Comparative study on anti-HCV testing using plasma, dried plasma spots (DPS), and dried blood spots (DBS)

The aim of this study was to evaluate the performance of an assay using dried plasma spot(DPS)and dried blood spot(DBS)samples for the serological detection of anti-hepatitis C virus(HCV)*** January and July 2019,plasma,DPS and DBS specimens were collected from individuals at high-risk for HCV *** were tested for anti-HCV by ELISA,and the performance of DPS and DBS specimens was examined using results from the plasma testing,as the *** samples were collected from 329 persons,including 129 men who have sex with men and 200 intravenous drug *** from the plasma testing indicated that 118 samples(59.0%)were HCV *** from the DPS sample testing showed sensitivity as 99.2%(95%confidence interval[CI]:0.95-1.00)and specificity as 100%(95%CI:0.98-1.00)for HCV detection,with Kappa of 99.3%(95%CI:0.98-1.00)while in DBS sample testing the sensitivity as 98.3%(95%CI:0.93-1.00)and specificity as 100%(95%CI:0.98-1.00),with Kappa of 98.7%(95%CI:0.97-1.00),***’s correlation coefficients for the comparisons between plasma and DPS specimen,plasma and DBS specimens,DPS and DBS specimens were 0.857,0.750,and 0.739,*** with the results in plasma,1 sample was not detected using the DPS specimens,and 2 samples were failed for the positive detection,using the DBS *** DPS and DBS samples were promising alternatives to plasma,for the detection of anti-HCV antibodies.

Background: Single-tablet regimen (STR) provides a convenient once-daily regimen for the prevention of human immunodeficiency virus (hiv) infection. Here, we investigated the safety and tolerability of coformulated bictegravir/emtricitabine/tenofovir alafenamide (BIC/FTC/TAF) as a three-drug, STR for post-exposure prophylaxis (PEP) in Chinese ***: This was a prospective, open-label, single-arm trial conducted in a sexually transmitted diseases and acquired immunodeficiency syndrome clinic of a tertiary hospital in Beijing, China. Adults requiring PEP were prescribed BIC/FTC/TAF one pill once a day for 28 days. Clinical and laboratory data were collected and analyzed at baseline, weeks 2, 4, 8, 12, and ***: Of 112 participants enrolled in the study, 109 (97.3%) were male and the mean age was 30 ± 8 years. PEP completion was 96.4% (95% confidence interval: 91.1-99.0%). Two participants stopped PEP after 2 days because the source partner was identified as hiv uninfected. One participant was excluded due to hepatitis B virus infection according to the exclusion criteria. One discontinued due to the participant’s decision. No participant acquired hiv through week 24. Adherence was 98.9% (standard deviation [SD]: 3.3%) by self-reporting and 98.5% (SD: 3.5%) by pill count. Only five participants experienced mild clinical adverse events attributed to the study drug (including headache, diarrhea, and nausea) and four participants had elevated serum creatinine (grade 1).Conclusions: A once daily, STR of BIC/FTC/TAF used as PEP was safe and well-tolerated with a high rate of completion and adherence in Chinese. BIC/FTC/TAF may be a good option for PEP.

Building a Simple Model to Assess the Impact of Case Investigation and Contact Tracing for Sexually Transmitted Diseases: Lessons From COVID-19

Introduction: During the COVID-19 pandemic, the U.S. Centers for Disease control and prevention developed a simple spreadsheet-based tool to help state and local public health officials assess the performance and impact of COVID-19 case investigation and contact tracing in their jurisdiction. The applicability and feasibility of building such a tool for sexually transmitted diseases were assessed. Methods: The key epidemiologic differences between sexually transmitted diseases and respiratory diseases (e.g., mixing patterns, incubation period, duration of infection, and the availability of treatment) were identified, and their implications for modeling case investigation and contact tracing impact with a simple spreadsheet tool were remarked on. Existing features of the COVID-19 tool that are applicable for evaluating the impact of case investigation and contact tracing for sexually transmitted diseases were also identified. Results: Our findings offer recommendations for the future development of a spreadsheet-based modeling tool for evaluating the impact of sexually transmitted disease case investigation and contact tracing efforts. Generally, we advocate for simplifying sexually transmitted disease–specific complexities and performing sensitivity analyses to assess uncertainty. The authors also acknowledge that more complex modeling approaches might be required but note that it is possible that a sexually transmitted disease case investigation and contact tracing tool could incorporate features from more complex models while maintaining a user-friendly interface. Conclusions: A sexually transmitted disease case investigation and contact tracing tool could benefit from the incorporation of key features of the COVID-19 model, namely its user-friendly interface. The inherent differences between sexually transmitted diseases and respiratory viruses should not be seen as a limitation to the development of such tool. © 2023 The Authors

