Telepsychiatry:The Evaluation and Treatment of Seniors in Rural Retirement Communities

Few studies examine the benefits from geriatric telepsychiatry in rural retirement ***:*** demonstrate that using telepsychiatry(a)standard approaches to psychiatric evaluation would yield diagnoses in Diagnostic and Statistical Manual of Mental Disorders(DSM-V);(b)psychotherapies and pharmacotherapy could be effectively *** examine the relationships among cognition,mood,agitation and functions at baseline and the response to treatment over ***:Prospective longitudinal ***:Geriatric Depression scale(GDS),Mini Mental State Exam(MMSE),Barthel Index(BI),Pittsburgh Agitation Scale(PAS).Setting:Video ***:Psychotherapy,***:428 Seniors over 55,met criteria for at least one DSM-V ***:Treatments were administered for a full range of psychiatric diagnoses and age-related medical *** most frequently prescribed pharmacological agents were:antidepressants(78%)antipsychotics(64%),memory enhancers(38%).Participants(66%)received psychotherapy:individual(31%),couple(7%),family(13%).Variation in the MMSE scores were observed:55%remained stable,11%declined,18%*** Scores improved from baseline to 26 weeks(p=0.02,d=0.99:95%CI 0.39-1.56).PAS scores declined from baseline to 52 weeks(McNemar’s S=11.27,p=0.0008,d=1.17:95%CI 0.63-1.68).Function(BI)at week 26 was not statistically significantly different from baseline(t(26)=1.66,p=0.11,d=0.65:95%CI-0.16-1.42).Participants maintained independence(64.5%)at 52 weeks(McNemar’s S=6.23,p=0.013,d=0.79:95%CI 0.19-1.36)Conclusion:This study demonstrates the feasibility and benefit of providing a full complement of services via telepsychiatry to seniors and provides a rationale for more comprehensive reimbursement plans.

Climate change refers to substantial long-term variations in weather patterns. In this work, we employ a Machine Learning (ML) technique, the Random Forest (RF) algorithm, to forecast the monthly average temperature for Brazilian’s states capitals (27 cities) and the whole country, from January 1961 until December 2022. To forecast the temperature at k-month, we consider as features in RF: i) global emissions of carbon dioxide (CO2), methane (CH4), and nitrous oxide (N2O) at k-month;ii) temperatures from the previous three months, i.e., (k − 1), (k − 2) and (k − 3)-month;iii) combination of i and ii. By investigating breakpoints in the times series, we discover that 24 cities and the gases present breakpoints in the 80’s and 90’s. After the breakpoints, we find an increase in the temperature and the gas emission. Thereafter, we separate the cities according to their geographical position and employ the RF algorithm to forecast the temperature from 2010-08 until 2022-12. Based on i, ii, and iii, we find that the three inputs result in a very precise forecast, with a normalized root mean squared error (NMRSE) less than 0.083 for the considered cases. From our simulations, the better forecasted region is Northeast through iii (NMRSE = 0.012). Furthermore, we also investigate the forecasting of anomalous temperature data by removing the annual component of each time series. As the intensity of global environmental challenges increases, so does the need to understand and strengthen the resilience of interconnected systems. This research underscores the crucial role of adaptive governance in navigating the complexities of the Water-Energy-Food-Ecosystem nexus, particularly under the pressures of changing climatic conditions. The study conducts an in-depth analysis of adaptive capacities by identifying critical components such as assets, flexibility, organization, learning, and agency as fundamental elements of adaptive governance in sanitation practices. Data were collected through structured questionnaires and analyzed using binary probit and logit models to examine how these attributes influence community engagement in sanitation. Descriptive analysis was also used to summarize respondent characteristics and attribute distributions. © 2025 The Author(s)

Sports facilities have high energy requirements and therefore, presents a great opportunity for energy savings. In fact, people generate a significant sensible and latent heat because of high activity level and the amount of moisture released through exercising activities and showers. These buildings are more challenging to control because of the specific occupants requirements. In this effort, dynamic control of roof top units subject to external weather predictions is implemented on a case study of a gymnasium. Multilayer modularity belief propagation to assess detectability of community structure

