Trends in care and treatment for persons aged ≥13 years with HIV infection 17 U.S. jurisdictions, 2012-2015

Background: Care and viral suppression national goals for HIV infection are not being met for many at-risk groups. Assessment of the trends in national outcomes for linkage to care, receipt of care, and viral suppression among these groups is necessary to reduce transmission. Methods: Data reported to the National HIV Surveillance System by December 2016 were used to identify cases of HIV infection among persons aged 13 years and older in one of 17 identified jurisdictions with complete laboratory reporting. We estimated national trends in HIV-related linkage to care, receipt of care and viral suppression using estimated annual percent change from 2012-2015 for various characteristics of interest, overall and stratified by sex and race/ethnicity. Results: Overall, trends in linkage to and receipt of care and viral suppression increased from 2012-2015. Generally, linkage to and receipt of care increased among young black and Hispanic/Latino males, those with infection attributed to male-to-male sexual contact, and those not in stage 3 [aids] at HIV diagnosis. All sub-groups showed improvement in viral suppression. Within years, there remains a substantial disparity in receipt of care and viral suppression among racial/ethnic groups. Conclusion: While trends are encouraging, scientifically proven prevention programs targeted to high-risk populations are the foundation for stopping transmission of HIV infection. Frequent testing to support early diagnosis and prompt linkage to medical care, particularly among young men who have male to male sexual contact, black and Hispanic/Latino populations, are key to reducing transmission at all stages of disease. © 2018 Karch et al.

Background:Men who have sex with men(MSM)has become the group with the fastest growing HIV epidemic in *** many Chinese MSM are conducting HIV self-testing,we aimed to determine the rate of HIV care seeking after self-testing,examine characteristics of“seekers”compared to“non-seekers,”and explore factors associated with HIV care-seeking ***:A cross-sectional study design was used and an online survey was conducted in Beijing,China in 2016,among users of a popular Chinese gay networking smart phone ***-square test was used to compare characteristics of those who sought HIV care(“seekers”)and those who did not(“non-seekers”).Univariate and multivariate logistic regression analyses were conducted to assess factors associated with HIV care ***:Among 21,785 screened,2383 participants(10.9%)were included in the study.A total of 380 participants(15.9%)reported seeking HIV care after HIV self-testing while 2003(84.1%)did *** of knowledge of the“window period”(adjusted odds ratio[AOR]=0.68,95%confidence interval[95%CI]=0.47-0.97,P=0.04)was associated with reduced odds of seeking HIV care,while lower monthly income(AOR=1.29,95%CI=1.03-1.62,P=0.03)and obtaining HIV self-testing kits from health facilities(AOR=2.40,95%CI=1.81-3.17,P<0.001),and non-governmental organizations(AOR=2.44,95%CI=1.79-3.34,P<0.001)was associated with increased odds of seeking HIV *** those who sought HIV care,a large majority(92.4%)had non-reactive HIV self-testing *** 29 out of 265 with reactive,uncertain,or unknown results sought HIV ***:We found a very low rate of HIV care seeking among our sample of urban Chinese *** observation that most with reactive,uncertain,or unknown results did not seek HIV care is a cause for *** people should be paid more attention and helped to enter the care *** findings highlight that interventions aimed at improving linkage to care after HIV self-testing are urgently n

Drivers of Patient Costs in Accessing HIV/aids Services in Tanzania

Background: Patient costs pose a challenge in accessing antiretroviral therapy for people living with HIV in sub-Saharan Africa. The study aimed at identifying drivers for out-of-pocket (OOP) costs in Tanzania. Methods: In 2009, 500 adult patients who attended 10 HIV clinics across 7 regions of Tanzania were asked about time and resources consumed to access HIV services. Bivariate and multivariate median regression models were used to determine the main drivers for OOP costs. Results: Male and female patients have a median OOP costs of $40.37 and $28.01 per year, respectively (P =.01). Males spend significantly more on travel ($26.51) than females ($19.68;P =.02). Living in rural areas and poor social economic status (SES) are associated with greater OOP costs (P =.001) for both sexes. Conclusion: Poor SES and rural residence are main drivers of OOP costs. Patients are less likely to seek health care unless they are in dire need, leading to expensive services. © The Author(s) 2018.

