Economic glocalization has been developed in the local food production system in Wonosobo. Glocalization is indicated by the existence of sustainability in the food production system and local empowerment. The aim of this study was to describe economic glocalization in the local food production system (carica and purwaceng) in Wonosobo. The qualitative method used in this study was based on the paradigm of constructivism with a phenomenological approach including knowledge, attitude and behavior. The result of this study showed that local products of carica (Carica candamsrcensis) and purwaceng (Pimpinella pruatjan) were able to compete with global product capitalism. The impact of the glocalization of carica and purwaceng was to develop local economic institutions such as productive economic enterprises and cooperatives. In addition, economic glocalization in the local food production system in Wonosobo had also the effect of the increase production capacity, product development, market expansion, local economy and eco-tourism.

Obstetrician's attitudes about delivery through cesarean section: A study in hospitals at Mexico City

Objective To identify the attitudes of medical obstetrician-gynecologists (Ob-Gyn) and its association with the cesarean section rate. Material and methods We performed a cross-sectional multicenter survey research, 197 Ob-Gyn were surveyed from eight hospitals, between November 2010 and May 2011. Data analysis included descriptive statistics on the general characteristics of Ob-Gyn, We used the χ 2 test for bivariate analyses of categorical variables and logistic regression models to associate Ob-Gyn attitudes and percentage of births by cesarean section. Results The percentage of cesarean sections births expressed by Ob-Gyn surveyed was 59.2%. Ob-Gyn expressed a preference to delivery by cesarean section in 33.5%, 60.9% of the Ob-Gyn considered themselves skillful when attending cesarean deliveries compared against vaginal delivery. Thirty five percent of Ob-Gyn has scheduled a cesarean section for convenience, while 83.8% of Ob-Gyn said that women prefer cesarean births. In the regression model five variables are significantly associated with the Ob-Gyn that perform 30% or more of their cesarean deliveries, among these include: perception that vaginal are safer procedures than cesarean deliveries and that women have right to choose the type of delivery, whether vaginal or cesarean, with an OR = 4.7 and 7.5 respectively. Conclusions We have shown that attitudes of Ob-Gyn who are associated with cesarean section rate. These attitudes could be related with the increase of the cesarean births. Background: Experimental and epidemiological studies indicate an association between exposure to particulate matter (PM) air pollution and increased risk of type 2 diabetes. In view of the high and increasing prevalence of diabetes, we aimed to quantify the burden of type 2 diabetes attributable to PM_2·5 originating from ambient and household air pollution. Methods: We systematically compiled all relevant cohort and case-control studies assessing the effect of exposure to household and ambient fine particulate matter (PM_2·5) air pollution on type 2 diabetes incidence and mortality. We derived an exposure–response curve from the extracted relative risk estimates using the MR-BRT (meta-regression—Bayesian, regularised, trimmed) tool. The estimated curve was linked to ambient and household PM_2·5 exposures from the Global Burden of Diseases, Injuries, and Risk Factors Study 2019, and estimates of the attributable burden (population attributable fractions and rates per 100 000 population of deaths and disability-adjusted life-years) for 204 countries from 1990 to 2019 were calculated. We also assessed the role of changes in exposure, population size, age, and type 2 diabetes incidence in the observed trend in PM_2·5-attributable type 2 diabetes burden. All estimates are presented with 95% uncertainty intervals. Findings: In 2019, approximately a fifth of the global burden of type 2 diabetes was attributable to PM_2·5 exposure, with an estimated 3·78 (95% uncertainty interval 2·68–4·83) deaths per 100 000 population and 167 (117–223) disability-adjusted life-years (DALYs) per 100 000 population. Approximately 13·4% (9·49–17·5) of deaths and 13·6% (9·73–17·9) of DALYs due to type 2 diabetes were contributed by ambient PM2·5, and 6·50% (4·22–9·53) of deaths and 5·92% (3·81–8·64) of DALYs by household air pollution. The implication of climate adaptation and mitigation research: capacity adaptation of rice paddy farmers to climate change

