Objectives Federal open data initiatives that promote increased sharing of federally collected data are important for transparency, data quality, trust, and relationships with the public and state, tribal, local, and territorial (STLT) partners. These initiatives advance understanding of health conditions and diseases by providing data to more researchers, scientists, and policymakers for analysis, collaboration, and valuable use outside CDC responders. This is particularly true for emerging conditions such as COVID-19 where we have much to learn and have evolving data needs. Since the beginning of the outbreak, CDC has collected person-level, de-identified data from jurisdictions and currently has over 8 million records, increasing each day. This paper describes how CDC designed and produces two de-identified public datasets from these collected data. Materials and Methods Data elements were included based on the usefulness, public request, and privacy implications;specific field values were suppressed to reduce risk of reidentification and exposure of confidential information. Datasets were created and verified for privacy and confidentiality using data management platform analytic tools as well as R scripts. Results Unrestricted data are available to the public through *** and restricted data, with additional fields, are available with a data use agreement through a private repository on ***. Practice Implications Enriched understanding of the available public data, the methods used to create these data, and the algorithms used to protect privacy of de-identified individuals allow for improved data use. In the United States, hepatitis C virus (HCV) transmission is rising among people who inject drugs (PWID). Many regions have insufficient prevention intervention coverage. Using modeling, we investigated the impact of scaling up prevention and treatment interventions on HCV transmission among PWID in Perry County, Kentucky, and San Francisco, California, where HCV seroprevalence among PWID is >50%. A greater proportion of PWID access medication-assisted treatment (MAT) or syringe service programs (SSP) in urban San Francisco (established community) than in rural Perry County (young, expanding community). We modeled the proportion of HCV-infected PWID needing HCV treatment annually to reduce HCV incidence by 90% by 2030, with and without MAT scale-up (50% coverage, both settings) and SSP scale-up (Perry County only) from 2017. With current MAT and SSP coverage during 2017–2030, HCV incidence would increase in Perry County (from 21.3 to 22.6 per 100 person-years) and decrease in San Francisco (from 12.9 to 11.9 per 100 person-years). With concurrent MAT and SSP scale-up, 5% per year of HCV-infected PWID would need HCV treatment in Perry County to achieve incidence targets—13% per year without MAT and SSP scale-up. In San Francisco, a similar proportion would need HCV treatment (10% per year) irrespective of MAT scale-up. Reaching the same impact by 2025 would require increases in treatment rates of 45%–82%. Achievable provision of HCV treatment, alongside MAT and SSP scale-up (Perry County) and MAT scale-up (San Francisco), could reduce HCV incidence.

To evaluate the hiv pandemic in Chongqing, the pooled PCR, IgG-capture BED enzyme immunoassay （BED-CEIA）, and cohort observations were used to estimate the hiv incidences among men who have sex with men （MSM）. 617 MSM subjects completed the survey at a voluntary counseling and testing （VCT） site. The observed hiv incidence was 12.5 per 100 P-Ys （95% CI = 9.1-15.7）.

Background: Ensuring that individuals who are living with hiv rapidly initiate antiretroviral therapy (ART) is an essential step in meeting the 90-90-90 targets. We evaluated the feasibility and outcomes of rapid ART initiation in the Botswana Combination prevention Project (BCPP). We aimed to establish whether simplified ART initiation with the offer of same-day treatment could increase uptake and reduce time from clinic linkage to treatment initiation, while maintaining rates of retention in care and viral suppression. Methods: We did a quasi-experimental before and after study with use of data from the BCPP. The BCPP was a community-randomised hiv-prevention trial done in 30 communities across Botswana from Oct 1, 2013, to June 30, 2018. Participants in the 15 intervention clusters, who were hiv-positive and not already taking ART were offered universal hiv-treatment and same-day ART with a dolutegravir-based regimen at first clinic visit. This rapid ART intervention was implemented mid-way through the trial on June 1, 2016, enabling us to determine the effect of rapid ART guidelines on time to ART initiation and rates of retention in care and viral suppression at 1 year in the BCPP intervention group. Findings: We assessed 1717 adults linked to study clinics before rapid ART introduction and 800 after rapid ART introduction. During the rapid ART period, 457 (57·1%, 95% CI 53·7–60·6) individuals initiated ART within 1 day of linkage, 589 (73·7%, 70·6–76·7) of 799 within 1 week, 678 (84·9%, 82·4–87·3) of 799 within 1 month, and 744 (93·5%, 91·6–95·1) of 796 within 1 year. Before the introduction of rapid ART, 163 (9·5%, 95% CI 8·2–11·0) individuals initiated ART within 1 day of linkage, 276 (16·1%, 14·4–17·9) within 1 week, 839 (48·9%, 46·5–51·3) within 1 month, and 1532 (89·2%, 87·7–90·6) within 1 year. 1 year after ART initiation, 1472 (90·5%, 87·4–92·8) of 1627 individuals who linked in the standard ART period were in care and had a viral load of less than 400

Background: Patient costs pose a challenge in accessing antiretroviral therapy for people living with hiv in sub-Saharan Africa. The study aimed at identifying drivers for out-of-pocket (OOP) costs in Tanzania. Methods: In 2009, 500 adult patients who attended 10 hiv clinics across 7 regions of Tanzania were asked about time and resources consumed to access hiv services. Bivariate and multivariate median regression models were used to determine the main drivers for OOP costs. Results: Male and female patients have a median OOP costs of $40.37 and $28.01 per year, respectively (P =.01). Males spend significantly more on travel ($26.51) than females ($19.68;P =.02). Living in rural areas and poor social economic status (SES) are associated with greater OOP costs (P =.001) for both sexes. Conclusion: Poor SES and rural residence are main drivers of OOP costs. Patients are less likely to seek health care unless they are in dire need, leading to expensive services. © The Author(s) 2018.
