Background:Low-fee female sex workers(FSWs)are at high risk of acquiring and spreading human immunodeficiency virus(hiv)/sexually transmitted diseases(stds)in *** is an urgent need to develop comprehensive intervention measures targeted towards low-fee FSWs to reduce hiv/std ***,this study aimed to reduce hiv/std infections among low-fee FSW through a matched-pair,comiDunity-based randomized intervention trial carried out in 12 cities in three provinces in ***:Four cities from Guangxi Zhuang Autonomous Region,four from Yunnan Province,and four from Hunan Province were paired and participants received either the intervention package(including condom promotion,hiv and syphilis testing,reimbursement for syphilis treatment costs,and free anti-retroviral therapy or the current standard of ***-based,convenience sampling was used to recruit FSWs.A face-to-face interview and hiv and syphilis blood testing was conducted at baseline and follow-up intervals of 24 *** linear mixed models(GLMM)were used to evaluate the effect of the intervention package on reducing hiv/std infection in the ***:A total of 1024 eligible FSWs were enrolled in the baseline survey and 843 in the *** results showed that syphilis infeaion was reduced by 49%in the intervention group compared to the current standard of care group(P=0.0378,OR=0.51;95%CI:0.27-0.96).FSWs aged 35 years or older were 2.38 times more likely to get syphilis infection compared to those younger than 35 years old(P<0.0001,OR=2.38,95%CI:1.55-3.65).The risk of syphilis infection among more educated FSWs was 0.43 times less than those with lower levels of education(P<0.05,OR=0.43,95%CI:0.63-0.93).Conclusions:This study demonstrates that comprehensive interventions can lead to significant declines in syphilis infection amongst low-tier *** both behavioral and biomedical intervention measures should be considered when developing programs for low-fee FS

Background: The number of cases of gonorrhoea in the USA and worldwide caused by Neisseria gonorrhoeae is increasing (555 608 reported US cases in 2017, and 87 million cases worldwide in 2016). Many countries report declining in vitro susceptibility of azithromycin, which is a concern because azithromycin and ceftriaxone are the recommended dual treatment in many countries. We aimed to identify strain types associated with decreased susceptibility to azithromycin. Methods: We did a genomic analysis of N gonorrhoeae isolates obtained by the US Gonococcal Isolate Surveillance Project. Isolates were whole-genome sequenced based on decreased susceptibility to azithromycin (minimal inhibitory concentration [MIC] ≥2 μg/mL, using agar dilution antibiotic susceptibility testing) and geographical representation. Bioinformatic analyses established genomic diversity, strain population dynamics, and antimicrobial resistance profiles. Findings: 410 isolates were sorted into more than 20 unique phylogenetic clades. One predominant persistent clade (consisting of 97 isolates) included the most isolates with azithromycin MICs of 2 μg/mL or higher (61 of 97 [63%] vs 59 of 311 [19%];p<0·0001) and carried a mosaic mtr (multiple transferable resistance) locus (68 of 97 [70%] vs two of 313 [1%];p<0·0001). Of the remaining 313 isolates, 57 (18%) had decreased susceptibility to azithromycin (MIC ≥4 μg/mL), which was attributed to 23S rRNA variants (56 of 57 [98%]) and formed phylogenetically diverse clades, showing various levels of clonal expansion. Interpretation: Reduced azithromycin susceptibility was associated with expanding and persistent clades harbouring two well described resistance mechanisms, mosaic mtr locus and 23S rRNA variants. Understanding the role of recombination, particularly within the mtr locus, on the fitness and expansion of strains with decreased susceptibility has important implications for the public health response to minimise gonorrhoea transmission. Background: Despite anxiety disorders being a significant public health concern, studies assessing their global burden among older adults are limited. We aimed to analyse trends in the global burden of anxiety disorders in older adults aged ≥ 55 years. Methods: Data on the incidence and disability-adjusted life-years (DALYs) for anxiety disorders were retrieved from the Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) 2021. Temporal trends were quantified using estimated annual percentage changes. Results: In 2021, global DALYs for anxiety disorders were estimated at 8.21 million (95 % UI 5.62–11.48 million), nearly 2.5 times higher than in 1990, with an age-standardised rate of 552.7 DALYs (95 % UI 378.1–772.5) per 100,000 population. Between 1990 and 2021, the age-standardised DALY rate (ASDR) remained stable (−0.05 % [95 % CI −0.16–0.05]), with minimal differences between men (0.04 % [−0.05–0.12]) and women (−0.08 % [−0.21–0.05]). The 55–59 age group showed the highest ASDR (538.5 [361.3–767.7] per 100,000 population). The highest increases in ASDR were observed in the low-middle (0.15 % [0.03–0.28]) and low sociodemographic index regions (0.09 % [0.02–0.15]). The regions with the highest ASDRs were Latin America, Western Europe, and High-income North America. Conclusion: While global trends in anxiety disorders among older adults remained stable, the total burden increased significantly, with marked regional disparities. Tailored mental health interventions, early detection, and treatment in primary care are essential to reduce the future burden of anxiety disorders in older adults, especially in low and middle-income countries. © 2025 Elsevier Ltd

Utilization pattern of other preventive services during the US Medicare annual wellness visit

Annual wellness visit (AWV) was introduced for Medicare patients in 2011 to help patients stay healthy. The object of this study is to assess whether AWV have an impact on the use of other preventive services in the eligible population. Medicare claims for the full sample of beneficiaries who were continuously enrolled in fee-for-service Medicare in 2013 and 2014 were analyzed. The association between AWV and three other preventive services (depression screening [DPS], influenza virus vaccine [IVV], and sexually transmitted infection screening [STI]) were assessed. In addition, the utilization pattern of these three preventative services at AWV visit by the calendar month when beneficiaries had an AWV service was also assessed. Of 28 million eligible Medicare beneficiaries, 16.0% had AWV in 2014. The patients who had AWV had a significantly higher percentage of three preventive services than those who had no AWV: 63.8% vs. 41.6% in IVV, 4.9% vs. 0.5% in DPS, and 2.3% vs. 1.8% in STI. The percentages of beneficiaries who received IVV during an AWV visit varied significantly by calendar month: from < 0.1% in June to 36.8% in October. AWV is associated with increased use of other preventive services. In addition, the association is significantly affected by type of other preventive services that may be highly related with seasonal factors. © 2017

Professional Development on Sexual Health Education Is Associated With Coverage of Sexual Health Topics

Background. Sexual health education plays a vital role in preventing adverse consequences of sexual behavior. Data are limited on how professional development (PD) for teachers is associated with teaching sexual health content. Method. We used data from the 2014 School Health Policies and Practices Study, a cross-sectional survey that included a nationally representative sample of middle and high school health education courses (n = 328;96.2% of those sampled). We examined whether teacher characteristics and receipt of PD on four sexual health domains (human sexuality and the prevention of pregnancy, hiv, and sexually transmitted diseases) were associated with teaching those four domains and whether PD was associated with teaching specific topics within each domain. In a second set of analyses, we compared the difference in coverage of specific topics by receipt of PD among only the teachers who reported teaching the domain. Results. The prevalence of teaching sexual health content was higher among courses in which teachers had a degree in health education or were certified to teach health education. A robust relationship between teachers’ receipt of PD and the teaching of nearly all sexual health topics as well as an increase in the mean number of topics taught and the time devoted to the domain was demonstrated. Conclusions. PD has a positive impact on the coverage of sexual health topics among middle and high school courses. These data suggest a need for more PD on sexual health education content to ensure that sexual health topics are taught in the classroom. © 2017 Society for Public Health Education.