Modularity based community detection encompasses a number of widely used, efficient heuristics for identification of structure in networks. Recently, a belief propagation approach to modularity optimization provided a useful guide for identifying non-trivial structure in single-layer networks in a way that other optimization heuristics have not. In this paper, we extend modularity belief propagation to multilayer networks. As part of this development, we also directly incorporate a resolution parameter. We show that adjusting the resolution parameter affects the convergence properties of the algorithm and yields different community structures than the baseline. We compare our approach with a widely used community detection tool, GenLouvain, across a range of synthetic, multilayer benchmark networks, demonstrating that our method performs comparably to the state of the art. Finally, we demonstrate the practical advantages of the additional information provided by our tool by way of two real-world network examples. We show how the convergence properties of the algorithm can be used in selecting the appropriate resolution and coupling parameters and how the node-level marginals provide an interpretation for the strength of attachment to the identified communities. Noise appears in the brain due to various sources, such as ionic channel fluctuations and synaptic events. They affect the activities of the brain and influence neuron action potentials. Stochastic differential equations have been used to model firing patterns of neurons subject to noise. In this work, we consider perturbing noise in the adaptive exponential integrate-and-fire (AEIF) neuron. The AEIF is a two-dimensional model that describes different neuronal firing patterns by varying its parameters. Noise is added in the equation related to the membrane potential. We show that a noise current can induce continuous and noncontinuous transitions in neuronal interspike intervals. Moreover, we show that the noncontinuous transition occurs mainly for parameters close to the border between tonic spiking and burst activities of the neuron without noise. We demonstrate that a first order isotropic-to-nematic phase transition in liquid crystals can be succesfully modeled within the generalized Landau-de Gennes theory by selecting an appropriate combination of elastic constants. The numerical simulations of the model established in this paper qualitatively reproduce the experimentally observed configurations that include interfaces and topological defects in the nematic phase. The standard equilibrium Green–Kubo and nonequilibrium molecular dynamics (MD) methods for computing thermal transport coefficients in solids typically require relatively long simulation times and large system sizes. To this end, we revisit here the homogeneous nonequilibrium MD method by Evans [Phys. Lett. A 91, 457 (1982)] and generalize it to many-body potentials that are required for more realistic materials modeling. We also propose a method for obtaining spectral conductivity and phonon mean-free path from the simulation data. This spectral decomposition method does not require lattice dynamics calculations and can find important applications in spatially complex structures. We benchmark the method by calculating thermal conductivities of three-dimensional silicon, two-dimensional graphene, and a quasi-one-dimensional carbon nanotube and show that the method is about one to two orders of magnitude more efficient than the Green–Kubo method. We apply the spectral decomposition method to examine the long-standing dispute over thermal conductivity convergence vs divergence in carbon nanotubes.

Three strategies for creating probabilistic forecast outlooks for El Niño-Southern Oscillation (ENSO) are compared. One is subjective and is currently used by the NOAA/Climate Prediction Center (CPC) to produce official ENSO outlooks. A second is purely objective and is based on the North American Multimodel Ensemble (NMME).Anew third strategy is proposed in which the forecaster only provides the expected value of the Niño-3.4 index, and then categorical probabilities are objectively determined based on past skill. The new strategy results in more confident probabilities compared to the subjective approach and higher verification scores, while avoiding the significant forecast busts that sometimes afflict the NMME-based objective approach. The higher verification scores of the new strategy appear to result from the added value that forecasters provide in predicting the mean, combined with more reliable representations of uncertainty, which is difficult to represent because forecasters often assume less confidence than is justified. Moreover, the new approach can produce higher-resolution probabilistic forecasts that include ENSO strength information and that are difficult, if not impossible, for forecasters to produce. To illustrate, a nine-category ENSO outlook based on the new strategy is assessed and found to be skillful. The new approach can be applied to other outlooks where users desire higher-resolution probabilistic forecasts, including the extremes. © 2019 American Meteorological Society.
