AbstractObjectivesIntegration of time to physical activity (PA) patterns is novel and may be linked to health. Temporal PA patterns integrating time and PA were created to determine their association with health indicators: body mass index (BMI), waist circumference (WC), fasting plasma glucose, hemoglobin A1c, triglyceride, high-density lipoprotein cholesterol, total cholesterol, blood pressure, and chronic diseases obesity, diabetes, and metabolic syndrome in US adults 20–65 years. MethodsObjectively measured PA accelerometry data collected from the National Health and Nutrition Examination Survey 2003–2006 was used to pattern absolute PA intensity and time of activity from 1627 non-pregnant adults with one random valid PA day. Modified dynamic time warping and kernel k-means clustering grouped individuals representing temporal PA patterns. Multivariate regression models controlling for potential confounders and adjusting for multiple comparisons ( P < 0.05/6) determined associations between clusters and health outcomes. ResultsA cluster representing a temporal PA pattern with the highest average intensity peaking at 11:00 and tapering off through the day was associated with lower BMI ( P < 0.0001), WC ( P < 0.0001), and 65% lower odds of obesity compared to a cluster with lower average PA intensity peaking at 12:00 and tapering off through the day (95%CI: 0.2,0.5) and a cluster with the lowest average PA intensity with no distinct activity peaks (95%CI: 0.2,0.4). Another cluster with a temporal PA pattern with high average PA intensity peaking at 19:00 was associated with lower BMI ( P = 0.0003), WC ( P = 0.001), and 60% lower odds of obesity compared to a cluster with lower average PA intensity peaking at 12:00 and tapering off through the day (95%CI: 0.2,0.7) and a cluster with the lowest average PA intensity with no distinct peaks (95%CI: 0.2,0.6). Temporal Dietary Patterns Are Associated with Body Mass Index, Waist Circumference and Obesity

AbstractObjectivesThe integration of time with dietary patterns throughout a day, or temporal dietary patterns (TDP), have been linked with dietary quality but links to health outcomes are unknown. TDP were created with the objective to examine their association with health status indicators including body mass index (BMI), waist circumference (WC), fasting plasma glucose, hemoglobin A1c, triglyceride, high-density lipoprotein cholesterol, total cholesterol, blood pressure, and chronic diseases obesity, diabetes, and metabolic syndrome in US adults 20–65 years. MethodsThe first-day 24-hour dietary recall from 1627 non-pregnant US adult participants of the cross-sectional National Health and Nutrition Examination Survey 2003–2006 was used to determine energy intake (kcal), time of intake (min), and sequence of intake occasions throughout the 24-hour day. Modified dynamic time warping coupled with kernel k-means algorithm, clustered participants into four groups representing distinct TDP. Multivariate regression models determined associations between TDP clusters and all outcomes, controlling for potential confounders, energy misreporting, and adjusting for multiple comparisons and the complex survey design ( P < 0.05/6). ResultsThe cluster representing a TDP with proportionally equivalent average energy at three main eating occasions from 8:00 to 23:00 with peaks reaching 175 kcal at 9:00, 13:00, and 19:00, had statistically significant and clinically meaningful lower BMI ( P < 0.0001), WC ( P < 0.0001) and 75% lower odds of obesity compared to three other clusters representing distinct patterns of much higher average peak energy intake of 500 kcal at 13:00 (odds ratio (OR): 4.4; 95% confidence interval (CI)): 2.5, 7.9), 530 kcal at 18:00 (OR: 5.3; 95% CI: 2.8, 10.1), and 550 kcal at 20:00 (OR: 6.7; 95%CI: 3.9, 11.6). This paper explores the exposome concept and its role in elucidating the interplay between environmental exposures and human health. We introduce two key concepts critical for exposomics research. Firstly, we discuss the joint impact of genetics and environment on phenotypes, emphasizing the variance attributable to shared and nonshared environmental factors, underscoring the complexity of quantifying the exposome's influence on health outcomes. Secondly, we introduce the importance of advanced data-driven methods in large cohort studies for exposomic measurements. Here, we introduce the exposome-wide association study (ExWAS), an approach designed for systematic discovery of relationships between phenotypes and various exposures, identifying significant associations while controlling for multiple comparisons. We advocate for the standardized use of the term "exposome-wide association study, ExWAS," to facilitate clear communication and literature retrieval in this field. The paper aims to guide future health researchers in understanding and evaluating exposomic studies. Our discussion extends to emerging topics, such as FAIR Data Principles, biobanked healthcare datasets, and the functional exposome, outlining the future directions in exposomic research. This abstract provides a succinct overview of our comprehensive approach to understanding the complex dynamics of the exposome and its significant implications for human health.