Challenges in the control of COVID-19 outbreaks caused by the delta variant during periods of low humidity:an observational study in Sydney,Australia

Background: Since the appearance of severe acute respiratory coronavirus 2 (SARS-CoV-2) and the coronavirus disease 2019 (COVID-19) pandemic, a growing body of evidence has suggested that weather factors, particularly temperature and humidity, influence transmission. This relationship might differ for the recently emerged B.1.617.2 (delta) variant of SARS-CoV-2. Here we use data from an outbreak in Sydney, Australia that commenced in winter and time-series analysis to investigate the association between reported cases and temperature and relative ***: Between 16 June and 10 September 2021, the peak of the outbreak, there were 31,662 locally-acquired cases reported in five local health districts of Sydney, Australia. The associations between daily 9:00 am and 3:00 pm temperature (°C), relative humidity (%) and their difference, and a time series of reported daily cases were assessed using univariable and multivariable generalized additive models and a 14-day exponential moving average. Akaike information criterion (AIC) and the likelihood ratio statistic were used to compare different models and determine the best fitting model. A sensitivity analysis was performed by modifying the exponential moving ***: During the 87-day time-series, relative humidity ranged widely (< 30-98%) and temperatures were mild (approximately 11-17℃). The best-fitting (AIC: 1,119.64) generalized additive model included 14-day exponential moving averages of 9:00 am temperature (P < 0.001) and 9:00 am relative humidity (P < 0.001), and the interaction between these two weather variables (P < 0.001). Humidity was negatively associated with cases no matter whether temperature was high or low. The effect of lower relative humidity on increased cases was more pronounced below relative humidity of about 70%;below this threshold, not only were the effects of humidity pronounced but also the relationship between temperature and cases of the delta variant becomes ***

Development and Spatial External Validation of a Predictive Model of Survival Based on Random Survival Forest Analysis for People Living With hiv and aids After Highly Active Antiretroviral Therapy in China: Retrospective Cohort Study

BACKGROUND:hiv infection remains a global public health challenge, with an estimated 42.3 million cumulative deaths to date. Given the heterogeneity among people living with hiv and aids, there is a critical need to develop robust prognostic models to predict survival and guide individualized clinical management. OBJECTIVE:We aimed to develop and externally validate a predictive model for the survival of people living with hiv and aids following the initiation of highly active antiretroviral therapy (HAART) in China. METHODS:We used data from the hiv and aids epidemic surveillance system of the National Center for aids/std control and prevention, Chinese Center for Disease control and prevention, for this retrospective cohort study. The training set and the external validation set included people living with hiv and aids from the cities of Nanjing and Nantong, respectively. The prediction model was developed by using the random survival forest (RSF), and its performance was evaluated against the Cox model, integrated area under the curve (iAUC), consistency index (C index), calibration curves, integrated Brier score (iBS), and decision curve analysis. RESULTS:A total of 8960 patients were eligible for this study, consisting of 5261 (58.71%) cases in the training set (mean age 32.39, SD 13.30 years; n=4891, 92.97% male patients) and 3699 (41.28%) cases in the external validation set (mean age 43.31, SD 14.18 years; n=3086, 83.42% male patients). The RSF model was developed based on the top 7 variables ranked by variable importance, including hemoglobin, age at HAART treatment, infection route, white blood cell count, education level, blood glucose, and the CD4 count before HAART. The RSF model exhibited good performance, with an iBS of 0.129 in the internal validation set and 0.113 in the external validation set, and a C index of 0.896 (95% CI 0.885-0.906) in the internal validation set and 0.756 (95% CI 0.730-0.782) in the external validation set, respectively.