Climate change requires adjustment, resilience, anticipation, coping with stress or changes to sustainable agricultural development in socioeconomic and biophysical systems. Climate change affects the plants, livestock and also the environment, thus a reactive adaptation by paddy farmers is necessary. A purposive sampling on 30 respondents who had accomplished the Climate field school (CFS) program in 'Mandiri' and group of women farmers 'Dalima'. The study was conducted from October to November 2017 in Situ Gede sub-district, Bogor, Indonesia. This study used the descriptive analysis with frequency distribution table to describe the respondent distribution of each variable. The research objective is to describe the ability of farmers to adapt to climate change and to realize the sustainability of rice farming. The results obtained are the application of adaptation ability of paddy farmers to changes in rainfall patterns in the form of technical capabilities (knowledge), institutions, social capital, access to business capital is categorized medium, while the technical ability of adaptation (action) is high. Furthermore, the ability of farmers to adapt to the sustainability of socio-cultural lowland rice farming is high in assessing persistence and togetherness are important factors in farming success. The implication is that the technical adaptability of rice farmers will be high if the socio-cultural values and cooperation of rice farmers are maintained as in planning the timing of planting, harvesting and marketing.

Background: In an era of shifting global agendas and expanded emphasis on non-communicable diseases and injuries along with communicable diseases, sound evidence on trends by cause at the national level is essential. The Global Burden of Diseases, Injuries, and Risk Factors Study (GBD) provides a systematic scientific assessment of published, publicly available, and contributed data on incidence, prevalence, and mortality for a mutually exclusive and collectively exhaustive list of diseases and injuries. Methods: GBD estimates incidence, prevalence, mortality, years of life lost (YLLs), years lived with disability (YLDs), and disability-adjusted life-years (DALYs) due to 369 diseases and injuries, for two sexes, and for 204 countries and territories. Input data were extracted from censuses, household surveys, civil registration and vital statistics, disease registries, health service use, air pollution monitors, satellite imaging, disease notifications, and other sources. Cause-specific death rates and cause fractions were calculated using the Cause of Death Ensemble model and spatiotemporal Gaussian process regression. Cause-specific deaths were adjusted to match the total all-cause deaths calculated as part of the GBD population, fertility, and mortality estimates. Deaths were multiplied by standard life expectancy at each age to calculate YLLs. A Bayesian meta-regression modelling tool, DisMod-MR 2.1, was used to ensure consistency between incidence, prevalence, remission, excess mortality, and cause-specific mortality for most causes. Prevalence estimates were multiplied by disability weights for mutually exclusive sequelae of diseases and injuries to calculate YLDs. We considered results in the context of the Socio-demographic Index (SDI), a composite indicator of income per capita, years of schooling, and fertility rate in females younger than 25 years. Background: High-resolution estimates of HIV burden across space and time provide an important tool for tracking and monitoring the progress of prevention and control efforts and assist with improving the precision and efficiency of targeting efforts. We aimed to assess HIV incidence and HIV mortality for all second-level administrative units across sub-Saharan Africa. Methods: In this modelling study, we developed a framework that used the geographically specific HIV prevalence data collected in seroprevalence surveys and antenatal care clinics to train a model that estimates HIV incidence and mortality among individuals aged 15–49 years. We used a model-based geostatistical framework to estimate HIV prevalence at the second administrative level in 44 countries in sub-Saharan Africa for 2000–18 and sought data on the number of individuals on antiretroviral therapy (ART) by second-level administrative unit. We then modified the Estimation and Projection Package (EPP) to use these HIV prevalence and treatment estimates to estimate HIV incidence and mortality by second-level administrative unit. Findings: The estimates suggest substantial variation in HIV incidence and mortality rates both between and within countries in sub-Saharan Africa, with 15 countries having a ten-times or greater difference in estimated HIV incidence between the second-level administrative units with the lowest and highest estimated incidence levels. Across all 44 countries in 2018, HIV incidence ranged from 2·8 (95% uncertainty interval 2·1–3·8) in Mauritania to 1585·9 (1369·4–1824·8) cases per 100 000 people in Lesotho and HIV mortality ranged from 0·8 (0·7–0·9) in Mauritania to 676·5 (513·6–888·0) deaths per 100 000 people in Lesotho. The flood that occurred in Tangerang City still remains an issue that has not been resolved. The flood arose from Bogor through the Cisadane River. Cipabuaran Channel is one of the channels in the Sabi River watershed who often overflows. Sabi River itself is a tributary of the Cisadane River. In this study, the flood control simulation in Cipabuaran Channel was carried out using a 1D-2D numerical model. Simulation is carried out with 4 scenarios based on the design flow of the 10 year return period. For scenario 1, flood control is carried out using retention pond in the downstream of Cipabuaran Channel, scenario 2 is carried out by using two retention ponds in the upstream and downstream of Cipabuaran Channel, scenario 3 using retention ponds in the downstream of Cipabuaran Channel and bypass channel to Cisadane river, and scenario 4 performed by using retention pond in the downstream of Cipabuaran Channel and normalization on the Channel. Simulation results show the effectiveness of flood reduction for each scenario, in scenario 1 flood can be reduced by 9%, scenario 2 flood can be reduced by 32.52%, scenario 3 flood can be reduced by 77.34% and for scenario 4 flood can be reduced by 95%. From these results, it can be concluded that scenario 4 where flood control is carried out using a retention pond in the downstream of Cipabuaran Channel and normalizing the River can reduce flooding significantly.