Hepatitis C: Review of the Epidemiology, Clinical Care, and Continued Challenges in the Direct Acting Antiviral Era

PURPOSE OF REVIEW:This review highlights key studies and recently published data, policies, and recommendations related to hepatitis C virus (HCV) epidemiology, transmission, and treatment. RECENT FINDINGS:HCV is a leading cause of liver-related deaths, cirrhosis, and hepatocellular carcinoma. Since 2011 and accelerating since 2013, new, safe, tolerable, and curative therapies have considerably altered clinical and public health frameworks related to the prevention, control and clinical management of HCV. Nevertheless, there are several populations in the United States that are important to consider because of disparities in HCV prevalence and transmission risk. Adults born during 1945-1965 have an estimated anti-HCV antibody prevalence of ~3%, which is six times higher than among other adults, are often unaware of their infections, and are at increased risk of having HCV-associated morbidity and mortality from decades of chronic infection. Since the early 2000s, increasing incidence of acute HCV infections among young, white, non-urban people who inject drugs have been reported. Despite promising therapeutic advances, significant challenges remain for reducing HCV-associated morbidity and mortality. SUMMARY:The high burden of HCV and significant health consequences associated with chronic infection make HCV a critical public health priority. Advances in HCV treatment have created new opportunities for reducing HCV-associated morbidity and mortality. These treatments are safe, well-tolerated, and highly effective; however, benefits cannot be realized without a significant increase in the number of persons tested for HCV so that all chronically infected individuals can be aware of their diagnosis and linked to appropriate clinical care.

HIV care and viral suppression during the last year of life: A comparison of HIV-infected persons who died of HIV-attributable causes with persons who died of other causes in 2012 in 13 US jurisdictions

Background: Little information is available about care before death among human immunodeficiency virus (HIV)-infected persons who die of HIV infection, compared with those who die of other causes. Objective: The objective of our study was to compare HIV care and outcome before death among persons with HIV who died of HIV-attributable versus other causes. Methods: We used National HIV Surveillance System data on CD4 T-lymphocyte counts and viral loads within 12 months before death in 2012, as well as on underlying cause of death. Deaths were classified as “HIV-attributable” if the reported underlying cause was HIV infection, an aids-defining disease, or immunodeficiency and as attributable to “other causes” if the cause was anything else. Persons were classified as “in continuous care” if they had ≥2 CD4 or viral load test results ≥3 months apart in those 12 months and as having “viral suppression” if their last viral load was <200 copies/mL. Results: Among persons dying of HIV-attributable or other causes, respectively, 65.28% (2104/3223) and 30.88% (1041/3371) met aids criteria within 12 months before death, and 33.76% (1088/3223) and 50.96% (1718/3371) had viral suppression. The percentage of persons who received ≥2 tests ≥3 months apart did not differ by cause of death. Prevalence of viral suppression for persons who ever had aids was lower among those who died of HIV but did not differ by cause for those who never had aids. Conclusions: The lower prevalence of viral suppression among persons who died of HIV than among those who died of other causes implies a need to improve viral suppression strategies to reduce mortality due to HIV infection. © William K Adih, H Irene Hall, Richard M Selik, Xiuchan Guo. Associations and trends in cause-specific rates of death among persons reported with HIV infection, 23 U.S. jurisdictions, through 2011

Background: Published death rates for persons with HIV have not distinguished deaths due to HIV from deaths due to other causes. Cause-specific death rates would allow better assessment of care needs. Methods: Using data reported to the US national HIV surveillance system, we examined a) associations between selected decedent characteristics and causes of death during 2007-2011, b) trends in rates of death due to underlying causes among persons with aids during 1990-2011, and among all persons with diagnosed HIV infection (with or without aids) during 2000-2011. Results: During 2007-2011, non-HIV-attributable causes of death with the highest rates per 1,000 person-years were heart disease (2.0), non- aids cancers other than lung cancer (1.4), and accidents (0.8). During 1990-2011, among persons with aids, the annual rate of death due to HIV-attributable causes decreased by 89% (from 122.0 to 13.2), and the rate due to non-HIV-attributable-causes decreased by 57% (from 20.0 to 8.6), while the percentage of deaths caused by non-HIV-attributable causes increased from 11% to 43%. During 2000-2011, among persons with HIV infection, the rate of death due to HIV-attributable causes decreased by 69% (from 26.4 to 8.3), and the rate due to non-HIV-attributable causes decreased by 28% (from 10.5 to 7.6), while the percentage of deaths caused by non-HIV-attributable causes increased from 25% to 48%. Conclusion: Among HIV-infected persons, as rates of death due to HIV-attributable causes decreased, rates due to non-HIV-attributable causes also decreased, but the percentages of deaths due to non-HIV-attributable causes, such as heart disease and non-aids cancers increased. © Adih et al.;Licensee Bentham Open.
