Trends in care and treatment for persons aged ≥13 years with hiv infection 17 U.S. jurisdictions, 2012-2015

Background: Care and viral suppression national goals for hiv infection are not being met for many at-risk groups. Assessment of the trends in national outcomes for linkage to care, receipt of care, and viral suppression among these groups is necessary to reduce transmission. Methods: Data reported to the National hiv Surveillance System by December 2016 were used to identify cases of hiv infection among persons aged 13 years and older in one of 17 identified jurisdictions with complete laboratory reporting. We estimated national trends in hiv-related linkage to care, receipt of care and viral suppression using estimated annual percent change from 2012-2015 for various characteristics of interest, overall and stratified by sex and race/ethnicity. Results: Overall, trends in linkage to and receipt of care and viral suppression increased from 2012-2015. Generally, linkage to and receipt of care increased among young black and Hispanic/Latino males, those with infection attributed to male-to-male sexual contact, and those not in stage 3 [aids] at hiv diagnosis. All sub-groups showed improvement in viral suppression. Within years, there remains a substantial disparity in receipt of care and viral suppression among racial/ethnic groups. Conclusion: While trends are encouraging, scientifically proven prevention programs targeted to high-risk populations are the foundation for stopping transmission of hiv infection. Frequent testing to support early diagnosis and prompt linkage to medical care, particularly among young men who have male to male sexual contact, black and Hispanic/Latino populations, are key to reducing transmission at all stages of disease. © 2018 Karch et al.

Developing a Motion Comic for hiv/std prevention for Young People Ages 15-24, Part 2: Evaluation of a Pilot Intervention

In the United States, young people (ages 15-24 years) are disproportionately affected by human immunodeficiency virus (hiv) and other sexually transmitted diseases (stds), due at least in part to inadequate or incorrect hiv/std-related knowledge, attitudes, beliefs, and behavioral intentions (KABI). Comic book narratives are a proven method of hiv/std prevention communication to strengthen KABI for hiv/std prevention. Motion comics, a new type of comic media, are an engaging and low-cost means of narrative storytelling. The objective of this study was to quantitatively evaluate the effectiveness of a pilot six-episode hiv/std-focused motion comic series to improve hiv/std-related KABI among young people. We assessed change in hiv/std knowledge, hiv stigma, condom attitudes, hiv/std testing attitudes, and behavioral intentions among 138 participants in 15 focus groups immediately before and after viewing the motion comic series. We used paired t-tests and indicators of overall improvement to assess differences between surveys. We found a significant decrease in hiv stigma (p < .001) and increases in both hiv knowledge (p = .002) and behavioral intentions to engage in safe sex (p < .001). In summary, this motion comic intervention improved hiv/std-related KABI of young adult viewers by reducing hiv stigma and increasing behavioral intentions to engage in safer sex. Our results demonstrate the promise of this novel intervention and support its use to deliver health messages to young people.

Oral pre-exposure prophylaxis (PrEP) effectively prevents hiv among men who have sex with men, but its adherence faces significant hindrances. We evaluated the effectiveness of real-time digital intervention in promoting oral PrEP adherence through a randomized controlled trial using electronic medication monitors. The trial was registered on the Chinese Clinical Trial Registry (ChiCTR1900025604) and randomized 442 MSM to intervention (247) or control group (195). At the 6-month follow-up, the intervention group exhibited significantly higher oral PrEP adherence than the control (83.1% vs. 59.8%; adjusted net difference: 21.0%, 95% confidence interval: 12.9-29.2%, p < 0.001), while no differences were detected in the number of male sexual partners, condomless anal intercourse prevalence, or substance use disorder, with consistent results across both daily and event-driven oral PrEP regimens. Therefore, digital intervention significantly increased oral PrEP adherence over 6 months in the daily and event-driven subgroups but demonstrated no effect on high-risk behaviors.

Objectives This study aims to assess the cumulative incidence and rate of loss to follow-up (LTFU) among people living with hiv (PLhiv) in Nepal who begin antiretroviral therapy (ART) early, as well as to identify factors associated with LTFU in the context of the universal test and treat approach. Study design Retrospective cohort study Methods We retrospectively analyzed nationally representative routine programme data for all PLhiv initiated on ART from February 19, 2017, to February 18, 2020, and followed up until May 10, 2022. LTFU was defined as a client not returning to the hiv clinic for at least 3 months from the date of their last scheduled appointment. We calculated cumulative incidence rate (IR) and fitted a multivariable Cox proportional hazards regression model to determine factors associated with LTFU. Results Of the 8,192 clients included in our sample, 6,797 (82.9%) started ART within seven days following their hiv diagnosis. The overall IR of LTFU was 4.22 (95% CI =3.95-4.51) per 100 person years of observation. The cumulative incidence of LTFU increased over time on ART, from 3.81% (95% CI =3.40-4.26) at 6 months, 6.51% (95% CI =5.97-7.09) at 12 months to 13.41% (95% CI = 12.51-14.37) at 48 months. In the multivariable model, factors associated with higher odds of LTFU were starting treatment within 7 days, younger age, being unmarried, belonging to Dalit ethnic group, WHO stage 1 at baseline, and being started on an NNRTI regimen. Among key populations, sex workers, their clients, MSM and transgender (TG), PWID were at higher risk of dropout compared to migrants. Conclusions In this nationwide cohort, the risk of LTFU increased with time on ART. To optimize the test-and-treat strategy in Nepal, it is crucial to address the unique needs of youth and certain key populations and manage any early adverse drug reactions.
