ASMR), the estimated annual percentage changes (EAPC), and demographic projections through 2050. The region-specific and country-specific BC burden for women of all ages and for young women (< 40 years old) was reorganized and re-plotted to highlight subgroup differences. Linear regression was used to explore the link between ASIR/ASMR and the human development index (HDI). Transitioning countries referred to those with low or medium HDI, while transitioned countries were those with high or very high HDI. Results: In 2022, an estimated 2.3 million new BC cases and 666,000 BC-related deaths occurred globally, accounting for 23.8 % and 15.4 % of all cancer cases and deaths in women, respectively. Regionally, Eastern Asia reported the highest number of cases (480,019, ASIR: 37.54/100,000), while South-Central Asia had the highest number of deaths (135,348, ASMR: 13.41/100,000). At the country level, China had the highest number of cases due to its large population, whereas India reported the highest number of deaths. ASIR for both overall and early-onset BC increased with HDI, while ASMR for early-onset BC decreased with HDI (P < 0.05). Overall BC showed an increasing trend in ASIR during 2003–2015 (EAPC: 0.92 %) and a decreasing trend in ASMR during 2006–2016 (EAPC:-1.06 %). Early-onset BC showed a more significant rise in ASIR (EAPCs: 1.4 %) and a slight increase in ASMR (EAPCs: 0.16 %). If national rates remain stable, BC cases and deaths will increase by 54.7 % and 70.9 %, respectively, by 2050. Notably, increased early-onset BC cases are only observed

Globally,lung cancer is the most prevalent cancer(11.4%of all cancers)and the leading cause of cancer deaths(18%of all cancers)[1],[2].With one-fifth of the global population,China is a top contributor to the lung cancer burden,accounting for 37%of new cases and 30%of deaths worldwide[2].Several studies have estimated the tracheal,bronchus,and lung cancer(TBLC)burden at the global and national levels[3],[4],[5].However,the drivers and patterns of TBLC burden in China are different from those in other *** China’s vast territory,uneven economic development,and disparities in medical resources of China,a detailed description and analysis of the current and temporal trends in TBLC and its attributable risks across different Chinese populations would help to identify priority populations that require attention and targeted strategies to reduce the TBLC burden.

Objectives: To evaluate changes in the age at menarche in Asian populations. Study design: Retrospective cohort study. Methods: We included 548,830 women from six countries in Asia. The data were sourced from 20 cohorts participating in the Asia Cohort Consortium (ACC) and two additional cohort studies: Japan Multi-institutional Collaborative Cohorts (J-MICC), and Japan Nurse Health Study (JNHS) with data on age at menarche. Joinpoint regression was used to evaluate changes in age at menarche by birth year and by country. Results: The study includes data from cohorts in six Asian countries namely, China, Iran, Japan, Korea, Malaysia and Singapore. Birth cohorts ranged from 1873 to 1995. The mean age of menarche was 14.0 years with a standard deviation (SD) of 1.4 years, ranged from 12.6 to 15.5 years. Over 100 years age at menarche showed an overall decrease in all six countries. China showed a mixed pattern of decrease, increase, and subsequent decrease from 1926 to 1960. Iran and Malaysia experienced a sharp decline between about 1985 and 1990, with APC values of −4.48 and −1.24, respectively, while Japan, South Korea, and Singapore exhibited a nearly linear decline since the 1980s, notably with an APC of −3.41 in Singapore from 1993 to 1995. Conclusions: Overall, we observed a declining age at menarche, while the pace of the change differed by country. Additional long-term observation is needed to examine the contributing factors of differences in trend across Asian countries. The study could serve as a tool to strengthen global health campaigns. © 2024 The Royal Society for Public Health

While male breast cancer (MBC) remains a rare and understudied disease, comparatively little is known about its prognosis in contrast to female breast cancer (FBC). There is a paucity of large population-based studies comparing the prognosis of MBC patients to FBC patients in Japan. This study analyzed 181,540 breast cancer cases, 1058 (0.6%) males and 180,482 (99.4%) females, from 12 prefectures in Japan diagnosed between 1993 and 2011. Five- and ten-year net survival (NS) were estimated and stratified by sex, period, age, stage, and histological groups. Excess hazard ratios (EHR) were adjusted for period, age, stage, and histological group. Cochran's Q test was utilized to assess heterogeneity across these factors. The overall 5- and 10-year NS estimates for MBC patients were 90.7% (95% CI: 86.3%–93.7%) and 83.7% (95% CI: 72.2%–90.8%), respectively, while those for FBC patients were 88.3% (95% CI: 88.1%–88.5%) and 79.1% (95% CI: 78.7%–79.4%), respectively. The survival of MBC patients was comparable to that of FBC patients, with EHR of 0.88 [95% CI: 0.70–1.09] and 0.86 [95% CI: 0.69–1.07] for 5- and 10-year survival, respectively. Heterogeneity analysis revealed no significant sex-based differences in survival across these strata. This study offers a comprehensive analysis of breast cancer survival in Japanese men and women and enhances understanding of MBC prognosis relative to FBC. © 2025 The Author(s). cancer Science published by John Wiley & Sons Australia, Ltd on behalf of Japanese cancer Association.

A Bayesian Joint Modelling for Misclassified Interval-censoring and Competing Risks

In active surveillance of prostate cancer, cancer progression is interval-censored and the examination to detect progression is subject to misclassification, usually false negatives. Meanwhile, patients may initiate early treatment before progression detection, constituting a competing risk. We developed the Misclassification-Corrected Interval-censored Cause-specific Joint Model (MCICJM) to estimate the association between longitudinal biomarkers and cancer progression in this setting. The sensitivity of the examination is considered in the likelihood of this model via a parameter that may be set to a specific value if the sensitivity is known, or for which a prior distribution can be specified if the sensitivity is unknown. Our simulation results show that misspecification of the sensitivity parameter or ignoring it entirely impacts the model parameters, especially the parameter uncertainty and the baseline hazards. Moreover, specification of a prior distribution for the sensitivity parameter may reduce the risk of misspecification in settings where the exact sensitivity is unknown, but may cause identifiability issues. Thus, imposing restrictions on the baseline hazards is recommended. A trade-off between modelling with a sensitivity constant at the risk of misspecification and a sensitivity prior at the cost of flexibility needs to be decided. © 2024, CC BY-NC-ND.

Pilot Study of a Cognitive Behavioral Intervention for Pelvic Exam Anxiety Among Survivors of Endometrial and Cervical cancer

No evidence-based interventions exist for anxiety related to pelvic exams after cancer treatment. A 3-session cognitive behavioral intervention, Pelvic Examination and Anxiety Coping Skills for Empowerment (PEACE), was developed. We conducted a proof-of concept pilot study (N=16) to evaluate intervention acceptability and feasibility and changes in anxiety and pain symptoms. Participants completed 3 assessments (baseline, post-intervention, post-pelvic exam) and post-exam interviews. Exploratory outcomes included exam-related distress and pain, and intervention mechanisms of change. The mixed methods findings indicated preliminary acceptability (e.g., 71% endorsed PEACE “made things a lot better”) and feasibility (e.g., 87.5% completed the protocol). PEACE shows promise for reducing procedural distress, which may ultimately improve care engagement. © 2024 Society for Menstrual Cycle Research.