HCV is a hepatotropic RNA virus recognized for its frequent virulence and fatality worldwide. Despite many vaccine development programs underway, researchers are on a quest for natural bioactive compounds due to their multivalent efficiencies against viral infections, considering which the current research aimed to figure out the target-specificity and therapeutic potentiality of α, β, and δ subunits of amyrin, as novel bioactive components against the HCV influx mechanism. Initially, the novelty of amyrin subunits was conducted from 203 pharmacophores, comparing their in-silico pharmacokinetic and pharmacodynamic profiles. Besides, the best active site of CD81 was determined following the quantum tunneling algorithm. The molecular dynamic simulation was conducted (100 ns) following the molecular docking steps to reveal the parameters- RMSD (Å); Cα; RMSF (Å); MolSA (Å); Rg (nm); PSA (Å); SASA (Å), and the MM-GBSA dG binding scores. Besides, molecular strings of CD81, along with the co-expressed genes, were classified, as responsible for encoding CD81-mediated protein clusters during HCV infection, resulting in the potentiality of amyrins as targeted prophylactics in HCV infection. Finally, in vivo profiling of the oxidative stress marker, liver-specific enzymes, and antioxidant markers was conducted in the DMN-induced mice model, where -amyrin scored the most significant values in all aspects.

Data collected by PLN staff on customer's complaints in the usage of electricity are very huge and accumulate. Previously, there was no information about these kinds of complaints in all sub-districts included inside the company, PT. PLN Binjai. On the other hand, each year company experienced problems to classify all complaints in order to obtain customers with information to later be used as a basis for making policy/decision. Data mining used clustering method and K-Means algorithm to classify the data so that important information is obtained from customer's complaints on the electricity usage, using variable: types of complaints, power using by consumer and consumer's region. Data was analyzed using Matlab to produce cluster centers and obtained a relationship between variables obtained with groups with high number of complaints. Results revealed that from 500 customers who have complained, cluster 1 had 218 complaints namely NT Fuse Putus with power used was 1300 watt and was located in Binjai. © Published under licence by IOP Publishing Ltd.

Despite substantial declines since 2000, lower respiratory infections (LRIs), diarrhoeal diseases, and malaria remain among the leading causes of nonfatal and fatal disease burden for children under 5 years of age (under 5), primarily in sub-Saharan Africa (SSA). The spatial burden of each of these diseases has been estimated subnationally across SSA, yet no prior analyses have examined the pattern of their combined burden. Here we synthesise subnational estimates of the burden of LRIs, diarrhoea, and malaria in children under-5 from 2000 to 2017 for 43 sub-Saharan countries. Some units faced a relatively equal burden from each of the three diseases, while others had one or two dominant sources of unit-level burden, with no consistent pattern geographically across the entire subcontinent. Using a subnational counterfactual analysis, we show that nearly 300 million DALYs could have been averted since 2000 by raising all units to their national average. Our findings are directly relevant for decision-makers in determining which and targeting where the most appropriate interventions are for increasing child survival. © 2022, The Author(s).

