Consumer participation in quality improvements for chronic disease care: Development and evaluation of an interactive patient-centered survey to identify preferred service initiatives

Background: With increasing attention given to the quality of chronic disease care, a measurement approach that empowers consumers to participate in improving quality of care and enables health services to systematically introduce patient-centered initiatives is needed. A Web-based survey with complex adaptive questioning and interactive survey items would allow consumers to easily identify and prioritize detailed service initiatives. Objective: The aim was to develop and test a Web-based survey capable of identifying and prioritizing patient-centered initiatives in chronic disease outpatient services. Testing included (1) test-retest reliability, (2) patient-perceived acceptability of the survey content and delivery mode, and (3) average completion time, completion rates, and Flesch-Kincaid reading score. Methods: In Phase I, the Web-based Consumer Preferences Survey was developed based on a structured literature review and iterative feedback from expert groups of service providers and consumers. The touchscreen survey contained 23 general initiatives, 110 specific initiatives available through adaptive questioning, and a relative prioritization exercise. In Phase II, a pilot study was conducted within 4 outpatient clinics to evaluate the reliability properties, patient-perceived acceptability, and feasibility of the survey. Eligible participants were approached to complete the survey while waiting for an appointment or receiving intravenous therapy. The age and gender of nonconsenters was estimated to ascertain consent bias. Participants with a subsequent appointment within 14 days were asked to complete the survey for a second time. Results: A total of 741 of 1042 individuals consented to participate (71.11% consent), 529 of 741 completed all survey content (78.9% completion), and 39 of 68 completed the test-retest component. This paper proposes that a reliable and efficient integration of distributed generators (DGs) needs strategic planning and operation for distribution systems. The paper illustrates that today’s policies such as plug-and-play and those outlined under IEEE standard 1547 cannot support a large penetration of DGs sending power to the grid. The paper explores possible challenges and potential opportunities in integrating a large penetration of DGs. The key point of the paper is designing a model-based adaptive policy to link the methods engineers use to policy design for achieving efficient and reliable performance. Two strategies that could be used to achieve this are outlined and discussed: enhanced plug-and-play and third party siting and operating review.

The aim of this work is to create a framework of definitions and notations to hide part of a Petri net, facing a possible delivery, maintaining the privacy of the critical, secret, or complex parts of the system. 低效的溶酶体降解是多种脑部疾病发展的中心环节,但其具体机制及参与的细胞种类仍不清楚。既往研究显示,星形胶质细胞大量吞噬死亡的细胞,但将这些吞噬的物质储存在细胞内而不是进行降解。本研究对星形胶质细胞降解消化功能减弱的原因进行研究,并探索增强星形胶质细胞降解消化功能的方法。结果显示,长期存在于吞噬泡周围的肌动环阻碍了溶酶体的吞噬溶解作用。此外,Rab27a蛋白可以通过Nox2减少溶酶体的酸化,而星形胶质细胞高表达Rab27a蛋白,阻碍抗原呈递。本研究还发现,Nox2与星形胶质细胞摄取的物质共定位,且表达主要组织相容性复合体II,提示这可能影响星形胶质细胞内的抗原呈递。使用酸化纳米粒子对星形胶质细胞溶酶体进行长时间的酸化处理,能增加星形胶质细胞对所摄取物质和死亡细胞的消化。但随着酸化时间的延长,星形胶质细胞的消化能力再次回复到较低水平,提示随着酸化作用的增强,细胞的对抗酸化的通路也相应增强了。

Social force model is a widely-used crowd model that simulates pedestrian dynamics based on Newtonian mechanics. It combines physical forces (caused by body compression) with psychological repulsive forces (caused by the pedestrians' inner desire to keep a distance from others or walls) to represent the intentional collision avoidance mechanism. The model can reproduce many collective phenomena (e.g., lane formation, and the 'faster is slower' effect). However, there are still things unrealistic in the original social force model. For example, we observe that when pedestrians approach a wall with different velocities, the repulsive forces exerted on them depend only on the distance between them and the wall. As a result, a pedestrian with a higher velocity may experience a stronger collision with the wall than others. However, in reality, he/she is able to predict this stronger collision, and would slow down much earlier than others. This paper introduces a modified social force model in which the pedestrians predict possible collisions using the information of not only positions, but also the velocities: pedestrians predict positions in the next time step based on the current positions and velocities, and the psychological repulsive forces on pedestrians are determined by the predicted positions instead of current positions. With this modified collision avoidance mechanism, pedestrians behave more realistic. Simulation results show that the modified model reduces the oscillation significantly and is more realistic. We also study the relationship between the total evacuation time and the time step of prediction in a specified scenario. © 2013 IEEE.

A new technology known as Virtual Traffic Lights (VTL) was recently proposed as a self-organizing new paradigm for traffic management. This new technology uses the vehicle-to-vehicle (V2V) communications as its premise. VTL can revolutionize traffic management in urban areas as it can substantially reduce commute time of urban workers, increase productivity, and lead to a greener environment. In a VTL environment, vehicles self-organize to elect a leader which serves as a virtual traffic light to decide the right of way at that intersection, thus replacing the current physical traffic lights. Implementing VTL technology with partial penetration, however, is an outstanding issue that needs to be addressed. This paper addresses this issue by proposing a co-existence model whereby VTL equipped vehicles can co-exist with vehicles that do not have VTL. Simulation results show that the transition model proposed here could provide drivers with strong incentives to adopt the VTL technology.

The urban mobility report and congested corridors report by Texas Transportation Institute (TTI) are the best known congestion studies in the United States. The data source TTI uses, however, limits the accuracy and reliability of the reported measures. The low resolution speed dataset and arbitrarily distributed volume profile could lead to considerable errors for both reliability and total congestion measures. In this paper, we evaluate the degree of measuring improvement by applying our high resolution traffic speed and volume data collected from the entire Los Angeles County highways. Our study shows the TTI report underestimates peak hour unreliability and overestimates total congestion. The estimated planning time indexes and buffer indexes on the 28 congested corridors studied are 18 percent and 51 percent greater than the TTI report respectively. Annual delays and congestion costs are only two-thirds of the TTI estimates. This case study highlights the need to improve the TTI estimates for it to be a best decision basis for planners, drivers and researchers. It is also found the keys to improve these congestion measures are a high temporal resolution speed dataset and a nationwide real-world traffic volume dataset.

Allowing an improved and ubiquitous sharing of the precious radio spectrum among providers has been identified as a key goal by many governmental agencies. In this paper, we propose a novel approach to foster more sharing of the radio spectrum via the use of regulatory power. We develop a spectrum subsidization scheme in which providers are incentivized by cheaper spectrum allocation fees from the regulators in return for “proof-of-sharing”. The providers are offered discounted spectrum bands, potentially at different locations, but will be asked to provide coverage to users that are not subscribed to them so as to maintain their subsidy incentives from the government. We introduce a game-theoretic framework to analyze the advantages and drawbacks of such a subsidy scheme and explore potential regulatory guidelines.