The Body Appreciation Scale-2 (BAS-2) is a widely used measure of a core facet of the positive body image construct. However, extant research concerning measurement invariance of the BAS-2 across a large number of nations remains limited. Here, we utilised the Body Image in Nature (BINS) dataset - with data collected between 2020 and 2022 - to assess measurement invariance of the BAS-2 across 65 nations, 40 languages, gender identities, and age groups. Multi-group confirmatory factor analysis indicated that full scalar invariance was upheld across all nations, languages, gender identities, and age groups, suggesting that the unidimensional BAS-2 model has widespread applicability. There were large differences across nations and languages in latent body appreciation, while differences across gender identities and age groups were negligible-to-small. Additionally, greater body appreciation was significantly associated with higher life satisfaction, being single (versus being married or in a committed relationship), and greater rurality (versus urbanicity). Across a subset of nations where nation-level data were available, greater body appreciation was also significantly associated with greater cultural distance from the United States and greater relative income inequality. These findings suggest that the BAS-2 likely captures a near-universal conceptualisation of the body appreciation construct, which should facilitate further cross-cultural research.

Purpose: Hyperkyphosis, accentuated curvature of the thoracic spine, is often attributed to osteoporosis, yet its underlying pathophysiology is not well understood. Bone turnover markers (BTM) reflect the dynamic process of bone formation and resorption. This study examined the association between serum BTM levels and kyphosis in community-dwelling older adults. Methods: Between 2003 and 2006, 760 men and women in the Rancho Bernardo Study age 60 and older had blood drawn and kyphosis measured. Fasting serum was assayed for N-telopeptide (NTX) and procollagen type 1 n-terminal propeptide (P1NP), markers of bone resorption and formation, respectively. Participants requiring two or more 1.7 cm blocks under their head to achieve a neutral supine position were classified as having accentuated kyphosis. Analyses were stratified by sex and use of estrogen therapy (ET). Odds of accentuated kyphosis were calculated for each standard deviation increase in log-transformed BTM. Results: Mean age was 75 years. Overall, 51% of 341 non-ET using women, 41% of 111 ET-using women, and 75% of 308 men had accentuated kyphosis. In adjusted models, higher P1NP and NTX were associated with decreased odds of accentuated kyphosis in non-ET using women (P1NP: OR = 0.78 [95% CI, 0.58–0.92];NTX: OR = 0.68 [95% CI, 0.54–0.86]), but not in men or ET-using women (p > 0.05). Conclusions: The selective association of higher bone turnover with reduced odds of accentuated kyphosis in non-ET using women suggests that elevated BTM were associated with a lower likelihood of hyperkyphosis only in the low estrogen/high BTM environment characteristic of postmenopausal women who are not using ET. © 2016 The Authors
