Acute brain diseases such as acute stroke and transit ischemic attacks are the leading causes of mortality and morbidity worldwide, responsible for 9% of total death every year. ‘Time is brain’ is a widely accepted concept in acute cerebrovascular disease treatment. Efficient and accurate computational framework for hemodynamic parameters estimation can save critical time for thrombolytic therapy. Meanwhile the high level of accumulated radiation dosage due to continuous image acquisition in CT perfusion (CTP) raised concerns on patient safety and public health. However, low-radiation will lead to increased noise and artifacts which require more sophisticated and time-consuming algorithms for robust estimation. We propose a novel efficient framework using tensor totalvariation (TTV) regularization to achieve both high efficiency and accuracy in deconvolution for low-dose CTP. The method reduces the necessary radiation dose to only 8% of the original level and outperforms the state-of-art algorithms with estimation error reduced by 40%. It also corrects over-estimation of cerebral blood flow (CBF) and under-estimation of mean transit time (MTT), at both normal and reduced sampling rate. An efficient computational algorithm is proposed to find the solution with fast convergence. © Springer International Publishing Switzerland 2014.

This paper is a work in progress that examines current consumer engagement with eHealth information through Smartphones or tablets. We focus on three activity types: seeking, posting and 'other' engagement activity and compare two age groups, 25-40s and over 40-50s. Findings show that around 30% of the younger age group is engaging with Government and other Health providers' websites, receiving eHealth emails, and reading other people's comments about health related issues in online discussion groups/websites/blog. Approximately 20% engage with Government and other Health providers social media, watch or listen to audio or video podcasts. For the older age group, their most active engagement with eHealth information is in the seeking category through Government or other health websites (approximately 15%), and less than 10% for social media sites. Their posting activity is less than 5%. Other activities show that less than 15% of the older age group engages through receiving emails and reading blogs, less than 10% watch or listen to podcasts, and their online consulting activity is less than 7%. We note that scores are low for both groups in terms of engaging with eHealth information through Twitter. © 2013 IEEE.

People take regular medical examinations mostly not for discovering diseases but for having a peace of mind regarding their health status. Therefore, it is important to give them an overall feedback with respect to all the health indicators that have been ranked against the whole population. In this paper, we propose a framework of mining Personal Health Index (PHI) from a large and comprehensive geriatric medical examination (GME) dataset. We define PHI as an overall score of personal health status based on a complement probability of health risks. The health risks are calculated using the information from the cause of death (COD) dataset that is linked to the GME dataset. Especially, the highest health risk is revealed in the cases of people who had been taking GME for some years and then passed away for medical reasons. The proposed framework consists of methods in data pre-processing, feature extraction and selection, and model selection. The effectiveness of the proposed framework is validated by a set of comprehensive experiments based on the records of 102,258 participants. As the first of this kind, our work provides a baseline for further research.

Consumer participation in quality improvements for chronic disease care: Development and evaluation of an interactive patient-centered survey to identify preferred service initiatives

Background: With increasing attention given to the quality of chronic disease care, a measurement approach that empowers consumers to participate in improving quality of care and enables health services to systematically introduce patient-centered initiatives is needed. A Web-based survey with complex adaptive questioning and interactive survey items would allow consumers to easily identify and prioritize detailed service initiatives. Objective: The aim was to develop and test a Web-based survey capable of identifying and prioritizing patient-centered initiatives in chronic disease outpatient services. Testing included (1) test-retest reliability, (2) patient-perceived acceptability of the survey content and delivery mode, and (3) average completion time, completion rates, and Flesch-Kincaid reading score. Methods: In Phase I, the Web-based Consumer Preferences Survey was developed based on a structured literature review and iterative feedback from expert groups of service providers and consumers. The touchscreen survey contained 23 general initiatives, 110 specific initiatives available through adaptive questioning, and a relative prioritization exercise. In Phase II, a pilot study was conducted within 4 outpatient clinics to evaluate the reliability properties, patient-perceived acceptability, and feasibility of the survey. Eligible participants were approached to complete the survey while waiting for an appointment or receiving intravenous therapy. The age and gender of nonconsenters was estimated to ascertain consent bias. Participants with a subsequent appointment within 14 days were asked to complete the survey for a second time. Results: A total of 741 of 1042 individuals consented to participate (71.11% consent), 529 of 741 completed all survey content (78.9% completion), and 39 of 68 completed the test-retest component. This paper proposes that a reliable and efficient integration of distributed generators (DGs) needs strategic planning and operation for distribution systems. The paper illustrates that today’s policies such as plug-and-play and those outlined under IEEE standard 1547 cannot support a large penetration of DGs sending power to the grid. The paper explores possible challenges and potential opportunities in integrating a large penetration of DGs. The key point of the paper is designing a model-based adaptive policy to link the methods engineers use to policy design for achieving efficient and reliable performance. Two strategies that could be used to achieve this are outlined and discussed: enhanced plug-and-play and third party siting and operating review.

低效的溶酶体降解是多种脑部疾病发展的中心环节,但其具体机制及参与的细胞种类仍不清楚。既往研究显示,星形胶质细胞大量吞噬死亡的细胞,但将这些吞噬的物质储存在细胞内而不是进行降解。本研究对星形胶质细胞降解消化功能减弱的原因进行研究,并探索增强星形胶质细胞降解消化功能的方法。结果显示,长期存在于吞噬泡周围的肌动环阻碍了溶酶体的吞噬溶解作用。此外,Rab27a蛋白可以通过Nox2减少溶酶体的酸化,而星形胶质细胞高表达Rab27a蛋白,阻碍抗原呈递。本研究还发现,Nox2与星形胶质细胞摄取的物质共定位,且表达主要组织相容性复合体II,提示这可能影响星形胶质细胞内的抗原呈递。使用酸化纳米粒子对星形胶质细胞溶酶体进行长时间的酸化处理,能增加星形胶质细胞对所摄取物质和死亡细胞的消化。但随着酸化时间的延长,星形胶质细胞的消化能力再次回复到较低水平,提示随着酸化作用的增强,细胞的对抗酸化的通路也相应增强了。
