Lung adenocarcinoma is the most common type of lung cancer. Known risk variants explain only a small fraction of lung adenocarcinoma heritability. Here, we conducted a two-stage genome-wide association study of lung adenocarcinoma of East Asian ancestry (21,658 cases and 150,676 controls; 54.5% never-smokers) and identified 12 novel susceptibility variants, bringing the total number to 28 at 25 independent loci. Transcriptome-wide association analyses together with colocalization studies using a Taiwanese lung expression quantitative trait loci dataset (n = 115) identified novel candidate genes, including FADS1 at 11q12 and ELF5 at 11p13. In a multi-ancestry meta-analysis of East Asian and European studies, four loci were identified at 2p11, 4q32, 16q23, and 18q12. At the same time, most of our findings in East Asian populations showed no evidence of association in European populations. In our studies drawn from East Asian populations, a polygenic risk score based on the 25 loci had a stronger association in never-smokers vs. individuals with a history of smoking (P = 0.0058). These findings provide new insights into the etiology of lung adenocarcinoma in individuals from East Asian populations, which could be important in developing translational applications.

Background: High-resolution estimates of HIV burden across space and time provide an important tool for tracking and monitoring the progress of prevention and control efforts and assist with improving the precision and efficiency of targeting efforts. We aimed to assess HIV incidence and HIV mortality for all second-level administrative units across sub-Saharan Africa. Methods: In this modelling study, we developed a framework that used the geographically specific HIV prevalence data collected in seroprevalence surveys and antenatal care clinics to train a model that estimates HIV incidence and mortality among individuals aged 15–49 years. We used a model-based geostatistical framework to estimate HIV prevalence at the second administrative level in 44 countries in sub-Saharan Africa for 2000–18 and sought data on the number of individuals on antiretroviral therapy (ART) by second-level administrative unit. We then modified the Estimation and Projection Package (EPP) to use these HIV prevalence and treatment estimates to estimate HIV incidence and mortality by second-level administrative unit. Findings: The estimates suggest substantial variation in HIV incidence and mortality rates both between and within countries in sub-Saharan Africa, with 15 countries having a ten-times or greater difference in estimated HIV incidence between the second-level administrative units with the lowest and highest estimated incidence levels. Across all 44 countries in 2018, HIV incidence ranged from 2·8 (95% uncertainty interval 2·1–3·8) in Mauritania to 1585·9 (1369·4–1824·8) cases per 100 000 people in Lesotho and HIV mortality ranged from 0·8 (0·7–0·9) in Mauritania to 676·5 (513·6–888·0) deaths per 100 000 people in Lesotho. Background: The sustainable development goals (SDGs) aim to end HIV/AIDS as a public health threat by 2030. Understanding the current state of the HIV epidemic and its change over time is essential to this effort. This study assesses the current sex-specific HIV burden in 204 countries and territories and measures progress in the control of the epidemic. Methods: To estimate age-specific and sex-specific trends in 48 of 204 countries, we extended the Estimation and Projection Package Age-Sex Model to also implement the spectrum paediatric model. We used this model in cases where age and sex specific HIV-seroprevalence surveys and antenatal care-clinic sentinel surveillance data were available. For the remaining 156 of 204 locations, we developed a cohort-incidence bias adjustment to derive incidence as a function of cause-of-death data from vital registration systems. The incidence was input to a custom Spectrum model. To assess progress, we measured the percentage change in incident cases and deaths between 2010 and 2019 (threshold >75% decline), the ratio of incident cases to number of people living with HIV (incidence-to-prevalence ratio threshold <0·03), and the ratio of incident cases to deaths (incidence-to-mortality ratio threshold <1·0). Findings: In 2019, there were 36·8 million (95% uncertainty interval [UI] 35·1–38·9) people living with HIV worldwide. There were 0·84 males (95% UI 0·78–0·91) per female living with HIV in 2019, 0·99 male infections (0·91–1·10) for every female infection, and 1·02 male deaths (0·95–1·10) per female death. Global progress in incident cases and deaths between 2010 and 2019 was driven by sub-Saharan Africa (with a 28·52% decrease in incident cases, 95% UI 19·58–35·43, and a 39·66% decrease in deaths, 36·49–42·36). Chronic care manages long-term, progressive conditions, while acute care addresses short-term conditions. Chronic conditions increasingly strain health systems, which are often unprepared for these demands. This study examines the burden of conditions requiring acute versus chronic care, including sequelae. Conditions and sequelae from the Global Burden of Diseases Study 2019 were classified into acute or chronic care categories. Data were analysed by age, sex, and socio-demographic index, presenting total numbers and contributions to burden metrics such as Disability-Adjusted Life Years (DALYs), Years Lived with Disability (YLD), and Years of Life Lost (YLL). Approximately 68% of DALYs were attributed to chronic care, while 27% were due to acute care. Chronic care needs increased with age, representing 86% of YLDs and 71% of YLLs, and accounting for 93% of YLDs from sequelae. These findings highlight that chronic care needs far exceed acute care needs globally, necessitating health systems to adapt accordingly. © 2025. The Author(s).