Background: Posttraumatic stress disorder (PTSD) is a highly disabling condition associated with alterations in multiple neurobiological systems, including increases in inflammatory and sympathetic function, responsible for maintenance of symptoms. Treatment options including medications and psychotherapies have limitations. We previously showed that transcutaneous Vagus Nerve Stimulation (tcVNS) blocks inflammatory (interleukin (IL)-6) responses to stress in PTSD. The purpose of this study was to assess the effects of tcVNS on PTSD symptoms and inflammatory responses to stress. Methods: Twenty patients with PTSD were randomized to double blind active tcVNS (N=9) or sham (N=11) stimulation in conjunction with exposure to personalized traumatic scripts immediately followed by active or sham tcVNS and measurement of IL-6 and other biomarkers of inflammation. Patients then self administered active or sham tcVNS twice daily for three months. PTSD symptoms were measured with the PTSD Checklist (PCL) and the Clinician Administered PTSD Scale (CAPS), clinical improvement with the Clinical Global Index (CGI) and anxiety with the Hamilton Anxiety Scale (Ham-A) at baseline and one-month intervals followed by a repeat of measurement of biomarkers with traumatic scripts. After three months patients self treated with twice daily open label active tcVNS for another three months followed by assessment with the CGI. Results: Traumatic scripts increased IL-6 in PTSD patients, an effect that was blocked by tcVNS (p<.05). Active tcVNS treatment for three months resulted in a 31% greater reduction in PTSD symptoms compared to sham treatment as measured by the PCL (p=0.013) as well as hyperarousal symptoms and somatic anxiety measured with the Ham-A p<0.05). IL-6 increased from baseline in sham but not tcVNS. Open label tcVNS resulted in improvements measured with the CGI compared to the sham treatment period p<0.05). What is the best way to deploy solar energy to maximize clean energy growth while equitably sharing benefits? A promising model is community shared solar, which enables energy consumers to purchase shares of electricity generated in an offsite project. Noting how different states and utilities have approached program design, we explore how design decisions affect access to solar and the equity of cost and benefit sharing. We conclude with a set of questions for future research. © 2017 Elsevier Inc.

The plasmonic antenna probe is designed using a silver bar embedded in a modified add-drop filter within a Mach Zehnder interferometer (MZI). The Rabi antenna is formed when the space-time control has reached the two-level system oscillation, which can be applied as a quantum brain-sensingprobe. A photonic neural network is designed using brain-Rabi antenna communication, wherein the transmission is connected by the neurons. The communication signals are transferred by the electron spins (up-down) and tunable Rabi frequency. The hidden variables and deep brain signals can be obtained by the external detection. We have developed a Rabi antenna via simulation using computer Simulation Technology (CST) software. We have developed a communication device using the Optiwave program (OptiFDTD). The output signal is plotted by the Matlab program using parameters from the results of the OptiFDTD simulation. The proposed antenna resonates in the frequency range of 192THz-202THz with a maximum gain of 22.4dBi. The sensor sensitivity is calculated along withelectron spin results and applied to form the quantum-brain interconnection. Moreover, an intelligent machine learning algorithm is proposed for the identification of high-quality transmission and predicting the behavior of the transmission in near future. In rural areas where access to a power grid often is not available off-grid systems are only solution for a power supply of the consumers. For the micro scale loads (such as cottages) diesel or gasoline generators are usually used. To avoid usage of fossil fuels and to avoid greenhouse gases emission renewable energy sources are very suitable for off-grid systems. In this paper optimization of the renewable energy sources based off-grid system for usage in rural areas is presented. Optimization is performed for different scenarios of capacity shortage value. Goal of the optimization is to find a system with minimum net present cost (NPC). Influence of the different operation and maintenance cost scenarios on the NPC is considered. Two cases in the paper are considered. In the first case optimization of the off-grid system is performed for the micro scale consumers such as houses, cottages etc. In the second case optimization of the off-grid system is performed for the small-scale consumers in rural areas such as small villages which can help in set up of small energy communities. Optimizations are performed using HOMER software.
