There are many reasons to reflect upon the importance of paradigms in healthcare, their explicit use in health system restructuring and the possibility to anticipate the health field evolution with paradigms. For instance, it is known that an improving or restructuring strategy is needed to adapt health systems to major changes in the way of thinking or doing that occurred in the recent years in the health sector and the society. Such changes are often characterized as paradigm shifts where a paradigm is intended as a reference model of fundamental value widely accepted in a particular sector of activity. The paper claims that an explicit use of paradigms can speed up the restructuring process in healthcare. Some paradigms recently transferred from business modelling and engineering to healthcare could be more helpful if explicitly used in health system restructuring. Two recent paradigms in software engineering that will be found particularly useful to health informatics are also introduced.

The use of glide symmetry in radiofrequency devices to introduce dispersive effects has been recently proposed and demonstrated. One of these effects is to control the propagation constant of the structure. Here, we propose a mm-wave phase shifter whose elements have a glide-symmetric configuration to achieve a greater phase shift in the same waveguide space than the non-glide-symmetric case. The glide-symmetric phase shifter is implemented in waveguide technology and is formed by rows of metallic pins that produce the desired phase shift. To assess the better performance of the glide-symmetric phase shifter, it is compared to its non-glide-symmetric version whose metallic pins are located only in one of the broad sides of the waveguide. The operating frequency range of the phase shifter is 67 to 75 GHz. Results show a 180 degree phase shift in regard to the reference waveguide without pins, and 50 degrees more than the non-glide-symmetric version.

Several profilers for general-purpose languages like Java offer visualizations to support users in understanding the execution of a program and identifying the causes of a performance issue. Unfortunately, these performance visualizations are difficult to reuse in profilers for declarative model transformations since they cannot display transformation-specific information. For example, a profiler for a declarative model transformation language must provide information on the traversal of the input model since it impacts the performance but is hidden from the developer. Moreover, the respective visualization must scale for input models that consist of several thousand model elements. Hence, we developed performance visualizations for the declarative model transformation language Henshin that provide insights into the transformation execution. Subsequently, we performed a mixed methods study with 18 Henshin novices to evaluate the effectiveness of our visualizations. In our study, the participants tried to improve the execution performance of four transformations by performing a root cause analysis using our visualizations. The results of our study show that depending on the task, between 16 and 18 participants understood the execution of a transformation correctly based on our visualizations. Moreover, between 12 and 18 participants proposed effective optimizations using our visualizations.