To investigate the genetic basis of type 2 diabetes (T2D) to high resolution, the GoT2D and T2D-GENES consortia catalogued variation from whole-genome sequencing of 2,657 European individuals and exome sequencing of 12,940 individuals of multiple ancestries. Over 27M SNPs, indels, and structural variants were identified, including 99% of low-frequency (minor allele frequency [MAF] 0.1-5%) non-coding variants in the whole-genome sequenced individuals and 99.7% of low-frequency coding variants in the whole-exome sequenced individuals. Each variant was tested for association with T2D in the sequenced individuals, and, to increase power, most were tested in larger numbers of individuals (>80% of low-frequency coding variants in ~82 K Europeans via the exome chip, and ~90% of low-frequency non-coding variants in ~44 K Europeans via genotype imputation). The variants, genotypes, and association statistics from these analyses provide the largest reference to date of human genetic information relevant to T2D, for use in activities such as T2D-focused genotype imputation, functional characterization of variants or genes, and other novel analyses to detect associations between sequence variation and T2D.

Impact of asthma 101 training on level of nursing students' knowledge

The Asthma 101 training, developed by the American Lung Association, was offered in 2009 to 93 senior bachelor of science in nursing (BSN) students. It was offered again in 2010 to 65 senior BSN students enrolled at the University of Texas Pan American in Edinburg, Texas. The training included both a pretest and posttest to evaluate changes in level of knowledge. Nursing students who were trained in 2009 participated in an asthma study offering asthma education to local elementary and middle schools students and their parents. The goal of Asthma 101 training was to educate nursing students so that they could provide general information about asthma to elementary school children and their parents, which could be done in a variety of settings such as health fairs and in classrooms. Test results for those who received the 2009 training showed an improvement in their knowledge about asthma. More than 95% of participants were very or somewhat satisfied with the Asthma 101 training. Test results in 2010 showed a significant improvement in participants' knowledge in 2 areas: taking asthma medications to prevent an asthma episode (P =.001) and occurrence of asthma symptoms (P =.016). Two questions of statistical significance related to the use of medications and asthma episode demonstrated that nursing students had increased their knowledge in these areas. These findings suggest that health professionals can acquire additional knowledge from a brief yet focused training, such asthe Asthma 101 training outlined in this article © 2010 The Author(s).

Each stage of the sewage treatment process emits odor causing compounds and these compounds may vary from one location in a sewage treatment works to another. In order to determine the boundaries of legal standards, reliable and efficient odor measurement methods need to be defined. An electronic NOSE equipped with 12 different polypyrrole sensors is used for the purpose of characterizing sewage odors. Samples collected at different locations of a WWTP were classified using a fuzzy clustering technique and a neural network trained with a back-propagation algorithm. © Sringer-Verlag Berlin Heidelberg 2005.

An Inverse Approach to the Analysis of Uncertainty in Models of environmental Systems

An inverse methodology that integrates qualitative and quantitative aspects of uncertainty in environmental modeling is presented. The methodology, RIMME (Random-search Inverse Methodology for Model Evaluation), comprises three Monte Carlo procedures: (i) Regionalized Sensitivity Analysis (RSA); (ii) Tree-Structured Density Estimation (TSDE); and (iii) Uniform Covering by Probabilistic Rejection (UCPR). Unlike conventional direct predictive approaches, inverse methods work backwards to identify attributes of the model, and the corresponding real system, that are critical to attaining a prescribed endpoint. RIMME is capable of integrating scientific uncertainties, in the model and empirical data, with non-standard, qualitative forms of uncertainty, such as the value-laden policy and stakeholder issues that feature prominently in contemporary environmental assessments. RIMME is applied to a case study of Lake Lanier, Georgia (USA), a key resource whose water quality and ecological integrity is perceived by society to now be threatened by rapid urbanization within and around its watershed. Results indicate that RIMME provides an effective bridge across the gap between traditional science and the now emerging post-normal science era.

Simulation models are commonly employed for predicting both the impacts of natural stressors and the outcomes of anthropogenic actions on the environment. Such model-based forecasting depicts a forward approach to environmental systems analysis. This paper presents a complementary backward approach that uses prescribed future scenarios to identify the dominant components of the modeled system. In particular, two Monte Carlo methods for uncertainty analysis – namely, Regionalized Sensitivity Analysis (RSA) and Tree-Structured Density Estimation (TSDE) – are applied in detecting structural change between observed past and speculated future ecological behavior in a small southeastern US Piedmont reservoir (Lake Oglethorpe, GA). Results indicate that sediment-water-nutrient interactions are key elements of structural change that distinguish between the past and future scenarios analyzed. General implications for predicting structural change in environmental systems are discussed. © 2003 The Authors.
